BINOCULAR INTERACTION IN AMBLYOPIA.

The review presents new ideas about developmental mechanisms of amblyopia, which are currently discussed in literature. Objective evidence has accumulated that amblyopia affects both monocular and binocular functions in visual processing. Given the increasing evidence of fundamental and clinical research, it is most likely that binocular dysfunction is primary, and monocular reduction is visual acuity is secondary to this disease. Amblyopia is rather binocular than monocular pathology, and binocular interaction seems to play a critical role in the pathogenesis of strabismic, anisometropic and

Multifocal electroretinography in amblyopia.

PURPOSE: To identify whether there are functional abnormalities in the retina of amblyopic eyes using multifocal electroretinography (mfERG). METHODS: This is a prospective study of patients ≥ 7 years of age identified with unilateral amblyopia (strabismic or anisometropic). Multifocal ERG and flash ERG were performed to compare parameters between the amblyopic and non-amblyopic fellow eyes. A complete analysis of the five ring averages was done including the central ring. RESULTS: Thirty-eight patients were included: mean age was 14.3 ± 7.3 years; 18 patients were strabismic and 20 were ani

Amblyopia: Detection and Treatment.

Amblyopia is the leading cause of monocular vision loss in children. Early recognition and treatment are important to prevent vision loss. The U.S. Preventive Services Task Force recommends vision screening for all children at least once between three and five years of age to detect the presence of amblyopia or its risk factors. The American Association for Pediatric Ophthalmology and Strabismus and the American Academy of Pediatrics recommend routine, age-appropriate red reflex testing, examination for signs of strabismus, and vision chart testing. Photoscreening may be a useful adjunct to tr

The other side of amblyopia.

Amblyopia is defined as a loss of letter recognition visual acuity in the affected eye; however, studies in both nonhuman primates and man have shown that other important aspects of vision, including color, motion, and contour perception, are also abnormal. The anatomical changes that occur in the lateral geniculate nucleus and visual cortex following monocular visual deprivation affect both eyes and follow very different patterns with deprivation that begins at different ages and differ markedly in the magnocellular and parvocellular pathways. The interactions between the eyes and the require

The challenges of amblyopia treatment.

The treatment of amblyopia, particularly anisometropic (difference in refractive correction) and/or strabismic (turn of one eye) amblyopia has long been a challenge for many clinicians. Achieving optimum outcomes, where the amblyopic eye reaches a visual acuity similar to the fellow eye, is often impossible in many patients. Part of this challenge has resulted from a previous lack of scientific evidence for amblyopia treatment that was highlight by a systematic review by Snowdon et al. in 1998. Since this review, a number of publications have revealed new findings in the treatment of amblyopia

Interventions for strabismic amblyopia.

BACKGROUND: Amblyopia is reduced visual acuity in one or both eyes in the absence of any demonstrable abnormality of the visual pathway. It is not immediately resolved by the correction of refractive error. Strabismus develops in approximately 5% to 8% of the general population. The aim of treatment for amblyopia is to obtain the best possible level of vision in the amblyopic eye. Different treatment options were examined within the review. OBJECTIVES: By reviewing the available evidence we wanted to establish the most effective treatment for strabismic amblyopia. In particular this review ai

Congenital ptosis and amblyopia.

PURPOSE: To investigate the incidence and causes of amblyopia in congenital ptosis. METHODS: The frequency and causes of amblyopia were evaluated in 83 eyes of 72 patients with congenital ptosis. Patient ages ranged between 10 months and 70 years. Amblyopia was defined as best-corrected visual acuity of 0.7 or less. After a complete ophthalmologic examination, the amount of ptosis and levator function was determined and the following amblyogenic factors were identified: ametropia, anisometropia, and strabismus. In cases with no other reason for amblyopia, stimulus deprivation amblyopia due to

The attentional blink in amblyopia.

Amblyopia is a disorder of visual acuity in one eye, thought to arise from suppression by the other eye during development of the visual cortex. In the attentional blink, the second of two targets (T2) in a Rapid Serial Visual Presentation (RSVP) stream is difficult to detect and identify when it appears shortly but not immediately after the first target (T1). We investigated the attentional blink seen through amblyopic eyes and found that it was less finely tuned in time than when the 12 amblyopic observers viewed the stimuli with their preferred eyes. T2 performance was slightly better throu

Amblyopia in congenital ptosis.

This study evaluates the association between congenital ptosis and amblyopia. Amblyopia was detected in 7 of 36 (19%) patients with congenital ptosis. Two patients (6%) with amblyopia had no contributing factors other than the presence of congenital ptosis. A statistically significant correlation between severe nonocclusive ptosis (greater than or equal to 4 mm) and the development of amblyopia was identified. No new cases of amblyopia developed after surgical repair of the ptosis, suggesting early surgery for severe nonocclusive congenital ptosis may decrease the incidence of amblyopia.

Running towards amblyopia recovery.

Amblyopia is a neurodevelopmental disorder of the visual cortex arising from abnormal visual experience early in life which is a major cause of impaired vision in infants and young children (prevalence around 3.5%). Current treatments such as eye patching are ineffective in a large number of patients, especially when applied after the juvenile critical period. Physical exercise has been recently shown to enhance adult visual cortical plasticity and to promote visual acuity recovery. With the aim to understand the potentialities for translational applications, we investigated the effects of vol

Amblyopia risk factor prevalence.

PURPOSE: In 2003, the American Association for Pediatric Ophthalmology and Strabismus (AAPOS) published a set of risk factors for amblyopia. The intent was to promote uniformity of reporting and development in screening. Because this prevalence is not yet known, this meta-analysis is an attempt to estimate it. METHODS: Major community preschool eye examination studies were reviewed and AAPOS cut-offs estimated. RESULTS: The approximate prevalence of anisometropia is 1.2%, hyperopia is 6%, astigmatism is 15%, myopia is 0.6%, strabismus is 2.5%, and visual acuity less than 20/40 is 6%. The mea

Baseline and outcome stereoacuity of children with anisometropic amblyopia undergoing dichoptic amblyopia treatment.

BACKGROUND: One rationale for dichoptic amblyopia therapy is that it may promote recovery of binocular function. Yet data on binocular outcomes in anisometropic amblyopia following dichoptic therapy are sparse. We report factors associated with pre- and post-treatment binocular function in anisometropic amblyopia, and examine binocular function in children who recover normal visual acuity compared to those with residual amblyopia. METHODS: Baseline and outcome stereoacuity and binocular function (BF) scores were pooled across 185 children (3-12 years of age) with anisometropic amblyopia who p

The tropical amblyopia syndrome (or tropical nutritional amblyopia) in the Mid-Western State of Nigeria.

One hundred and seven patients from the Mid-Western State of Nigeria with the tropical amblyopia syndrome are reported. Ninety-five patients (88.8%) had the amblyopia syndrome mainly; twelve patients (11.2%) had amblyopia and other manifestations of the tropical ataxic neuropathy. The young, aged 10-20 years, represented by sixty-six patients (61.8%), are predominantly affected by the uncomplicated syndrome. Thirteen patients (12.1%) showed muco-cutaneous evidence of avitaminosis. Fifty patients (46.7%) had temporal pallor of the optic discs, bilaterally, another fifty (46.7%) had normal discs

Standard Amblyopia Therapy in Adults with Longstanding Amblyopia Improves Visual Acuity and Contrast Sensitivity.

PURPOSE: Perceptual learning or dichoptic training may result in improved acuity in adult amblyopes. However, for amblyopic children (<18 years), most clinicians recommend standard part-time patching. The purpose of this study was to determine if standard amblyopia therapy results in an enhancement in vision in the amblyopic eye of adults. PATIENTS AND METHODS: Fifteen amblyopes (20/30 or worse) were recruited and nine (age (SD) 32.9 (16.31)) with anisometropia or anisometropia and strabismus (ie, combined mechanism amblyopia) completed the study. Previous therapy did not exclude subjects. Th
