[Central venous catheterization via the axillary vein].

Central venous catheterization is a common procedure in the intensive care unit. The vessels usually selected for access include those of the arm and the external and internal jugular, subclavian and femoral veins. We find the axillary vein also suitable. It is a safe and reliable route with few complications, and is especially recommended in ventilated and/or tracheotomized patients. We describe our experience in 80 patients, with a success rate of 90% and very few complications.

Axillary and subclavian vein stenosis: percutaneous angioplasty.

Twenty-nine percutaneous balloon dilations of the axillary and subclavian veins were performed in 19 patients. Stenoses occurred in typical locations of anatomic narrowing or at sites of previous trauma. The initial success rate was 76%, with a 1-year patency rate of 35% and a 2-year patency rate of 6%. Angioplasty can be performed on an outpatient basis with a very low rate of significant complications and can be repeated numerous times to keep a vein patent for many years. This procedure is especially valuable in dialysis patients who have limited access sites.

Urokinase perfusion for axillary-subclavian vein thrombosis.

We report a single case of urokinase perfusion of axillary-subclavian vein thrombosis with the restoration of normal venous outflow of the arm. It is hoped that this will halt the usual sequelae causing chronic venous insufficiency.

Percutaneous axillary vein approach in pediatric pacing: comparison with subclavian vein approach.

AIMS: The subclavian vein approach has been used for 20 years in our center for pacemaker (PM) implantation in children, but it carries risks of hemothorax/pneumothorax and lead fracture, which could be reduced by axillary vein approach. METHODS AND RESULTS: This is a prospective study enrolling the first 48 consecutive pediatric patients (age: 12.3 ± 4.6 years) who underwent PM/implantable cardioverter-defibrillator leads implantation through axillary vein (guided by contrast venography) between 2009 and 2012 (group I). A comparison was made with the outcomes of the subclavian vein approach

Axillary vein approach for pulmonic valvuloplasty in infants with iliac vein obstruction.

Repeated pulmonic balloon valvuloplasty from the axillary vein was performed in two infants with initial transvalvular gradients of 131 and 162 mm Hg. The first angioplasty was performed through the femoral vein and improved the valve gradient in both patients. Because of obstruction of the iliac system the axillary vein approach was used for the second angioplasty. The transaxillary pulmonic valvuloplasty decreased the gradient to acceptable levels, and no complications were noticed. The axillary vein approach can be the alternative to the femoral one in cases with obstructed iliac system or

Real-time multimodal axillary vein imaging enhances the safety and efficacy of axillary vein catheterization in neurosurgical intensive care patients.

BACKGROUND: Controversy exists regarding the increased safety profile when ultrasound is used for central venous catheters inserted in the subclavian or axillary vein. The critically ill neurosurgical patient presents unique considerations for the optimal central line approach. METHODS: This report is a retrospective chart review of 6 neurosurgical intensive care patients in whom an ultrasound-guided, transpectoral, axillary vein catheterization was attempted. A sterile technique was observed. The anatomy was confirmed using combined transverse, longitudinal, and Doppler flow images. The need

The axillary vein and its tributaries are not in the mirror image of the axillary artery and its branches.

INTRODUCTION: The axillary and cephalic veins are used for various clinical purposes but their anatomy is not fully understood. Increased knowledge and information about them as well as superficial veins in the upper arm would be useful. OBJECTIVE: The aim of this study is to contribute to the literature regarding the anatomy of the venous drainage of the upper extremity. METHODS: The veins of forty upper extremities from twenty one adult cadavers were injected and their axillary regions dissected. The course and pattern of drainage of the venous tributaries in the axillary region were ident

Anatomical study of the accessory axillary vein in cadavers: a contribution to the axillary surgical approach.

BACKGROUND: The axillary vein is an important blood vessel that participates in drainage of the upper limb. Some individuals present a second axillary vein (accessory axillary vein), which is an important collateral drainage path. OBJECTIVES: The goal of this study was to determine the incidence of the accessory axillary vein and to describe this vessel's topography. METHODS: In this study, axillary dissections were carried out on twenty-four (24) human cadavers of both sexes that had been fixed with 10% formaldehyde. The upper limbs of the cadavers were still attached to the bodies and the

How to make the axillary vein larger? Effect of 90° abduction of the arm to facilitate ultrasound-guided axillary vein puncture.

PURPOSE: Placement of central venous catheters by the infraclavicular route can be achieved by ultrasound-guided puncture of the axillary vein. However, in some cases, the axillary vein may be difficult to puncture because it is too deep or too small or because it is collapsing significantly during breathing. The objective of this observational study was to determine the effect of 90° abduction of the arm associated with forward position of the shoulder on axillary vein diameters. MATERIAL AND METHODS: In a group of 30 healthy volunteers and in a group of 40 patients during spontaneous breath

Right axillary vein cannulation for percutaneous cardiopulmonary support.

A 34-year-old male with a past history of permanent inferior vena cava (IVC) filter placement was referred to us for chronic thromboembolic pulmonary hypertension. Percutaneous cardiopulmonary support (PCPS) was required for the lung hemorrhage and reperfusion injury, although the thromboendarterectomy was successfully completed. The arterial cannula was inserted into the femoral artery, and the venous cannula was inserted into the right axillary vein. The patient was weaned from PCPS 1 day after the operation and was discharged 35 days after the operation. Axillary vein cannulation is thought

Primary subclavian-axillary vein thrombosis: consensus and commentary.

Fifteen multiple-choice questions concerning options in the management of primary subclavian-axillary vein thrombosis were discussed by a panel of experts and then voted upon by 25 attending vascular surgeons with a special interest in venous disease and considerable experience in subclavian-axillary vein thrombosis. The large majority favored, or agreed upon: (1) early clot removal for active, healthy patients with a need/desire to use the involved limb in work or sport; (2) catheter-directed thrombolysis as initial therapy; (3) further therapy based on follow-up positional venography; (4) su

Scintigraphic assessment of "effort" axillary-subclavian vein thrombosis.

"Effort" axillary-subclavian vein thrombosis (Paget-Schroetter syndrome) is an uncommon deep venous thrombosis due to repetitive activity of the upper limb. Clinical suspicion is usually confirmed by contrast venography. This report describes a case where the diagnosis was made by radionuclide venography (Tc-99m DTPA). The examination was carried out in the acute phase and clearly demonstrated the presence and the site of the obstruction as well as the collateral vessels. Despite a complete clinical recovery, obtained by medical therapy, further examinations performed in the follow-up period s

Fibrinolytic therapy for idiopathic subclavian-axillary vein thrombosis.

We report our results in eight consecutive patients with idiopathic subclavian-axillary vein thrombosis treated at a community hospital with systemic streptokinase therapy. Seven of the eight patients were treated within 1 week of symptoms. All seven patients had partial or total recanalization documented by venography. One patient developed rethrombosis that did not respond to therapy with tPA and had mild persisting symptoms of postphlebitic syndrome. None of the other patients had symptoms of postphlebitic syndrome on follow-up up to 5 years' duration.
