[Autonomic nervous system and obstructive sleep apneas].

Understanding of the pathophysiology of obstructive sleep apnoea, a common yet relatively newly recognised condition, has advanced rapidly in recent years. This condition produces major acute haemodynamic changes and causal relationships with arterial hypertension and cardiovascular morbidity have been proposed. The role that the autonomic nervous system plays in mediating these cardiovascular changes has been the focus of intensive research activity and the development of new techniques in physiological monitoring, such as spectral analysis of heart rate variability, Finapres blood pressure m…

[Atrial fibrillation and the autonomic nervous system].

The autonomic nervous system plays an important role in the genesis of various cardiac rhythm disorders. Experimental studies have indicated that in atrial fibrillation particularly vagal activity may have a decisive influence on the electrophysiologic properties of atrial myocardium. In patients with paroxysmal atrial fibrillation, a carefully taken history and electrocardiographic findings obtained during Holter monitoring can help to distinguish vagally mediated from adrenergically mediated atrial fibrillation. The former is considered to represent a form of lone atrial fibrillation affecti…

Vibration stress and the autonomic nervous system.

Raynaud's phenomenon has been considered to be due to activation of the central sympathetic vasoconstrictor reflex, and may represent part of a larger dysfunction of higher autonomic centers. Symptoms, such as sweating disturbances, orthostatic hypotension, insomnia and impotence have been reported to be more common among vibration exposed workers. We studied 217 male forest workers and selected samples of this population for electromyographic (N = 80), autonomic nervous system function, controlled breathing, tilting bed and valsalva manoeuvre (N = 88) tests, and a full clinical neurological e…

The autonomic nervous system and perinatal metabolism.

The development of the autonomic nervous system in relation to perinatal metabolism is reviewed with particular attention given to the adipocyte, hepatocyte and the A and B cells of the islets of Langerhans. Adrenergic receptors develop in the B cell independently of normal innervation and by the time of birth, in most species studied, the pancreas, liver and adipose tissue respond appropriately to autonomic signals. Birth is associated with a huge surge in circulating catecholamines which is probably responsible for the early postnatal rise in free fatty acids and glucagon concentrations in p…

Evaluating dysfunction of the autonomic nervous system.

Simple bedside measurements of blood pressure and systolic pressure response to the Valsalva maneuver will confirm a clinical impression of orthostatic hypotension. Careful questioning of the patient usually elicits other symptoms of autonomic nervous system dysfunction, such as impotence, urinary and fecal incontinence, constipation or diarrhea, blurred vision, or sweating changes. Drugs are the most common cause of autonomic dysfunction, and their benefits should be weighed against the severity of the dysfunction. In addition, diabetes mellitus, uremia, amyloidosis, acute intermittent porphy…

Clinical Assessment of the Autonomic Nervous System.

This review explores standardized clinical assessments for evaluating autonomic nervous system function. Here, we emphasize components of the autonomic medical history as the first pivotal test of the autonomic assessment. We further focus on standard noninvasive cardiovascular autonomic tests including heart rate responses to deep breathing, Valsalva maneuvers, and orthostatic challenges, along with insights from neurochemical tests and quantitative sudomotor axon reflex testing. The article also covers practical aspects of cardiovascular autonomic monitoring, including patient set-up using b…

The cardiac autonomic nervous system: an introduction.

In recent decades, numerous anatomical and physiological studies of the cardiac autonomic nervous system (ANS) have investigated the complex relationships between the brain and the heart. Autonomic activation not only alters heart rate, conduction, and hemodynamics, but also cellular and subcellular properties of individual myocytes. Moreover, the cardiac ANS plays an essential role in cardiac arrhythmogenesis. There is mounting evidence that neural modulation either by ablation or stimulation can effectively control a wide spectrum of cardiac arrhythmias. This article discusses anatomic aspec…

Autonomic nervous system control of multiple myeloma.

The autonomic nervous system (ANS), which consists of antagonistic sympathetic (adrenergic) and parasympathetic (cholinergic) arms, has emerged as an important regulator of neoplastic development, yet little is known about its role in multiple myeloma (MM). Clinical findings that anti-adrenergic β-blocker intake reduces risk of disease-specific death and overall mortality in patients with MM have indicated that adrenergic input may worsen myeloma outcome. However, preclinical studies using β-adrenergic receptor agonists or antagonists produced controversial results as to whether sympathetic pa…

Electroceutical Targeting of the Autonomic Nervous System.

Autonomic nerves are attractive targets for medical therapies using electroceutical devices because of the potential for selective control and few side effects. These devices use novel materials, electrode configurations, stimulation patterns, and closed-loop control to treat heart failure, hypertension, gastrointestinal and bladder diseases, obesity/diabetes, and inflammatory disorders. Critical to progress is a mechanistic understanding of multi-level controls of target organs, disease adaptation, and impact of neuromodulation to restore organ function.

Autonomic Nervous System Dysfunction in Pediatric Sepsis.

The autonomic nervous system (ANS) plays a major role in maintaining homeostasis through key adaptive responses to stress, including severe infections and sepsis. The ANS-mediated processes most relevant during sepsis include regulation of cardiac output and vascular tone, control of breathing and airway resistance, inflammation and immune modulation, gastrointestinal motility and digestion, and regulation of body temperature. ANS dysfunction (ANSD) represents an imbalanced or maladaptive response to injury and is prevalent in pediatric sepsis. Most of the evidence on ANSD comes from studies o…

Multiple sclerosis and the autonomic nervous system.

Symptoms related to alterations of the autonomic nervous system are frequent in patients with multiple sclerosis (MS). Bladder or bowel dysfunction or impairment of sexual performance is highly distressing for most MS patients, whereas the clinical relevance of other autonomic symptoms is less clear. Cardiovascular autonomic alterations might relate to clinical signs such as orthostatic intolerance; a relationship with fatigue is uncertain. However, the frequency of abnormal findings in tests for the cardiovascular autonomic system varies due to the lack of standardized test performance or diff…

Autonomic nervous system involvement in Behçet's disease.

In this study, R-R interval variation (RRIV) and sympathetic skin response (SSR) were used to evaluate autonomic nervous system (ANS) involvement in 25 patients with Behçet's disease. Normative values of RRIV and SSR were determined in a group of 25 healthy volunteers. R-R interval variation at rest (R%) and during deep breathing (D%), the difference between D% and R% (D-R), and the ratio of D% to R% (D/R) were determined. R-R interval variations did not show a significant difference between patients and the control group. There was a negative correlation with age for R%, D%, and D-R in the co…
