Several physiological functions of adenosine (Ado) appear to be mediated by four G protein-coupled Ado receptors. Ado is produced extracellularly from the catabolism of the excreted ATP, or intracellularly from AMP, and then released through its transporter. High level of intracellular Ado occurs only at low energy charge, as an intermediate of ATP breakdown, leading to hypoxanthine production. AMP, the direct precursor of Ado, is now considered as an important stress signal inside cell triggering metabolic regulation through activation of a specific AMP-dependent protein kinase. Intracellular…

The nucleoside adenosine is used clinically for the treatment of paroxysmal atrioventricular junctional tachycardia. Surgically oriented experimental studies indicate an improvement of myocardial protection when adenosine is administered as an adjunct to cardioplegic protection. This regards both postischemic functional recovery and the regeneration of myocardial energy-rich phosphates. However, as yet it remains to be determined which of the various effects of adenosine is truly protective. Likewise it is still unclear if the beneficial effects of "ischemic preconditioning" may be simulated b…

Adenosine, an endogenous nucleoside has been recently approved for use in the treatment of paroxysmal supraventricular tachycardia. Adenosine is nearly 100% effective in terminating tachycardia in which the atrioventricular node forms part of the reentrant circuit. Although most ventricular tachycardias are insensitive to adenosine, this substance is effective in ventricular tachycardia induced by catecholamines or exercise. An intravenous bolus dose of 6 mg is the initial dose. If no effect is noted a further bolus of 12 mg can be given. The most common side effects are dyspnea, chest pressur…

The results, briefly summarized above, indicate that adenosine could be a physiologically important modulator of several aspects of cardiovascular regulation. Most cells are equipped with adenosine receptors. These receptors are of at least two subtypes which can be defined by the relative agonist potency. At these adenosine receptors, methylxanthines, including caffeine and theophylline, act as competitive antagonists. The role of adenosine antagonism, as a mechanism behind the cardiovascular effects of these xanthines, was recently reviewed (Fredholm, 1984). The concentrations of adenosine a…

Nearly 90 years ago, Drury and Szent-Györgyi revealed that adenosine produced profound hypotension and bradycardia, and it affected kidney function in mammals [1]. [...].

Adenosine is an endogenous nucleoside with a short half-life that regulates many physiological functions involving the heart and cardiovascular system. Among the cardioprotective properties of adenosine are its ability to improve cholesterol homeostasis, impact platelet aggregation and inhibit the inflammatory response. Through modulation of forward and reverse cholesterol transport pathways, adenosine can improve cholesterol balance and thereby protect macrophages from lipid overload and foam cell transformation. The function of adenosine is controlled through four G-protein coupled receptors…

Adenosine (Adenocard) is an endogenous purine nucleoside that has been approved recently for intravenous treatment of paroxysmal supraventricular tachycardia. With a serum half-life of 10 seconds, reported side effects including facial flushing, dyspnea, and chest pressure are common, but very transient. An elderly woman who received adenosine for paroxysmal supraventricular tachycardia had a prolonged anaphylactoid reaction that required pharmacological treatment. This is the first reported case of a prolonged anaphylactoid reaction to adenosine.

Adenosine (Adenocard) is a new injectable antiarrhythmic agent indicated for the acute treatment of paroxysmal supraventricular tachycardia (PSVT) including PSVT associated with the Wolff-Parkinson-White syndrome. Many physicians are not yet familiar with its actions, indications and side effects.

Since it has been recognized that adenosine had potent electrophysiologic effects, investigators in several laboratories have shown that it is useful for terminating episodes of PSVT that require AV nodal conduction for continuation and that it may be a useful diagnostic maneuver to expose the atrial mechanism in patients whose arrhythmias are not terminated. Preliminary experience suggests that acute intravenous use of adenosine is both safe and highly efficacious, but data systematically comparing it to other current modes of therapy are not yet available. Future goals of research will inclu…

The authors describe a case of a 71 year old man who suffered from chronic lung disease and had atrial tachycardias. 18 mg adenosine terminated his paroxysmal supraventricular tachycardia. The Holter monitoring showed that the tachycardia began at the late diastole with normal PQ interval, suggesting that an ectopic atrial tachycardia was terminated. They propose that adenosine is a "new" drug for termination of some form of atrial tachycardia.

Limited data suggest that adenosine termination of atrial tachycardia is uncommon. To investigate further the effect of adenosine on atrial tachycardia, adenosine (6-12 mg) was administered during sustained atrial tachycardia in 17 patients. All patients underwent electrophysiological study to exclude other mechanisms of supraventricular tachycardia. Mean patient age was 51 +/- 20 years (range 18-82 years). Seven patients had no structural heart disease. The mean atrial tachycardia cycle length was 390 +/- 80 msecs (range 260-580). Sustained atrial tachycardia was induced with atrial extrastim…
