Autopsies of 12 consecutively born infants with anencephaly showed varying degrees of aganglionosis and lateralization defects in four of them. This seemingly regular occurrence of these three defects together suggests that they are caused by an aberration of blastogenesis that results in a polytopic field defect.

Considerations of the genetics of anencephaly have been based on the premise that this dysraphism is a single entity. However, morphologically anencephaly is a heterogeneous group made up of at least four major morphologic types-anencephalus craniorachischisis, anencephalus acrania, microcephalus acrania and microcephalus craniorachischisis. While I believe that the various types of anencephaly represent only degrees of severity of the same basic underlying defect(s), the possiblity of genetic as well as morphologic heterogeneity should be considered.

About a third of anencephalic fetuses are born alive, but they are not conscious or viable, and soon die. This neural tube defect can be limited by dietary consumption of foliates, and detected prenatally by ultrasound and other means. Many laws permit abortion, on this indication or on the effects of pregnancy and prospects of delivery on a woman's physical or mental health. However, abortion is limited under some legal systems, particularly in South America. To avoid criminal liability, physicians will not terminate pregnancies, by induced birth or abortion, without prior judicial approval.

Limb deficiencies (LDs) are rarely reported in anencephalic infants. A review of 662 patients in the literature on non-neural defects in anencephaly only showed five patients with LDs. Information was collected on 2,197 stillbirths and neonatal deaths on the island of Jamaica during the 12 month period September 1986 to August 1987 as part of the population based nationwide Jamaican Perinatal Morbidity and Mortality Survey. There were 14 cases of anencephalus giving an incidence of 0.26 per 1,000 total births. One of these cases was associated with the amniotic band syndrome; excluding this, the incidence would be even lower (0.24 per 1,000).
