[Current views on "hangman's fracture"].

The international literature calls "Hangman's fracture" (HF) the injury of the upper cervical spine with characteristic lesions of the epistropheus. The typical bone lesion in this injury is the bilateral fracture of the penduncles of the epistropheus, the dislocation of the arch, the luxation between CII and CIII and the injury of the disk, by chanche other additional fractures of the vertebrae CII and CIII too. Two forms are known: that of hyperextension distraction mechanism, leading to the classical death by hanging with very severe neurological lesion, and that with hyperextension compres

The anterior retropharyngeal approach to the upper part of the cervical spine.

Since 1959, we have used a superior extension of the anterior approach to the cervical spine of Robinson and Smith in a consecutive series of seventeen patients. This approach provided anterior access to the neural elements from the clivus to the body of the third cervical vertebra, without the need for posterior dissection of the carotid sheath or entrance into the hypopharynx or oral cavity. It also provided adequate exposure for the insertion of iliac or fibular strut grafts, which was necessary in thirteen patients. The approach gave excellent exposure for anterior intralesional excision o

[Little known sequelae of sprains of the cervical spine].

After cervical sprain not only pain and neuropsychological disturbances may occur, but also the following sequelae: cervical dystonia, and torticollis, dizziness, hearing loss for low frequencies, dysphonia and globus. Except for dystonia the symptoms often respond to manipulation of a blocked articulation between occiput and atlas or axis and the third cervical vertebra.

Mandible and tongue-splitting approach for giant cell tumor of axis.

In two cases with a giant cell tumor of the axis, the mandible and tongue-splitting approach permitted excision of the tumor and anterior fusion of the spine from the atlas to the third cervical vertebra. With this approach, there is no need to extend or rotate the cervical spine during the operation. In addition, the operative field extends from the clivus to the fourth cervical vertebra, and safe and sufficient anterior decompression is possible. Although this approach has some disadvantages, contemporary techniques of intravenous hyperalimentation and postoperative respiratory management pr

[Treatment of fractures of the upper cervical spine (C1 and C2)].

The authors provide a review of current opinions on the treatment of fractures in the region of C1 and C2. It is necessary to take the possibility of this localization of fractures into consideration in polytraumatized patients, in those unconscious, and in cases of algesia in the cervical region. A significant post in diagnostics belongs to CT examination which reveals the extent and type of fracture and the degree of dislocation. The authors refer to the classification of Aebi and Nazarian which should be accepted in spondylo-surgical practice. The authors recommend conservative therapy of a

Implantation of C2 prosthesis with dorsal fusion C0-C4 due to pathologic C2 fracture. Case report and literature review.

INTRODUCTION: Pathological destruction of the axis vertebra leads to a highly unstable condition in an upper cervical spine. As surgical resection and anatomical reconstruction of the second cervical vertebrae represents a life threatening procedure, less radical approaches are preferred and only few cases of C2 prosthesis are described in literature. CASE DESCRIPTION: The focus of this case report is a 21-year-old man with a pathological fracture of C2 managed primarily surgically with the C1-C3 dorsal fusion. Due to the progression of giant cell tumor and destruction of the axis vertebra, C
