Sickle cell trait (SCT) for years has been a controversial disorder within the aerospace medicine community. Some authorities consider SCT to be an entirely benign condition without particular significance, at least to moderate altitudes. Others are in disagreement and consider it a serious enough threat that flying duties should be precluded for all individuals with this disorder. These issues are addressed by reviewing SCT, its relationship to altitude, and its risk to aviation. The author concludes that flying organizations must establish their own policy on flying status for individuals wi

The structure and features of the smallpox virus are described and an account is given of the clinical picture of the disease. Tables and graphs are cited to illustrate the progress of the WHO's eradication campaign over the last 10 years. In dealing with part played by the aeroplane in the epidemiology of smallpox, attention is given to the high diffusion potential of the disease owing to increased air travel and the confluence of multi-ethnic and multi-national groups in airports. Prevention should thus be concentrated on airports as opposed to the rest of the country, and both aircrews and

BACKGROUND: Physiological episodes are a top safety concern for aviators across the United States military. While many cases and a variety of causes for physiological episodes have been described, few cases, if any, have been reported of hyperventilation-induced hypocapnia and transient loss of consciousness.CASE REPORT: Here we describe a case of an aviator who experienced tingling extremities, confusion, and loss of consciousness during a flight. The aviator incorrectly believed he was experiencing hypoxia and continued to take multiple steps to troubleshoot the wrong underlying problem for

BACKGROUND: Diphyllobothriasis is estimated to afflict 10-20 million people worldwide; however, this is the first case reported in a United States military aviator. Among the largest parasites of humans, the "fish tapeworm" grows from 2-15 m in length, can live >20 yr in the intestines, and is contracted through consumption of uncooked, unfrozen freshwater or anadromous fish species.
CASE REPORT: A 32-yr-old male F-22 pilot presented with mild stomach cramping, bloating, nausea, and intermittent loose stools. Symptoms were relieved with bismuth subsalicylate until several days later when the

OBJECTIVE: Hearing loss among aviators is often reported but it still remains unclear whether this hearing loss occurs as a consequence of the surrounding noise or aging. The effect of flight noise on hearing loss in Turkish aviators is evaluated.
METHODS: Audiometric tests were performed in 300 cadets assessed for entry to the Air Force Academy, 107 experienced aviators undergoing periodic health examination, and 100 people for control group. Audiometric test results were compared between the Academy cadets and the experienced aviators and between the Academy cadets and the control group. Th

Dystonia is both a symptom and the name for group of illnesses called the dystonias. The physical manifestation consists of sustained, involuntary contractions of the muscles in one or more parts of the body, resulting in twisting or distortion of that part of the body. For focal dystonias including torticollis, blepharospasm and spasmodic dysphonia, botulinum toxin injections have become the treatment of choice because of the ability of this toxin to sufficiently weaken the muscle to reduce the spasm but not so much as to cause paralysis. This paper involves the fate of four airmen all afflic

Erythema nodosum is an uncommon skin eruption frequently associated with underlying infections, systemic diseases, or drug toxicity. We report the case of a 27-year-old aviator with three occurrences of erythema nodosum attributed to streptococcal pharyngitis. We review the causes of erythema nodosum to emphasize the importance of a thorough investigation to rule out serious underlying disease prior to returning an aviator to flight status.

An analogy based on the likenesses of the APH-6 aviator's protective helmet and the hangman's noose has some interesting relationships to aircraft accident investigation and to the associated traumatic pathology. A superficial examination of the similarities might propose that the inferior edge of the helmet, when considered part of the continuous circle completed by the nape strap and the chin strap, forms a loop which can be likened to a hangman's noose. The analogy might be extended to the comparison of lesions made about the neck by the straps or the edge of the helmet compared with abrasi

During the period 1969-1975, 12 disqualified U.S. Army helicopter aviators with various finger amputations were evaluated by the U.S. Army Aeromedical Consultation Service for return to flying duties. The evaluations included both medical examinations and in-flight performance observed by aviation medicine specialists and standardization instructor pilots. The criteria for aeromedical disposition are presented with a discussion of the hand and finger requirements to fly U.S. Army UH-1 rotary wing aircraft. All evaluated aviators were presented to a board of senior U.S. Army flight surgeons, wh

Hypoxia represents a major physiological challenge in aviation, often underestimated due to its subtle onset and individually variable symptoms. This article summarizes the physical basis of decreasing oxygen partial pressure with altitude and its impact on human performance. Even at moderate altitudes (2500-4000 m), cognitive functions, reaction times, and visual performance decline significantly-often without the affected person noticing. At higher altitudes, time of useful consciousness rapidly decreases, especially in cases of sudden decompression. Although the body initiates compensatory

Noise is defined as "unwanted sound." Aircraft noise is one, if not the most detrimental environmental effect of aviation. It can cause community annoyance, disrupt sleep, adversely affect academic performance of children, and could increase the risk for cardiovascular disease of people living in the vicinity of airports. In some airports, noise constrains air traffic growth. This consensus paper was prepared by the Impacts of Science Group of the Committee for Aviation Environmental Protection of the International Civil Aviation Organization and summarizes the state of the science of noise ef

"Simulator Sickness" describes a symptom complex frequently reported by pilots during or after flight simulator training. There were 112 helicopter pilots at a U.S. Army AH-1 Cobra Flight Weapons Simulator (FWS) who completed a symptom-oriented subjective questionnaire. Of these, 40% reported symptoms of dysequilibrium; pilots developing simulator sickness had significantly more total and AH-1 flight time. Adaptation to the syndrome occurred with increasing FWS experience. The history and aeromedical significance of simulator sickness are briefly reviewed, and a case report presented. A mandat

Aviation medicine has researched many important facts on pilots, but little on direct relationships between controllers, aviation medicine and air safety. The unsuspecting flying public accepts a 'blind faith' in aircraft and pilots, unaware that aircraft are controlled within 'suspect' ATC systems. The deceptive simplicity of controlling air traffic in apparently limitless skies belies the complexity of man-machine ATC systems operated in ever-crowded airspace, sometimes with antiquated equipment and indifferent communications. The indivisible operational controller/pilot team strives to meet

A detailed situation awareness of the local environment is essential for safe flight in General Aviation. When operating under Visual Flight Rules, eyesight is crucial for maintaining situation awareness and objects may be overlooked. Technical solutions such as Flarm have been sought, but they only work on a basis of co-operation: obstacles without the proper equipment are invisible. Recent developments in the field of radar technology, partly empowered by the demand for sensors for autonomous cars, have improved the size and power consumption of available hardware. Today, the hardware exists

Confidentiality, considered a fundamental principle of medical ethics, is potentially at odds with public safety when an airplane pilot is experiencing symptoms of mental illness. Reporting requirements for pilots vary across the world, and pilots can be subject to multiple national regulatory authorities for international flights. Stigma exists about getting treated for depression. If one's livelihood and core sense of identity is placed at risk by seeking treatment, people will naturally avoid assessments and engagement with mental health clinicians, the very treatment that could mitigate ri

INTRODUCTION: Until recently, glaucoma requiring treatment was disqualifying for U.S. pilots and required an aeromedical special issuance waiver. Since 2013 Aerospace Medical Examiners (AMEs) have been authorized to evaluate third-class pilots with mild glaucoma using similar protocols and issue medical certificates without a waiver if these criteria specified in the AME Guide are met.
METHODS: The FAA's medical database was searched for pilots with glaucoma between 2005 and 2014. The National Transportation Safety Board (NTSB) accident database was then searched to determine which pilots wit

Aviation alters the composition of the atmosphere globally and can thus drive climate change and ozone depletion. The last major international assessment of these impacts was made by the Intergovernmental Panel on Climate Change (IPCC) in 1999. Here, a comprehensive updated assessment of aviation is provided. Scientific advances since the 1999 assessment have reduced key uncertainties, sharpening the quantitative evaluation, yet the basic conclusions remain the same. The climate impact of aviation is driven by long-term impacts from CO2 emissions and shorter-term impacts from non-CO2 emissions
