[Osteoarthritis of the upper ankle joint].

The majority of cases of upper ankle joint (UAJ) osteoarthritis are due to secondary causes. Clinically, osteoarthritis is usually characterized by an increasing limitation in dorsal extension of the UAJ with often relatively mild symptoms. In the course of time the full scale of the typical symptoms and the progressive restriction of the global function of the joint develop. Conservative therapy is often able to provide long-term improvement of the symptoms for the majority of patients by means of intermittent analgesics and orthoses as well as shoe modifications. Operative treatment strategi…

[Revision arthroplasty of the ankle joint].

In the last 20 years total ankle replacement has become a viable alternative to arthrodesis for end-stage osteoarthritis of the ankle. Numerous ankle prosthesis designs have appeared on the market in the past and attracted by the encouraging intermediate results reported in the literature, many surgeons have started to perform this procedure. With increased availability on the market the indications for total ankle replacement have also increased in recent years. In particular, total ankle replacement may now be considered even in younger patients. Therefore, despite progress in total ankle ar

[Injury of the ankle joint ligaments].

The diagnosis of lateral collateral ankle ligament trauma is based on patient history, clinical examination and clinical stress tests. If the clinical stress test is positive, stress radiography can be performed. There is, however, no consensus about the usefulness of stress radiography in acute ankle sprain, and in particular about the cut-off talar tilt angle beyond which a two-ligament rupture would be certain, ranging from 5 degrees to 30 degrees. Today, magnetic resonance imaging (MRI) is not used in this area, although it does allow controlled positioning of the foot and defined section

[Ligament pathology of the ankle joint].

Recent advances in ultrasound technology and the development of high-resolution ultrasound transducers have enabled detailed depiction of superficial musculoskeletal structures. The advantages of ultrasound include wide availability and dynamic evaluation. The main disadvantage is the uneasiness of performing ultrasonography, especially for foot and ankle. In addition, diagnostic accuracy requires time, knowledge, and meticulous attention to technical parameters. Ultrasound can be used to evaluate joints, ligaments, tendons, plantar fascias, fore-foot diseases and to look for foreign bodies. P

Septic arthritis of the ankle joint.

BACKGROUND: Septic arthritis of the ankle joint is a rare but serious disease. Very few reports in the literature have mentioned the method of care and treatment results of septic ankle. This study was designed to retrospectively review the treatment results and to analyze the prognostic factors of septic arthritis of the ankle inpatients at our hospital. We also formulated a protocol for the management of septic arthritis of the ankle joint. METHODS: All records of those patients with a diagnosis of septic ankle from 1985 to 1997 were retrospectively reviewed. There were 29 patients that met…

[MRI arthrography of the ankle joint].

Due to its superior soft tissue contrast conventional MRI is the imaging method of choice in the evaluation of ankle joint disorders. Conventional MR imaging can accurately demonstrate normal or acutely injured ligaments; however, in subacute and chronic injury joint fluid necessary for delineation of injured ligaments is absent and MR arthrography should be performed. MR arthrography uses the intraarticular injection of contrast material to distend the joint, yielding improved discrimination of intraarticular structures. This joint distension with MR arthrography is also helpful in the stagin…

Lateral instability of the ankle joint.

Acute lateral ankle ligament ruptures are successfully treated nonoperatively with physiotherapy, peroneal strengthening, and coordination training. About 10-20% of patients may develop functional instability despite adequate nonoperative treatment. Chronic functional instability is not always a severe disability, but reconstruction of the lateral ankle ligaments may be necessary for patients with high demands on ankle stability. More than 50 different surgical procedures for the treatment of chronic lateral ankle joint instability have been described. Most of these are tenodeses where one of …

[The ankle joint fork and tarsus].

Severe soft tissue injury occurring in sprains and dislocations may lead to painful disability after plaster-immobilisation for 6-12 weeks. Therefore, primary operation is indicated for ligamentous repair, osteochondral fracture-fixation and internal K-wire stabilization of dislocations after reduction. After 1 week of immobilisation, the patient should begin active daily muscle exercises; in the 2nd week patients are mobilized with support of a dynamic, partial weight-bearing splint, allowing continued muscle exercises until weight-bearing is possible 6-8 weeks after surgery. In 23 patients u

[The ankle joint in fibula malformations].

The ankle joints of 109 extremities with fibula reductions wer examined. The authors' findings led them to conclude that the shape of the ankle joint must be regarded as the result of an adaptation to altered functions. Ball-shaped ankle joints are frequently found in cases of hypoplasia of the fibula; partial and subtotal aplasias of the fibula are more likely to be associated with a valgus deformity of the ankle joint (known as Volkmann's ankle joint deformity) or, if more severe, with kyphosis of the tibia. If there is no laterodorsal restraint by coarse fibrous strands in cases of fibular …

Fractures and dislocations of the ankle joint.

The classification of fractures of the ankle joint is based on the mechanism of injury, which provides the clue to successful reduction. The physiological movements of the ankle and foot are defined, and the pathological movements associated with injury are described.

[Combined tibial and ankle joint fractures].

The authors report upon 773 fractures of the lower leg investigated from 1979 to 1989. They included 38 tibial fractures combined with fractures of the ankle joint. According to the mechanism of the fractures they were divided into: isolated tibial shaft fractures, transverse, segmental and communicated fractures and external rotation fractures of the tibial shaft combined with additional lesions of the ankle joint.
