MicroRNAs fingerprint of bicuspid aortic valve.

Aortic valve tissue is largely exposed to high blood flow. Cells belonging to aortic valve tissues are able to detect and respond to flow conditions changes. Bicuspid aortic valve (BAV) presents altered morphology, with only two abnormal cusps instead of three. This results in an alteration of blood flow dynamics on valve cusps and aortic wall, which may, in turn, increase the risk to develop aortic stenosis and/or regurgitation, endocarditis, aortopathy and/or aortic dissection. MicroRNAs (miRNAs) are short RNA strands regulating gene expression mainly through the inhibition of their target m…

Bicuspid aortic valve: Basics and beyond.

Bicuspid aortic valve, one of the most common congenital cardiac malformations, is associated with other valvular abnormalities and aortopathies. It increases the risk of thoracic aortic aneurysm and dissection, infective endocarditis, and sudden cardiac death. Patients require regular surveillance for progressive valvular disease and aortopathy. Although bicuspid aortic valve may occur sporadically, many cases are inherited, so first-degree relatives should be evaluated and genetic counseling considered.

Modified reimplantation of bicuspid aortic valve.

We present a modified bicuspid aortic valve reimplantation operation using a unique technique of second line, continuous suturing during implantation of the aortic valve scallop to the prosthesis that mimics stentless, minicylinder implantation.  The feasibility of bicuspid aortic valve repair with associated aortic root management has been demonstrated. However, this repair seems to be less durable than tricuspid aortic valve repair, and this may be partly because of the connective tissue disorders that are an inherent feature of bicuspid aortic valve disease.  Annular progressive dilatation …

Genetics of bicuspid aortic valve aortopathy.

PURPOSE OF REVIEW: The incidence of aortic dilation and acute complications (rupture and dissection) is higher in patients with a bicuspid aortic valve (BAV), the most frequent congenital heart defect. The present review focuses on the current knowledge in the genetics of BAV, emphasizing the clinical implications for early detection and personalized care. RECENT FINDINGS: BAV is a highly heritable trait, but the genetic causes remain largely elusive. NOTCH1 is the only proven candidate gene to be associated with both familial and sporadic BAV. Other genes have been reported to be associated w…

Bicuspid aortic valve in competitive athletes.

AIM: Bicuspid aortic valve (BAV) is a common congenital cardiac condition. The presence of BAV in non-elite athletes has been poorly investigated; it is usually asymptomatic until valvular stenosis, regurgitation or other vascular alterations are evident. DESIGN: Over a three-year period, 2273 competitive athletes were consecutively investigated with transthoracic echocardiography. The traditional parameters, the aortic root dimensions at four levels and the systolic and diastolic flow of aortic valve, were studied with continuous Doppler according to the echo guidelines. SETTING: The study …

Vascular malformation in a bicuspid aortic valve.

We report here a case of capillary vascular malformation developed in a bicuspid aortic valve incidentally detected during valve replacement in a 67-year-old male patient. The International Society for the Study of Vascular Anomalies (ISSVA) classification was used to classify this vascular lesion instead of using the term hemangioma. The differential diagnosis and the literature are reviewed.

Embryonic Development of the Bicuspid Aortic Valve.

Bicuspid aortic valve (BAV) is the most common congenital valvular heart defect with an overall frequency of 0.5%-1.2%. BAVs result from abnormal aortic cusp formation during valvulogenesis, whereby adjacent cusps fuse into a single large cusp resulting in two, instead of the normal three, aortic cusps. Individuals with BAV are at increased risk for ascending aortic disease, aortic stenosis and coarctation of the aorta. The frequent occurrence of BAV and its anatomically discrete but frequent co-existing diseases leads us to suspect a common cellular origin. Although autosomal-dominant transmi…

The bicuspid aortic valve and related disorders.

Bicuspid aortic valve (BAV) is the most common congenital cardiac malformation, affecting 1-2% of the population, with strong male predominance. Individuals may have a normally functioning BAV, and may be unaware of its presence and the potential risk of complications. However, they may easily develop aortic valve disorders: either stenotic or regurgitant, or both. Today, BAV is recognized as a syndrome incorporating aortic valve disorders and aortic wall abnormalities, including aortic dilation, dissection or rupture. Congenital or hereditary diseases such as ventricular septal defect, patent…

Outcomes in adults with bicuspid aortic valves.

CONTEXT: Bicuspid aortic valve is the most common congenital cardiac anomaly in the adult population. Cardiac outcomes in a contemporary population of adults with bicuspid aortic valve have not been systematically determined. OBJECTIVE: To determine the frequency and predictors of cardiac outcomes in a large consecutive series of adults with bicuspid aortic valve. DESIGN, SETTING, AND PARTICIPANTS: Cohort study examining cardiac outcomes in 642 consecutive ambulatory adults (mean [SD] age, 35 [16] years; 68% male) with bicuspid aortic valve presenting to a Canadian congenital cardiac center …

Repair of traumatic tricuspid regurgitation by bicuspidization.

Traumatic tricuspid regurgitation is a fairly rare complication following blunt chest trauma but not as uncommon as it was believed to be before the advent of transthoracic echocardiography. We report a case of severe tricuspid regurgitation with ruptured chordae tendineae 10 years after blunt chest trauma that was successfully repaired with a simple method. The operative technique consisted of plication of the anteroposterior commissure to create a bicuspid valve, along with ring annuloplasty.

The bicuspid aortic valve--an autopsy study.

Congenital Bicuspid Aortic Valve is a common congenital anomaly. We studied 48 cases of bicuspid aortic valve seen over a period of 20 years. 18 cases were associated with other congenital anomalies, patent ductus arteriosus being the most frequent. 30 were isolated lesions. The morphology of the cusps and position of raphe were noted. Calcification and infective endocarditis were frequently noted in the isolated forms of Bicuspid Aortic Valve. Coronary arteries were dissected in 32 cases to determine the dominance.

Bicuspid pulmonary and bicuspid aortic valve in association with Gasul phenomenon (triple combination): a case report and literature review.

BACKGROUND: Congenital heart disease (CHD) is a significant health concern affecting approximately 1% of live births. Among these anomalies, bicuspid aortic valve (BAV) is the most prevalent, while bicuspid pulmonary valve (BPV) remains exceptionally rare. This case report presents a unique instance of a 10-year-old girl diagnosed with the combination of BAV and BPV alongside a ventricular septal defect (VSD) and infundibular stenosis, referred to as the Gasul phenomenon. CASE PRESENTATION: The patient, initially identified with a heart murmur during infancy, exhibited dyspnea classified as N…

Symmetrical bicuspidization of the aortic valve in a child.

We present the case of a 4-year-old patient who underwent an aortic valve repair and concomitant septal myectomy and ascending aorta enlargement. The aortic valve was severely stenotic, bicuspid with a fused left-right commissure and a rudimentary right coronary leaflet. This case report demonstrates how this problem was repaired with a good result.

Patients with Bicuspid Aortopathy and Aortic Dilatation.

(1) Background: Bicuspid aortic valve (BAV) is the most frequent congenital cardiac disease. Alteration of ascending aorta diameter is a consequence of shear stress alterations due to haemodynamic abnormalities developed from inadequate valve cusp coaptation. (2) Objective: This narrative review aims to discuss anatomical, pathophysiological, genetical, ultrasound, and radiological aspects of BAV disease, focusing on BAV classification related to imaging patterns and flux models involved in the onset and developing vessel dilatation. (3) Methods: A comprehensive search strategy was implemented…
