Aplastic anemia is a bone marrow failure syndrome characterized by bone marrow hypoplasia and pancytopenia, often involving a T cell-mediated autoimmune mechanism. Treatment modalities aimed at hematopoietic recovery include immunosuppressive therapy, thrombopoietin receptor agonists (TPO-RA), and allogeneic hematopoietic stem cell transplantation (allo-SCT). The combination of TPO-RA with conventional immunosuppressive therapy using anti-thymocyte globulin and cyclosporine has become a standard treatment option. In addition to improved outcomes with allo-SCT from HLA-matched sibling donors, f

Aplastic anemia is an uncommon condition defined by peripheral pancytopenia in the context of hypocellular bone marrow. In the majority of cases, it is idiopathic in origin. However, exposure to certain drugs and toxins, autoimmune processes, and viral infections have been linked to this entity. This is the case of a 56-year-old female with an acute presentation of fever, odynophagia, and dysphagia. Physical examination revealed multiple hemorrhagic ulcers affecting her oropharyngeal mucosa with regions of necrosis. Mucosal biopsy was compatible with the presence of local necrosis and keratini

Aplastic anemia (AA) and myelodysplastic syndrome (MDS) are clonal diseases with hemopoietic stem cell (HSC) abnormalities, which are sometimes difficult to be distinguished from each other. The development of molecular detection techniques has facilitated our understanding of the molecular pathogenesis of the two diseases. This article reviewed the somatic mutation (SM) and cytogenetic changes of AA,and analyzed their molecular relationship with MDS and their roles in disease transformation. The most common somatic change in AA is the loss of PIGA and HLA alleles, which, along with trisomy 8 and de

The application of next-generation sequencing (NGS) has enhanced our understanding of the genetic landscape in acquired aplastic anemia (AA). Parallel progress has been in addressing aspects underlying immune dysregulation in disease pathogenesis. Novel insights into the molecular and biologic mechanisms have led to a shift in the paradigm of AA, from a solely autoimmune pathogenic concept toward its recognition as a multifaceted pathophysiology characterized by cytogenetic abnormalities, recurrent somatic mutations, telomere attrition, and immune dysregulation. The detection of recurrent driv

Acquired severe aplastic anemia (SAA) is a rare hematologic disease associated with significant morbidity and mortality. Immune destruction of hemopoietic stem cells plays an important role in pathogenesis, as shown by successful treatment with immunosuppressive agents, leading to transfusion independence or complete recovery of peripheral blood counts in a proportion of patients. Growth factors can be combined with immunosuppressive therapy (IST) and may improve response rates, as recently shown with thrombopoietin analogs. Anabolic steroids may still play a role in combination with IST. The

Aplastic anemia (AA) is a disease characterized by pancytopenia and hypoplastic bone marrow caused by the decrease of hematopoietic stem cells. The pathogenesis of AA is complex and involves an abnormal hematopoietic microenvironment, hematopoietic stem cell/progenitor cell deficiencies and immunity disorders. Survival in severe aplastic anemia (SAA) has markedly improved in the past 4 decades because of advances in hematopoietic stem cell transplantation, immunosuppressive and biologic drugs, and supportive care. Herein, we will update the main issues concern AA according to our literature re

A 2-year-old girl patient was admitted with intractable diarrhea, respiratory infections, and seizures. She was the first child of the first-degree parents. She was born at term with a birth weight of 2300 g. Physical examination revealed weight 6800 g, height 76 cm, and head circumference 41 cm, below the third percentile. Findings included sparse and dull hair, nail dystrophy, and proximally located thumbs (Figure). Aphthous lesions were observed on the oral mucosa. Neurologic examination disclosed poor head control. She could not sit without support and had hyper-reactive deep tendon reflex

Hepatitis-associated aplastic anemia (HAAA) is an uncommon but distinct variant of aplastic anemia in which pancytopenia appears two to three months after an acute attack of hepatitis. HAAA occurs most frequently in young male children and is lethal if leave untreated. The etiology of this syndrome is proposed to be attributed to various hepatitis and non hepatitis viruses. Several hepatitis viruses such as HAV, HBV, HCV, HDV, HEV and HGV have been associated with this set of symptoms. Viruses other than the hepatitis viruses such as parvovirus B19, Cytomegalovirus, Epstein bar virus, Transfus

Aplastic anemia, defined by the presence of pancytopenia and hypocellular bone marrow in the absence of any abnormal blood cells, is a serious complication of antipsychotic drug treatment. While hematological side effects from antipsychotic medications are rare, recent experience with clozapine, an atypical antipsychotic agent, has raised the awareness of clinicians to the serious and potentially fatal hematological changes that can occur with antipsychotic medications. A case of perphenazine-induced aplastic anemia in a 23-year-old schizophrenic man is reported.

BACKGROUND: Aplastic anemia (AA) encompasses a group of hematological syndromes often misdiagnosed, resulting in a decrease in the overall blood cell count and representing a form of bone marrow failure. The disease is defined by damage to hematopoietic stem cells due to various physical, chemical, and biological factors. The diagnosis of the disease is based on clinical presentation, as the underlying etiology remains poorly understood, which complicates treatment options. Understanding bone marrow failure disorders began with cytomorphological studies, which led to the identification of AA s

Aplastic anemia (AA) is an autoimmune disorder characterized by bone marrow and peripheral blood pancytopenia. Different environmental and genetical conditions could be effective in an outbreak of this disease. The exact pathogenesis of this disease, however, is still idiopathic. The present study is based on Pubmed database information (2002-2021) using the words "Aplastic Anemia," "Hematopoietic Stem Cells niche," "Signaling pathway," "Cytokines," and "Immuno cells." In this disease, both hematopoietic stem cells and mesenchymal stromal cells are impaired, which is associated with impaired h

Two unusual cases of hemophagocytic lymphohistiocytosis (HLH) complicating aplastic anemia (AA) are described. Each patient had a history of preexisting acute hepatitis of unknown cause at the time of HLH diagnosis and infection-associated secondary HLH. They developed high fever and pancytopenia. Hemophagocytes were seen in the bone marrow. With steroid (in combination with etoposide and CyA in 1 patient), high fever disappeared and the patients' liver function gradually recovered. As severe pancytopenia persisted, bone marrow became acellular and AA was diagnosed. Since HLH is known to be ab

Aplastic anemia, an unusual hematologic disease, is the paradigm of the human bone marrow failure syndromes. Absence of hematopoietic cells has been recognized from the characteristic morphology for a century; an immune pathophysiology has been inferred from improvement in blood counts with immunosuppressive therapy in the majority of patients. Molecular mechanisms underlying both T cell effector cells and the target marrow stem and progenitor cells are now being identified. Activated type 1 cytotoxic T cells and type 1 cytokines have been implicated in cell culture experiments; clues to the m
