Angiotensin-converting enzyme inhibitors, angiotensin-receptor blockers, and risk of appendicitis.

PURPOSE: Angiotensin-converting-enzyme (ACE) inhibitors and, less commonly, angiotensin-receptor blockers (ARBs), have been associated with angioedema, including small bowel angioedema. We sought to determine whether this process might be associated with appendicitis. METHODS: We conducted a nested case-control study of incident appendicitis in a subcohort of 305,958 commercially insured hypertensive adults throughout the United States. Individuals with appendicitis were matched on age, sex, region, and subscriber status with up to 10 controls, and we examined use of ACE inhibitors, ARBs, β-b

Clinical potential: angiotensin converting enzyme inhibitor or angiotensin II antagonist?

Blockade of the renin-angiotensin system by angiotensin converting enzyme (ACE) inhibitors has an established role in the management of hypertension, heart failure, patients post-myocardial infarction and renal impairment. The mechanism of action of angiotensin II antagonists offers the potential of more complete blockade of angiotensin II, selective inhibition of the AT1 receptor and specificity for the renin-angiotensin system. Whether these mechanistic differences enhance the clinical potential of these drugs remains to be established. Preliminary evidence suggests that ACE inhibitors and a

Plasma angiotensin II: dipsogenic levels and angiotensin-generating capacity of renin.

Activation of the renin-angiotensin system (RAS) or administration of angiotensin II (AII) will induce water intake. In the rat, the species in which the physiological mechanisms of thirst have been most thoroughly studied, the levels of circulating AII produced by systemic administration of dipsogenic doses of the octapeptide have not been established. Furthermore, the capacity of the endogenous RAS to generate AII sufficient to contribute to the production of a thirst state has not been well studied. In the present series of investigations, plasma levels of AII were determined following infu

Expression of ACE2, Soluble ACE2, Angiotensin I, Angiotensin II and Angiotensin-(1-7) Is Modulated in COVID-19 Patients.

The etiological agent of COVID-19 SARS-CoV-2, is primarily a pulmonary-tropic coronavirus. Infection of alveolar pneumocytes by SARS-CoV-2 requires virus binding to the angiotensin I converting enzyme 2 (ACE2) monocarboxypeptidase. ACE2, present on the surface of many cell types, is known to be a regulator of blood pressure homeostasis through its ability to catalyze the proteolysis of Angiotensin II (Ang II) into Angiotensin-(1-7) [Ang-(1-7)]. We therefore hypothesized that SARS-CoV-2 could trigger variations of ACE2 expression and Ang II plasma concentration in SARS-CoV-2-infected patients.

Angiotensin-Converting Enzyme 2 and Antihypertensives (Angiotensin Receptor Blockers and Angiotensin-Converting Enzyme Inhibitors) in Coronavirus Disease 2019.

Coronavirus disease 2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2, is being defined as the worst pandemic disease of modern times. Several professional health organizations have published position papers stating that there is no evidence to change the use of angiotensin-converting enzyme inhibitors (ACEIs) or angiotensin receptor blockers (ARBs) in the management of elevated blood pressure in the context of avoiding or treating COVID-19 infection. In this article, we review the evidence on the relationship between the renin-angiotensin-aldosterone system and COVID-

Angiotensin-(1-5) is a Potent Endogenous Angiotensin AT 2 -Receptor Agonist.

BACKGROUND: The renin-angiotensin system involves many more enzymes, receptors and biologically active peptides than originally thought. With this study, we investigated whether angiotensin-(1-5) [Ang-(1-5)], a 5-amino acid fragment of angiotensin II, has biological activity, and through which receptor it elicits effects. METHODS: The effect of Ang-(1-5) (1µM) on nitric oxide release was measured by DAF-FM staining in human aortic endothelial cells (HAEC), or Chinese Hamster Ovary (CHO) cells stably transfected with the angiotensin AT 2 -receptor (AT 2 R) or the receptor Mas. A potential vaso

Des-aspartate-angiotensin I, a novel angiotensin AT(1) receptor drug.

The review describes DAA-I (des-aspartate-angiotensin-I) as a prototype of a novel class of drugs that acts as agonists on the angiotensin AT1 receptor or ARAs (angiotensin receptor agonists). DAA-I is a component of the renin angiotensin system. Earlier studies showed that it was rapidly metabolized to angiotensin III. However, when administered at doses below the Km of enzymes, DAA-I produces specific actions that antagonize the deleterious actions of angiotensin II. DAA-I exerts protective actions in animal models of eight human pathologies in which angiotensin II is implicated. The patholo

Prevention of angiotensin II-induced cardiac remodeling by angiotensin-(1-7).

Cardiac remodeling, which typically results from chronic hypertension or following an acute myocardial infarction, is a major risk factor for the development of heart failure and, ultimately, death. The renin-angiotensin system (RAS) has previously been established to play an important role in the progression of cardiac remodeling, and inhibition of a hyperactive RAS provides protection from cardiac remodeling and subsequent heart failure. Our previous studies have demonstrated that overexpression of angiotensin-converting enzyme 2 (ACE2) prevents cardiac remodeling and hypertrophy during chro

Effects of angiotensin-converting enzyme inhibitors on tissue renin-angiotensin systems.

The renin-angiotensin system (RAS) plays a major role in the control of blood pressure and cardiovascular homeostasis and is involved in the pathogenesis of a number of cardiovascular disorders. The efficacy of angiotensin-converting enzyme (ACE) inhibitors in the treatment of hypertension and congestive heart failure has led to the widespread clinical use of ACE inhibitors in primary or secondary prevention of heart disease. The demonstration of the expression of the components of the RAS in several extrarenal tissues, as well as local generation of angiotensin II, has confirmed the existence

Plasma angiotensin II and the antihypertensive action of angiotensin-converting enzyme inhibition.

The measurement of immunoreactive "angiotensin II" in plasma cannot provide an accurate reflection of the efficacy of angiotensin-converting enzyme (ACE) inhibition because different angiotensin fragments interfere in all radioimmunoassays available so far. More complex methods are necessary in order to measure specifically angiotensin-(1-8)octapeptide. With such methodology it can be shown that no tolerance develops to the angiotensin II-reducing effect of ACE inhibitors after prolonged administration. Marked reduction of angiotensin II levels can be shown even in patients with primary aldost

Cerebrovascular reactivity to angiotensin and angiotensin-converting enzyme activity in cerebrospinal fluid.

The purpose of the present study was to test the vasomotor effect of angiotensin I (A I) and angiotensin II (A II) in feline cerebral arteries and to examine the presence of angiotensin converting enzyme (ACE) activity in the vessel wall and cerebrospinal fluid (CSF). A II (10(-8) -10(-5) M) induced concentration-dependent contractions of feline pial arteries (resting diameter, 98-286 microns) in situ with a maximum of 34% at 10(-4) M A II. A I produced dose-related contractions being approximately 20 times less potent than A II. The action of A I was significantly attenuated by the ACE inhibi

Effect of angiotensin II and an angiotensin II inhibitor on renin secretion in the dog.

This study examined the effect of angiotensin II (AII) and (Sar-1Gly-8) AII, a competitive inhibitor of AII, on renin release in anesthetized dogs. An intravenous infusion of AII (25 ng/kg per min) raised BP 24 mmHg and lowered plasma renin activity (PRA) from 18.8 plus or minus 2.8 to 6.2 plus or minus 1. 3 ng/ml per h (P smaller than .001). When the intravenous infusion of AII inhibitor (1.0 mug/kg per min) was superimposed on the AII infusion, PRA rose from 6.5 plus or minus 22 to 12.3 plus or minus 3.6 ng/ml per h (P smaller than .02). Renal hemodynamics, BP, and urinary sodium excretion (

Effect of angiotensin receptor blockers and angiotensin-converting enzyme 2 on plasma equilibrium angiotensin peptide concentrations in cats with heart disease.

BACKGROUND: Little is known about the effect of renin angiotensin aldosterone system-inhibiting (RAASi) drugs on alternative angiotensin peptides (APs) such as angiotensin 1-7 (Ang1-7), which are mediated by angiotensin-converting enzyme 2 (ACE2). HYPOTHESIS/OBJECTIVES: Angiotensin receptor blockers (ARBs) would alter balance of APs and differences would be magnified in vitro by incubation of plasma samples with recombinant human ACE2 (rhACE2). ANIMALS: Six cats with cardiomyopathy (CM), 8 healthy cats. METHODS: Prospective open label trial. Plasma equilibrium concentrations of APs were mea

Effect of angiotensin receptor blockers and angiotensin converting enzyme 2 on plasma equilibrium angiotensin peptide concentrations in dogs with heart disease.

BACKGROUND: The pathophysiology of heart failure involves maladaptive angiotensin peptides (APs) and enzymes, including angiotensin 2 (AT2) and angiotensin converting enzyme (ACE), as well as recently described alternative components, such as angiotensin 1-7 (Ang1-7) and angiotensin converting enzyme 2 (ACE2). The relative effects of different neurohormonal-targeting drugs on balance of APs in dogs with heart disease are unknown. HYPOTHESIS/OBJECTIVES: Plasma AP concentrations differ in dogs receiving angiotensin converting enzyme inhibitors (ACEIs) vs angiotensin receptor blockers (ARBs) and

[Angiotensin II and the kidney].

Angiotensin II (AngII) plays a central role for maintenance of GFR and Na balance particularly in volume depletion, when AngII preferentially increases the resistance of efferent arterioles as compared to afferent arterioles, enhancing the glomerular perfusion pressure. In addition, AngII enhances tubular reabsorption of sodium in proximal tubules directly and indirectly as a consequence of glomerulotubular balance. AngII also stimulates Na reabsorption in the collecting ducts by stimulating the release of aldosterone from the adrenal cortex. AngII augments tubuloglomerular feedback. Recently,
