[Reoperation in biliary atresia].

From July 1976 to February 1988 52 cases of extrahepatic biliary atresia were treated at our institution and 16 patients underwent reoperation. The original procedure were portojejunostomy in 11 cases (68 for 100), portocholecystostomy in four (25 for 100) and omento-duodenopexy in one (6 for 100). Age average at operation was 61 days; twelve had bile excretion. The reoperation was performed due to absence of bile drainage in three cases, cholangitis in 9 and bile peritonitis in four. Surgical procedures used at reoperation were: excision of scar tissue at the porta hepatis (12 patients) and p…

[CONGENITAL MALFORMATION: BILIARY ATRESIA].

Congenital anomalies of biliary tract development are frequent causes of lingering course of physiologic jaundice which need timely verification of diagnosis followed by surgical treatment. Clinical observation of biliary atresia in a 10-month-old girl permits to accent pediatricians’ attention to peculiarities of clinical picture, diagnosis and techniques for treatment of this pathology.

Biliary atresia: US diagnosis.

PURPOSE: To evaluate prospectively the sensitivity of ultrasonography (US) in the diagnosis of biliary atresia (BA), with surgery as the reference standard. MATERIALS AND METHODS: After institutional ethical approval and with informed parental consent, 90 consecutive fasting infants with conjugated hyperbilirubinemia underwent detailed US studies performed by a single operator with a 7.5-MHz curvilinear transducer and a 13.5-MHz linear-array transducer. The following features were prospectively recorded: gallbladder morphology, triangular cord sign, presence of a common bile duct, liver size …

Congenital extrahepatic biliary atresia.

Ten cases of congenital extrahepatic biliary atresia were studied ultrastructurally. Samples of liver were obtained from each and in six of the cases, fibrous tracts, which we hoped would contain extrahepatic bile ducts, were also secured. The observed extrahepatic biliary structures were real, but hypoplastic, bile ducts. In places, necrosed epithelial cells, without obvious inflammatory processes, could be observed. The ductular cell cytoplasmic changes and the inflammatory reaction are different according to whether extrahepatic or intrahepatic sites are considered. These differences, as we…

The Canadian Biliary Atresia Registry: Improving the care of Canadian infants with biliary atresia.

Biliary atresia is the most common cause of end-stage liver disease and liver cirrhosis in children, and the leading indication for liver transplantation in the paediatric population. There is no cure for biliary atresia; however, timely diagnosis and early infant age at surgical intervention using the Kasai portoenterostomy optimize the prognosis. Late referral is a significant problem in Canada and elsewhere. There is also a lack of standardized care practices among treating centres in this country. Biliary atresia registries currently exist across Europe, Asia and the United States. They ha…

Senescence and senotherapies in biliary atresia and biliary cirrhosis.

BACKGROUND: Premature senescence occurs in adult hepatobiliary diseases and worsens the prognosis through deleterious liver remodeling and hepatic dysfunction. Senescence might also arises in biliary atresia (BA), the first cause of pediatric liver transplantation. Since alternatives to transplantation are needed, our aim was to investigate premature senescence in BA and to assess senotherapies in a preclinical model of biliary cirrhosis. METHODS: BA liver tissues were prospectively obtained at hepatoportoenterostomy (n=5) and liver transplantation (n=30) and compared to controls (n=10). Sene…

Sibling occurrence of biliary atresia and biliary dilatation.

Familial occurrence of extrahepatic biliary atresia (EHBA) or biliary dilatation has been reported; however, the genetic influences on these disorders are still obscure. The authors previously reported the occurrence of biliary dilatation and EHBA in sisters, which is extremely rare. Recently, a similar pair of sisters was treated; one had pancreaticobiliary maljunction with minimal dilatation of the common bile duct, and the other also had EHBA. The authors' investigations suggest that there may be common etiologic factors for EHBA and biliary dilatation, such as pancreaticobiliary maljunctio…

Type I biliary atresia without extrahepatic biliary cyst.

Currently, magnetic resonance cholangiography (MRC) is used for the differentiation of biliary atresia (BA) from other causes of infantile cholestasis. The authors present a case of type I BA without an extrahepatic biliary cyst in a 2-month-old girl. MRC clearly visualized the patency of the gallbladder, cystic duct, and hepatic ducts with disappearance of the common bile duct. Intraoperative cholangiography demonstrated a cloudy appearance of the intrahepatic bile ducts, confirming the diagnosis of type I BA. We believe that this is the first reported case of type I BA without an extrahepati…

Hepato-biliary Scintigraphy in diagnosis of Biliary Atresia.

BACKGROUND: Biliary Atresia and Neonatal Hepatitis are the two major causes of Persistent Neonatal Jaundice. Differentiation is done by biochemical and radiological tests. Radiological investigations use intra-or extra-hepatic biliary dilation for diagnosing biliary atresia but this is not always reliable. METHODS: 14 neonates with persistent conjugated hyperbilirubinemia who had undergone hepato-biliary scintigraphy were retrospectively evaluated and those having Extrahepatic Biliary Atresia were analyzed with reference to operative findings. RESULTS: 11 out of 14 had Extrahepatic Biliary A…

Analysis of the biliary transcriptome in experimental biliary atresia.

BACKGROUND & AIMS: Discovery of the pathogenic mechanisms of biliary atresia has been limited by the inability to study extrahepatic biliary tissues from patients at early phases of disease. Here, we used a rotavirus-induced model of biliary atresia to investigate the entire biliary transcriptome for molecular networks activated at the onset and different phases of progression to duct obstruction. METHODS: We injected Balb/c mice with saline or rotavirus intraperitoneally within 24 hours of birth, microdissected the gallbladder and extrahepatic bile ducts en bloc 3, 7, and 14 days later, gene…

[Histopathology of biliary atresia: correlation with biliary flow].

The objective of KASAI's operation is to obtain a postoperative bile excretion and to maintain an internal biliary fistula. The purpose of this report is to study the relationship between the liver histopathology and the postoperative bile flow and survival. From july 1976 to january 1990, 66 patients underwent a corrective operation for biliary atresia. In 24 patients we performed a KASAI's operation with internalization of the biliary conduit, 36 with externalization, four gall-bladder KASAI and two liver biopsy. The mean age at operation was 66 days and postoperative bile excretion was obta…

Biliary Atresia Associated With Jejunal Atresia: A Case Report.

A girl who was born at 40 weeks of gestation weighing 3800 g presented with bilious vomiting and abdominal distension shortly after birth. A lower gastrointestinal contrast study showed a microcolon with small bowel atresia. Subsequently, laparotomy, small bowel resection and anastomosis were done. Intra-operative findings noted jejunal atresia type 3a. Post-operatively, the patient developed persistent conjugated hyperbilirubinaemia and hence, magnetic resonance cholangiopancreatography (MRCP) was performed. MRCP revealed possible biliary atresia (BA) of which the patient underwent Kasai hepa…

Preoperative evaluation of biliary atresia.

Biliary atresia can be a challenging diagnosis to make as there is no single definitive diagnostic laboratory or imaging study available and no single agreed upon diagnostic algorithm. The purpose of this article is to review the complex puzzle of clinical, laboratory, and imaging studies that aid in the evaluation of infants suspected of having biliary atresia. We have reviewed historical and current manuscripts and society guidelines, added our own experience in evaluating infants for biliary atresia, and then summarized the findings to provide a concise review of what we feel is the modern …
