Autonomic dysreflexia is a syndrome which occurs in patients with lesions of the spinal cord above T6, and it is characterized by exaggerated autonomic responses to stimuli which are innocuous in normal individuals. Clinical features of 68 patients with autonomic dysreflexia in traumatic myelopathy are presented and special attention is drawn to ocular manifestations of autonomic dysreflexia. The symptoms, signs, laboratory findings, differential diagnosis, pathophysiology and management have been critical analysed and pertinent literature has been extensively reviewed. A plea is made for thos…

BACKGROUND: Autonomic dysreflexia (AD) is a frequent complication of spinal cord injury (SCI), though current clinical practice patterns for medication management of this condition are unknown. Correspondingly, it is unclear if national differences in practice patterns exist.
OBJECTIVE: To determine trends in current pharmacologic management of AD throughout the Americas.
DESIGN: International survey of current physician practice patterns.
SETTING: Academic medical center.
PARTICIPANTS: Sixty physicians managing patients with SCI and prescribing medications to manage AD.
INTERVENTIONS: No…

Context: Autonomic dysreflexia (AD) is characterized by a sudden onset of hypertension in those with spinal cord injuries (SCI) at T6 or above. Prompt recognition and treatment of AD is important. Herein are two individuals with SCI and acute hypertension due to anxiety that may easily have been confused for AD. Findings: Case 1: A 77-year-old male with C4 AIS D SCI and a history of anxiety and AD presented for urodynamics to evaluate the effectiveness of onabotulinumtoxinA injections into his urethral sphincter. After lying down on the urodynamics table, his systolic blood pressure (SBP) was …

STUDY DESIGN: A case report of silent myocardial ischemia in a man with C5 AIS A (American Spinal Injury Association Impairment Scale) tetraplegia during an episode of autonomic dysreflexia (AD).
OBJECTIVE: The aim of this study was to show a clinical presentation of myocardial ischemia in individuals with spinal cord injury (SCI).
CASE REPORT: A 45-year-old man with chronic C5 complete SCI presented with an episode of uncontrolled AD. He denied any symptoms of typical myocardial ischemia. Despite initiation of the AD management protocol, his blood pressure remained elevated. Additional test…

Autonomic dysreflexia is a potentially life-threatening condition in which episodic hypertension occurs after injuries above the mid-thoracic segments of the spinal cord. Despite the seriousness of this condition, little is known of the molecular mechanisms that lead to its development. The completed sequencing of the mouse genome, its dense genetic map, and the large repository of engineered and spontaneous mouse mutants, make the mouse an ideal model organism in which to study the molecular mechanisms underlying autonomic dysreflexia. We subjected two wild-type strains of mice, 129Sv and C57…

BACKGROUND: Although autonomic dysreflexia (AD) is well documented in the spinal cord injury (SCI) population, its occurrence in persons with multiple sclerosis (MS) is not. A dense multiple sclerotic lesion in the spinal cord at or above the sixth thoracic level can cause interruption of descending inhibitory impulses and thus result in AD. A patient with MS presented to our facility with classic signs and symptoms of AD. We believe that lack of knowledge about the risks for this condition in MS led to a delay in diagnosis.
METHODS: Case report illustrates AD in a person with MS. A convenien…

Autonomic Dysreflexia (AD) is a potentially life-threatening complication for spinal cord injured individuals. It is one of the most serious emergencies that can develop post trauma. AD provides a challenge to nurses. A basic understanding of the patho-physiology involved in AD is recommended and encouraged for those people who care for the spinal cord injured. The purpose of this paper is to provide a tool for nurses to identify, understand and manage an AD crisis using a step-by-step approach.

Three patients with chronic traumatic cervical myelopathy had severe orthostatic hyperhidrosis. Orthostatic challenge revealed that hypotension preceded hyperhidrosis, hypertension, and chills, all manifestations of autonomic dysreflexia. Treatment of orthostatic hypotension with fludrocortisone acetate relieved these symptoms. Therefore, orthostatic hypotension may trigger autonomic dysreflexia and the usual way of managing such patients, propping them upright, may be counterproductive.

Autonomic dysreflexia has potentially life-threatening consequences in quadriplegics and high paraplegics. Seizures, subarachnoid hemorrhage, and fatal stroke may result. This article explains the pathophysiology of autonomic dysreflexia. Appropriate nursing interventions to prevent serious complications are described.

Seizures are a little-recognized component of the syndrome of autonomic dysreflexia. Three patients who exhibited seizures during episodes of dysreflexia independent of intracerebral pathology are described. The presence of seizures in patients with traumatic myelopathy T-6 should alert the physician to a possible dysreflexic episode, and measures should be taken to avert such an episode.

Chlorpromazine has proved useful in the control of sustained dysreflexia in patients with spinal cord injury. Sustained autonomic dysreflexia can be life threatening and may occur after dysfunctional urethrovesical activity or after urological operations, such as external sphincterotomy. Given by injection chlorpromazine reduced blood pressure safely and effectively.

Autonomic dysreflexia (AD) is not an uncommon clinical condition and it is usually detected in patients with complete spinal injuries at or above thoracic 6th vertebral level (T 6). This condition is reported to occur in 48- 60% of cases of spinal cord injury at or above the level of T6. But AD due to injury below T6 is rare. The basic mechanism is thought to be due to excessive, uncontrolled activation of sympathetic system. In the present case, we discuss a persistent AD in 55-yr-old tetraplegic patient with C5 American Spinal Injury Association (ASIA) grade A lesion due to a fall from 10 me…

Patients who are admitted to the critical care unit frequently have numerous diagnoses including ones that can interfere with their treatment. One secondary condition with significant impact on nursing care is autonomic dysreflexia. Noxious stimuli in the ICU can precipitate a life-threatening autonomic response that results in hemodynamic and neurologic complications. Critical care nurses and advanced practice nurses are critical in preventing or making an early identification of this complication.
