Antithrombin III is an anticoagulant synthesized in the liver cells. The aim of this study was to estimate the serum level of antithrombin III in cirrhotics. Investigations were carried out in 32 patients with cirrhosis and in 20 healthy controls. Antithrombin III measured by radial immunodiffusion was found significantly lower (19.2 +/- 6.8 mg%) than in controls (28.0 +/- +/- 5.2 mg%) (p less than 0.01). Antithrombin-(AT)-III-levels from 22 patients were determined at the day of delivery but also at the first and fifth day post partum. 7 patients had an AT-III-level in a pathologically low range. Antithrombin III (AT-III) is a serine protease inhibitor (serpin) that can be catalytically inactivated by human neutrophil elastase (HNE) without inhibiting HNE activity. Surgery is the basic treatment method of cholecystolithiasis. Traditional methods have been replaced laparoscopy. The aim of the study was the evaluation of haemostasis after classical and laparoscopic cholecystectomy. The studies were conducted in 48 patients suffering from chronic cholecystolithiasis--29 of them were operated on by laparoscopy, 19 patients were operated on by classical way. We revealed, that classical and laparoscopic cholecystectomy cause a similar increase of thrombin generating in vascular bed and are risk factors for occurrence of thrombo-embolic complications.

We report a newborn male who presented with severe central cyanosis on the third day of life. Partial thrombotic obstruction of the pulmonary trunk secondary to Antithrombin III (homozygous defect of heparin binding site) deficiency was subsequently diagnosed. Surgical thrombectomy, and infusions of Antithrombin III concentrate, led to a successful outcome. We postulate that intrauterine thrombosis occurred to give this unusual presentation.
