[Agraphia of Gerstmann syndrome--attempt at characterization].

A review is given of well documented cases of Gerstmann's syndrome covering one century from 1888 to 1991 complemented by two own observations, Convergencies as well as divergencies are pointed out. It is concluded, that although aphasia may be an accompanying condition, aphasic agraphia as described in linguistic terms is not a sufficient explanation of agraphia in Gerstmann's syndrome which implies spatial-graphic particularities like disturbances of symmetry, reduplications of strokes, defective line orientation, choice of a different letter type and occurrence of nonsense characters not co…

Agraphia caused by acute right parietal infarction.

Injury in the dominant language hemisphere typically leads to agraphia, however we report a patient with agraphia after injury to the right angular gyrus. A 71-year-old Korean woman presented with the complaint of an inability to write for the last 7 days. The patient had been illiterate for most of her life, but had started learning to write Hangul, the Korean alphabet, at a welfare center 3 years ago. On language screening she was unable to write although she could read, and other language functions showed no abnormalities. Brain MRI showed acute infarction in the right angular gyrus. Her wr…

Alexia without agraphia in Creutzfeldt-Jakob disease.

Progressive dementia uncommonly presents with focal deficits referable to posterior cortical dysfunction. We describe a 62 year-old man who presented with progressive visual disturbance in whom detailed cognitive testing documented alexia without agraphia. The only finding from diagnostic investigations was hypoperfusion of the posterior left temporoparietal region on brain SPECT scan. He rapidly progressed and Creutzfeldt-Jakob disease (CJD) was confirmed at autopsy. Although reading disorders may develop in association with posterior cortical atrophy due to CJD, this is the first reported au…

Acquired agraphia caused by focal brain damage.

Motor and linguistic aspects of writing were evaluated in 31 subjects with focal damage in 1 of 3 regions of the left hemisphere: (1) dorsolateral frontal lobe sparing primary motor cortex (group FL), (2) parietal lobe (group PL), or (3) temporal lobe (group TL). A standard procedure was used to evaluate writing for grapheme formation, spatial arrangement, spelling, word selection, grammar, and perseveration. It was predicted that agraphia would be observed in all 3 groups, and that the most severe impairments would be associated with frontal lobe damage, particularly in aspects of writing dep…

Alexia without agraphia associated with spleniogeniculate infarction.

Pure alexia, following an infarction in the distribution of the left posterior cerebral artery, is attributed to damage of the left occipital lobe and the splenium of the corpus callosum. We describe a case of pure alexia in a 57-year-old woman with infarction of the left lateral geniculate body and the splenium of the corpus callosum, a variation on this classic disconnection syndrome.

Pure agraphia after deep left hemisphere haematoma.

Pure agraphia is reported following haematoma in the left centrum semiovale sparing both parietal and frontal cortices. There was total inability to produce graphemes in the absence of limb apraxia. The lesion is assumed to have prevented linguistic and graphemic systems from gaining access to the frontal motor programme.

Alexia without agraphia: from infarctions to malignancies.

Alexia without agraphia is a neurological syndrome characterised by an acquired inability to read with a preserved ability to write. It is caused by the combined effect of two lesions: in the splenium of the corpus callosum and in the occipital lobe of the dominant hemisphere. Splenial lesions disconnect the language areas in the temporal and parietal lobes of the dominant hemisphere from the visual areas in the occipital cortex of the contralateral side, while lesions in the dominant occipital lobe cause homonymous hemianopia. We describe two patients with lesions affecting the splenium and d…

Dysexecutive agraphia: a major executive dysfunction sign.

Different types of writing disorders associated with brain pathology have been described. Limited mention the writing disturbances associate … rt required for writing, but also to organize the ideas in the written texts. The term dysexecutive agraphia is proposed to refer to this writing disorder. Three illustrative cases are presented. It is finally suggested that questions regarding the ability to write should be included in dementia questionnaires and executive functioning testing.

[Transient agraphia due to a left parieto-occipital glioma in a right-handed patient: amnesic or "pure" agraphia? (author's transl)].

The authors investigate a case of agraphia induced by a left parieto-occipital tumor (glioma) in a right-handed 67 year-old patient. After three successive neuropsychological examinations it still proves immensely difficult to determine whether defects of spontaneous writing, dictation and copy are due to apraxia, alexia or motor disturbances. The authors discuss whether the clinical findings indicate a "pure agraphia" syndrome or "amnesic agraphia". Finally, problems of aetiology and lesional localisation are examined in the light of the literature. (Acta neurol. belg., 1977, 77, 321-330).

Remediation of alexia without agraphia: a case study.

Following a left temporoparietal-occipital haemorrhage and surgery, a 43-year-old, right-handed male exhibited alexia without agraphia. A remediation programme consisted of training in head turning to compensate for a right visual field defect, letter-by-letter reading aloud and covertly, drill with flash-cards to improve word recognition and practice in naming objects to improve dysnomia. The patient's reading improved markedly over a 6-week period and he was able to resume work as a respiratory therapy supervisor. A post-morbid depression resolved concomitantly with the patient's return to w…

Apraxic agraphia following thalamic damage: Three new cases.

Apraxic agraphia (AA) is a so-called peripheral writing disorder following disruption of the skilled movement plans of writing while the central processes that subserve spelling are intact. It has been observed in a variety of etiologically heterogeneous neurological disorders typically associated with lesions located in the language dominant parietal and frontal region. The condition is characterized by a hesitant, incomplete, imprecise or even illegible graphomotor output. Letter formation cannot be attributed to sensorimotor, extrapyramidal or cerebellar dysfunction affecting the writing li…

[Lesion localization of non-aphasic alexia and agraphia].

The author reviews the lesion localization of non-aphasic alexia and agraphia and proposes a new classification of alexia and agraphia on this basis. The newly proposed alexia and agraphia are pure alexia for kana (Japanese phonograms), or more generally pure alexia for letters, caused by a lesion in the posterior occipital area (posterior fusiform/inferior occipital gyri), and pure agraphia for kanji (Japanese morphograms) caused by a lesion in the posterior middle temporal gyrus and also a lesion restricted to the angular gyrus. In addition, the anatomical lesions presumably responsible for …

Progressive agraphia can be a harbinger of degenerative dementia.

By investigating three patients with progressive agraphia, we explored the possibility that this entity is an early sign of degenerative dementia. Initially, these patients complained primarily of difficulties writing Kanji (Japanese morphograms) while other language and cognitive impairments were relatively milder. Impairments in writing Kana (Japanese syllabograms), verbal language, executive function, visuo- and visuospatial cognition and memory were identified by neuropsychological testing. The agraphia was compatible with a peripheral type, based on deficits at the interface between the c…

Agraphia in Alzheimer's disease: an independent lexical impairment.

This study was conducted to delineate the pattern of the writing impairments in 12 patients with Alzheimer type dementia. The patients perfo … ologic decision task using printed words. These results would suggest that the "lexical" deficit in agraphia, i.e., difficulty in retrieval or loss of the spelling representations of words, is independent of the lexical or semantic capacities involved in other modalities.
