OBJECTIVE: Arachnoid cysts constitute 1% of all intracranial mass lesions not resulting from trauma. Suprasellar arachnoid cysts (SACs) are uncommon. Obstructive hydrocephalus is the most common cause of initial symptoms and occurs in almost 90% of the patients with suprasellar arachnoid cyst. We report on 17 patients with suprasellar arachnoid cyst who were treated with neuroendoscopic intervention.

OBJECTIVE: To analyze the clinical manifestation and neuroendoscopic techniques in treatment of suprasellar arachnoid cysts.
METHODS: Between October 2000 and June 2006, 42 patients (22 male and 20 female with a mean age of 10.4 years) with suprasellar arachnoid cysts were treated by endoscopy. All patients presented with hydrocephalus in CT and MRI scanning before operation. Endoscopic ventriculocystostomy was performed in 39 patients and cystocisternostomy was also performed in the other 3 patients. The mean duration of follow-up was 20.8 months.

During the period from 1983 to 1999, 12 patients with symptomatic arachnoid cyst were treated. The mean patient age was 20.6 years. The most common location was the middle fossa (10 cases). Initial CT scan revealed associated subdural hygroma in 7 patients and hydrocephalus in one. Clinical symptoms were related to increased intracranial pressure, cranio-megaly and seizure. The results of surgical treatment in 21 cases of intracranial arachnoid cysts, mostly in adults, are reviewed. There were 19 supratentorial and 2 infratentorial cysts. Ten cases of arachnoid cysts of the ponto-cerebellar angle are presented. In most cases, local arachnoiditis is disclosed during surgery, directing the discussion toward acquired pathology, and perhaps toward local infection. The clinical symptoms are dominated by the cochleo-vestibular deficit, but the involvement of the V and the VII cranial nerves is inconstant and discreet. At the present time, C.T. scan allows a rapid diagnosis but the prognosis must be reserved, in account to the possibility of a local post operatory arachnoiditis .

Between 1980 and 1982, 8 children with congenital arachnoid cysts were treated at the University of Florida, 2 patients presented with unusual clinical syndromes, including isosexual precocity and a cephalic movement disorder. Cystoperitoneal shunting in combination with ventriculoperitoneal shunting for associated hydrocephalus was employed in the treatment of all supratentorial lesions, and proved to be a safe, efficacious mode of therapy.

Retroclival arachnoid cyst is a rare mass lesion, with only seven cases previously reported in literature. MR is the imaging modality of choice in its evaluation and in its differentiation from epidermoid cyst. The case reported here was surgically proved and had an uncharacteristic MR signal intensity that was higher than that of cerebrospinal fluid due to previous hemorrhage.

Familial congenital spinal arachnoid cysts causing progressive paraplegia are reported in two adolescent siblings as part of a hereditary syndrome. The other features of this unusual dominantly inherited disorder include double rows of eyelashes, partial ectropion of the lower eyelids, and acquired late-onset lymphedema of the lower extremities. The siblings' mother, who had roentgenographic evidence of a similar intraspinal lesion, was free of neurological symptoms but had the other characteristic features. Surgical treatment of the arachnoid cysts improves the progressive paraplegia.
