[Robotic adrenalectomy-current evidence].

Laparoscopic adrenalectomy is worldwide currently considered the gold standard technique for patients suffering from benign lesions. There has been an increasing number of robotic-assisted adrenalectomies in the last decade due to the systematic further development of minimally invasive surgery resulting in the implementation of robot-assisted surgery. Interestingly, the available studies show some perioperative advantages for robotic surgery but overall the current evidence is weak and there are economic aspects that are not negligible. Therefore, further high-quality studies about robotic-as…

Robot-Assisted Laparoscopic Adrenalectomy.

Adrenalectomies are increasingly performed using minimally invasive approaches. The widespread adoption of robot-assisted laparoscopy for other urologic surgeries has dramatically increased the popularity of this approach for adrenal surgery.

[Indications for conventional adrenalectomy].

INTRODUCTION: Conventional adrenalectomy still plays an important role, even in the era of minimally invasive endocrine surgery. It was the aim of our study to analyse the indications for conventional adrenalectomy in our own patients since the introduction of the minimally invasive technique in the year 1994 - laparoscopically and retroperitoneoscopically. PATIENTS AND METHODS: Between January 1994 and September 2006, a total of 412 adrenalectomies were performed in 380 patients. Out of these, 106 operations (25.7 %) were carried out conventionally in 98 patients, and 306 operations (74.3 %)…

Laparoscopic adrenalectomy for malignancy.

Laparoscopic adrenalectomy for primary malignancies and tumors metastatic to the adrenal is controversial. Most studies demonstrate that results of laparoscopic adrenalectomy for malignant lesions are similar to those of open adrenalectomy, without its morbidity. The results of laparoscopic adrenalectomy for tumor metastases suggest that it may benefit patients who have a metachronous metastasis from any of a variety of primary tumors. Selective laparoscopic adrenalectomy for potentially malignant tumors requires seeking signs of local invasion, lymphadenopathy, or distant metastasis; there ar…

Laparoscopic adrenalectomy for pheochromocytoma.

aparoscopic adrenalectomy is the standard for most surgical adrenal diseases. The aim of this study was to evaluate the safety and effectiveness of laparoscopic adrenalectomy for patients with pheochromocytoma. The medical records of 39 consecutive patients who underwent laparoscopic adrenalectomy for pheochromocytomas from 1994 to 2000 at the University of California-San Francisco were reviewed. Three groups of patients were identified. The first group comprised 17 patients with classic symptoms and signs of pheochromocytoma. The second group comprised 17 patients who had minimal symptoms and…

Laparoscopic adrenalectomy for cancer.

We will review the literature on the operative techniques and patient outcomes of laparoscopic adrenalectomy for cancer. Further, in our own study, an analysis of the preoperative assessment, operative, and hospital course, and postoperative follow-up was performed on all patients undergoing a laparoscopic adrenalectomy for cancer or metastasis from October 1996 through February 1998. Twelve laparoscopic resections were performed in 11 patients. There were six males and five females with an average age of 62 years (range, 40 to 79). The mean American Society of Anesthesiologists (ASA) score wa…

Adrenalectomy in adrenal hypertension.

Between 1965 and 1983, a total of 138 patients with endocrine hypertension were treated at the Münster Surgical Clinic. 71 catecholamine-producing tumors were removed in 56 patients, 60 patients had primary aldosteronism; 32 patients with Cushing's syndrome received uni- or bilateral adrenalectomy. The operative access to the adrenals was by anterior abdominal incision which we consider superior to lateral or transthoracic approaches. 35 complications occurred. The operative management of these forms of secondary hypertension, with its low overall rate of complications, is currently seen as th…

The case for laparoscopic adrenalectomy.

PURPOSE: The current status of laparoscopic adrenal surgery was assessed. MATERIALS AND METHODS: A current MEDLINE search revealed 308 articles pertaining to laparoscopic surgery of the adrenal gland. Based on this literature review laparoscopic surgical anatomy, current indications and contraindications, and laparoscopic techniques were identified. The role of laparoscopic surgery for various adrenal disorders, including aldosteroma, pheochromocytoma, Cushing's syndrome, incidentaloma and adrenal cancer, were evaluated. Studies specifically comparing open versus laparoscopic adrenalectomy an…

Treatment Outcomes of Robotic Adrenalectomy Compared to Laparoscopic Adrenalectomy: A Propensity Score Matching Study.

INTRODUCTION: Laparoscopic adrenalectomy is a common procedure for the treatment of adrenal tumors. However, the recent inclusion of robotic adrenalectomy in the list of insurance-covered procedures in Japan since April 2022 has raised questions regarding its broader clinical utility. This study aimed to elucidate the treatment outcomes of robotic adrenalectomy and conduct a comprehensive analysis comparing laparoscopic and robotic adrenalectomies. METHODS: The baseline characteristics of patients and perioperative outcomes between laparoscopic and robotic adrenalectomies performed at Hiroshi…

[Open adrenalectomy for adrenal metastases from lung cancer--usefulness and problem of laparoscopic adrenalectomy].

We examined the outcome of open adrenalectomy performed at our hospital to determine the effectiveness and problems of laparoscopic adrenalectomy for adrenal metastases from lung cancer. Between January 2001 and June 2004, eight open adrenalectomies were performed on six patients with adrenal metastases from lung cancer. Surrounding adhesion was observed in five of the eight cases (63%). The recurrence period was 17.8 months and 5.3 months in the cases with and without adhesion, respectively. Laparoscopic adrenalectomy for adrenal metastases may be safe and effective. However, potential surrou…

Adrenal gland function in a dog following unilateral complete adrenalectomy and contralateral partial adrenalectomy.

CASE DESCRIPTION: A 40.3-kg (88.7-lb) 6-year-old spayed female Labrador Retriever was evaluated because of acute unilateral epistaxis. CLIN … Epistaxis resolved after treatment and resolution of severe systemic hypertension. A partial right adrenalectomy was performed to remove the right adrenal gland mass. Results of histologic examination of the mass indicated it was a well-differentiated carcinoma of the cortex of the adrenal gland. Results of ACTH stimulation tests after surgery indicated the do …

Comparison of Laparoendoscopic Single-Site Adrenalectomy with the Aid of Intracavitary Retractors and Multiport Laparoendoscopic Adrenalectomy.

Objective: To evaluate the operative outcome of the use of intracavitary retractors in transumbilical laparoendoscopic single-site (LESS) adrenalectomy in comparison with the conventional multiport laparoendoscopic procedure. Methods: Between July 2021 and December 2023, 34 patients underwent transumbilical LESS adrenalectomy with intracavitary retractors, while 47 patients underwent conventional multiport laparoscopic adrenalectomy. Comprehensive data were compared, including demographics, intraoperative outcomes, perioperative complications, postoperative visual analog pain scale score, anal…

Robotic approach for partial adrenalectomy.

Although safe and feasible, partial adrenalectomy is not a widespread procedure. Endorsement of robotic technologies and fluorescence techniques in adrenal surgery might help develop partial adrenalectomy and could avoid unnecessary total adrenalectomies. When performed in selected cases, partial adrenalectomy is associated with good postoperative outcomes comparable with those reported after total adrenalectomy. It has been hypothesized that one of the advantages of the robotic approach in adrenal-sparing surgery is to reduce manipulation of the gland allowing preservation of the vascularizat…
