Psychoanalytic contributions to the theory of paranoia are reviewed. As a primary problem underlying paranoia, this paper emphasizes the importance of beating fantasy associated with a fragile personality structure rather than the theory of repressed homosexuality. Castration anxiety and homosexual conflict may precipitate paranoia, but all levels of psychosexual development and their corresponding danger situations contribute to the transformed fantasy of persecution and punishment; early infantile narcissism, aggression, and sadomasochism are especially important. Ciliary neurotrophic factor (CNTF) is produced by astrocytes and promotes neurogenesis and neuroprotection. Little is known about the role of CNTF in affective behavior. We investigated whether CNTF affects depressive- and anxiety-like behavior in adult mice as tested in the forced swim, sucrose preference and elevated-T maze tests. OBJECTIVE: Roles for estradiol in modulating cognition in men remain uncertain. We assessed the isolated effects of estradiol on cognition in men in the absence of testosterone.
DESIGN: Randomized trial of transdermal estradiol 0.9 mg daily, or matched placebo, for 6 months, hypothesizing that estradiol would improve verbal learning, verbal memory, and spatial problem solving over time.
PATIENTS: Men receiving androgen deprivation therapy (ADT) for prostate cancer.

Observations of a girl's development are presented to demonstrate the role of the transitional object in coping with the normal strains implicit in the separation-individuation thrust and in the anal-urinary and early sexual stages of development. When at eighteen months a separation experience led to a serious preoedipal castration reaction, the transitional object no longer was capable of serving its ordinary function and was replaced by a fetishistic object.

The hypotheses (including pathology in the mother-child relationship; defective object relations; aggression and sexuality, etc.) are based on a four-year clinical investigation of over 125 patients who requested sexual surgery.
