[Sleep apnea syndrome].

The occurrence of episodes of obstructive apnoea during sleep is encouraged by a variety of factors: local anatomical abnormalities, degree of activation of the autonomous nervous system, obesity, central nervous system depression from alcohol or drug consumption, deprivation or fragmentation of sleep. Each of these factors can play a predominant role in the development of the syndrome and their demonstration can contribute to our understanding of the success or failure of the different treatments. The authors' therapeutic experience includes mainly healthy living associated with treatment of

Central sleep apnea and complex sleep apnea in patients with epilepsy.

PURPOSE: We sought to examine the prevalence of central sleep apnea (CSA) and complex sleep apnea (CompSA) in patients with epilepsy and to examine their clinical profile, with respect to epilepsy type, etiology, medication use, and EEG abnormalities. METHODS: We undertook a retrospective analysis of 719 consecutive patients with epilepsy who underwent polysomnography (PSG) at our institution between 2004 and 2011. Of the 458 patients with complete data, we excluded 42 patients with congestive heart failure or left ventricular ejection fraction <40 %. Comparison of clinical and PSG variables …

[Clinical experience with the Apnea Check System in screening for sleep apnea].

We examined 17 patients (3 female, 14 male) of 40 to 60 years of age suspected of suffering from sleep apnoea (SA) according to anamnesis and pulse oximetry, using the apnoea check system (AC) and by polysomnography (PS). AC measures the flow at the mouth and nose by means of thermistors and records the apnoea number, maximal duration of apnoea, and mean duration of apnoea (A mean). The PS-measured values: apnoea index (AI), maximal duration of apnoea (A max) and mean duration of apnoea (A mean) were taken to be equal to 100% and the AC-measured values were referred to these. AI (AC) was 76-57

Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) is a common but frequently undiagnosed condition. Symptoms include loud snoring, nocturnal awakening, and daytime sleepiness. Treating OSA can meaningfully improve symptoms and achieve modest reductions in blood pressure. Although OSA care has traditionally been limited to sleep specialists, technologies are available that allow primary care clinicians to diagnose OSA and initiate treatment. Treatments include positional therapy, weight management, application of external positive airway pressure, mandibular advancement devices, and surgical interventions. Inpatie…

Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) is a very common and underdiagnosed condition across the world. It affects both pediatric and adult populations in unique but important ways. Long-term health risks associated with OSA include cardiovascular conditions, metabolic disorders, depression as well as poor work performance, and increased risk of motor vehicle accidents. Accurate and precise testing is vital to ensure accurate treatment, and specific testing methods are reviewed. Treatment options are discussed in detail for both adult and pediatric populations.

Central Sleep Apnea.

Central sleep apnea (CSA) is characterized by intermittent repetitive cessation and/or decreased breathing without effort caused by an abnormal ventilatory drive. Although less prevalent than obstructive sleep apnea, it is frequently encountered. CSA can be primary (idiopathic) or secondary in association with Cheyne-Stokes respiration, drug-induced, medical conditions such as chronic renal failure, or high-altitude periodic breathing. Risk factors have been proposed, including gender, age, heart failure, opioid use, stroke, and other chronic medical conditions. This article discusses the prev…

Central sleep apnea.

Neurophysiologically, central apnea is due to a temporary failure in the pontomedullary pacemaker generating breathing rhythm. As a polysomnographic finding, central apneas occur in many pathophysiological conditions. Depending on the cause or mechanism, central apneas may not be clinically significant, for example, those that occur normally at sleep onset. In contrast, central apneas occur in a number of disorders and result in pathophysiological consequences. Central apneas occur commonly in high-altitude sojourn, disrupt sleep, and cause desaturation. Central sleep apnea also occurs in numb…

Obstructive sleep apnea.

Obstructive sleep apnea (OSA) is a common disorder characterized by repetitive collapse of the pharyngeal airway during sleep. Control of pharyngeal patency is a complex process relating primarily to basic anatomy and the activity of many pharyngeal dilator muscles. The control of these muscles is regulated by a number of processes including respiratory drive, negative pressure reflexes, and state (sleep) effects. In general, patients with OSA have an anatomically small airway the patency of which is maintained during wakefulness by reflex-driven augmented dilator muscle activation. At sleep o…

Tramadol-induced apnea.

BACKGROUND AND OBJECTIVES: In contrast with other opioids, there are few cases of tramadol-related respiratory depression described in the literature, and renal impairment is a proposed risk factor. The aim of this study is to determine the prevalence of and predisposing factors for tramadol-related apnea in patients referred to our center. PATIENTS AND METHODS: All patients referred to Loghman-Hakim Hospital between February 2009 and April 2010 with pure tramadol intoxication were identified retrospectively. Data collected included the patient's age, sex, ingested dose, route of exposure, re…

Central sleep apnea.

Central apnea is caused by temporary failure in the pontomedullary pacemaker generating breathing rhythm, which results in the loss of ventilatory effort, and if it lasts 10 seconds or more it is defined as central apnea. This article reviews current knowledge on central sleep apnea.

[Sleep apnea syndrome].

Obstructive sleep apnea syndrome (OSAS) is strongly associated with cardiovascular disease which means these patients could suffer sudden death from these cardiovascular diseases, but it is a rare case that common OSAS itself causes sudden death directly. On the other hand, we never wrongly diagnose serious sleep-related breathing disorders (SBDs) such as sleep hypoventilation syndrome, OSAS patients with cardiovascular disease (i.e. ischemic heart disease, cardiac arrhythmia and cardiac failure) and bilateral vocal cord paralysis caused by multiple system atrophy as common OSAS. This section …

Central sleep apnea.

A central apnea is a disorder characterized by apneic events during sleep with no associated ventilatory effort. Central sleep apnea syndrome is characterized by repeated apneas during sleep resulting from loss of respiratory effort. Although the etiology of central apnea remains obscure in most cases, current investigations into breathing control system during sleep and association with certain diseases have pointed out possible mechanisms. Ventilation during sleep is highly dependent on the nonbehavioral control system. As a result, any diseases affecting this control system could influence …

Central sleep apnea.

CSA is a heterogeneous disorder with numerous causes and associations. It is much more common than previously believed, and is easily overlooked if not carefully sought. If undiagnosed, it may be associated with significant morbidity and even mortality. It is easily and noninvasively diagnosed, and therapy is often effective and can favorably impact the course of disease. Consequently, it would behoove all physicians to at least consider the diagnosis of CSA in any patient who has any predisposing condition, sign or symptom associated with this disease entity.

Central sleep apnea.

A central sleep apnea is the absence of respiratory effect, and, thus, the absence of airflow during sleep. Central hypopnea, a related disorder, is also discussed. The sensory component of central sleep apnea; defects involving the integrative and executive neurons; non-neurologic causes of central sleep apneas, including chronic obstructive pulmonary disease and congestive heart failure; diagnosis; treatment; and other topics are reviewed in detail.

[Sleep apnea syndrome].

Excessive daytime sleepiness (EDS) in certain patients with sleep apnea syndrome may be the result of intermittent upper airway obstruction during sleep. The possible occurrence of daytime hypertension and pulmonary hypertension (PH) in these patients has been emphasized. Transient and sometimes severe elevations of systemic and pulmonary arterial pressures during sleep as a result of intermittent upper airway obstruction may lead to EDS, daytime hypertension, and PH. The aim of the present study was to investigate whether daytime hypoxemia and hypoxemia during sleep contribute to EDS, daytime…

Drug-induced apnea.

Drugs have been in the past and will in the future still be liable to induce apnea in neonates, infants and older children. At these different stages of development, the child may be abnormally vulnerable to respiratory disorders and apnea, and doses of drugs, without any abnormal side effects in adult patients, can be harmful in younger subjects. Drugs responsible for apnea during development are numerous, but more than half of the problems are induced by sedatives and hypnotics, among which phenothiazines, barbiturates, benzodiazepines (included transplacentally acquired) and general anesthe…

Central sleep apnea.

The critical issue in considering the diagnosis and management of CSA is to determine the physiologic process underlying the disorder. CSA includes a pathophysiologically and clinically heterogeneous group of disorders that can be divided into two main groups on the basis of the awake PaCO2: a hypercapnic group, in whom the disorder is related to central alveolar hypoventilation or neuromuscular disease, and a nonhypercapnic group, in whom there is no identifiable underlying disorder. The common feature of these two groups is recurrent episodes of central apnea during sleep related to withdraw…
