Transcatheter Mustard Revision Using Endovascular Graft Prostheses.

We describe the use of percutaneously inserted, transcatheter endovascular graft prostheses to exclude large Mustard baffle leaks in a high-surgical-risk patient. We used 3-dimensional-printed models to determine feasibility and to plan the procedure. Telescoping thoracic and abdominal graft extensions were placed in the inferior and superior limbs of the systemic venous pathways. An atrial septal defect occluder device was also used to close a separate leak not covered by the endovascular graft prostheses. This approach may be useful in patients with complex, large intraatrial baffles that re

Single arterial access ECMELLA: A new concept and step-by-step procedure.

Therapy for cardiogenic shock using temporary mechanical circulatory support has improved significantly in the last decades, providing patients with new technologies for both acute phase stabilization and bridging to long-term therapies. A combination of a venoarterial extracorporeal life support system and the Impella left ventricular assist device (known as the ECMELLA approach) represents an effective therapy for severe cardiogenic shock that achieves high-flow circulatory support with simultaneous left-ventricular unloading. We present the new ECMELLA 2.0 concept, whereby a single arterial

Status of Branched Grafts for Thoracic Aortic Arch Endovascular Repair.

The aortic arch is one of the most anatomically complex segments of the thoracoabdominal aorta due the plurality of vital branch configurations, intrinsic and highly variable three-dimensional curvature, and involvement in a variety of thoracic aortic pathologies, such as aneurysmal degeneration, penetrating ulcer, and media wall dissection. Pathologies that extend the repair proximal to the left common carotid artery require more complex branch vessel management beyond a carotid-subclavian bypass. This article will review the current status of branched aortic arch endografts.

Valve-sparing aortic root replacement in Marfan syndrome.

Marfan syndrome is the most common inherited connective tissue disorder, affecting approximately 1 in 10,000 live births. The cardinal features of Marfan syndrome are the abnormalities of the skeleton (tall stature, arachnodactyly, and joint hyperelasticity), eye (lens subluxation), and aorta (root aneurysm with proclivity toward rupture and dissection). Aortic catastrophe accounts for most of the premature mortality among Marfan patients, a risk that climbs steeply during adolescence and results in death of half of Marfan patients by the age of 40 years. Most of the improvement in life expect

In situ stent-graft fenestration to preserve the left subclavian artery.

PURPOSE: To report our first clinical application of a new technique for in situ fenestration of a thoracic stent-graft. CASE REPORT: After completing a series of in vitro and in vivo experiments, in situ stent-graft fenestration was employed during endograft repair of a saccular thoracic aortic aneurysm in a 77-year-old woman. Because the stent-graft would have covered the left subclavian artery ostium, a modified Zenith TX1 thoracic stent-graft was deployed then fenestrated transluminally using a guidewire followed by serial cutting balloons, which created a fenestration over the LSA suffic

Frozen Elephant Trunk in Aortic Arch Disease: Different Devices for Different Pathologies.

The frozen elephant trunk technique (FET) requires the use of a pre-assembled hybrid prosthesis consisting of a standard Dacron vascular portion to replace the aortic arch and a stent graft component, which is placed into the proximal descending thoracic aorta (DTA) anterogradely in the proximal descending thoracic aorta. In Europe, two hybrid prostheses are available: the E-evita Open Plus hybrid stent graft system provided by JOTEC (Hechingen, Germany) and the ThoraflexTM Hybrid (Vascutek, Inchinnan Scotland). Recommendations for use are extensive pathologies of the arch in case of acute and

Complete closure of coronary fistula communicating with the descending aorta.

We report a rare case of complex coronary fistula that contained two aneurysms and was connected to the descending aorta. The fistula was also associated with further aortic pathologies necessitating surgery. Preoperative computed tomography angiography, which revealed an abnormal vessel communication, was crucial for choosing the appropriate operative strategy. During a one-stage procedure comprising closure of both arterial (aortic and coronary) fistula origins and repair of the aortic valve and thoracic aorta, the coronary origin of the fistula was ligated and the frozen elephant trunk tech

Open descending aortic replacement after Thoraflex™ hybrid graft implantation.

The Thoraflex™ device is a composite 4-branched graft with a distal endovascular stent that allows either a one-stage treatment or an endovascular second-stage treatment of multisegment aortic pathologies. Reports on open second-stage treatment after the Thoraflex™ device implantation are sparse especially regarding the technical aspects. We described our surgical procedure of open second-stage descending thoracic aortic replacement after Thoraflex™ implantation with a special focus on the anastomosis technique to a stent graft. We found excellent haemostasis and a good postoperative outcome u

Patient-specific computational simulation of coronary artery bifurcation stenting.

Patient-specific and lesion-specific computational simulation of bifurcation stenting is an attractive approach to achieve individualized pre-procedural planning that could improve outcomes. The objectives of this work were to describe and validate a novel platform for fully computational patient-specific coronary bifurcation stenting. Our computational stent simulation platform was trained using n = 4 patient-specific bench bifurcation models (n = 17 simulations), and n = 5 clinical bifurcation cases (training group, n = 23 simulations). The platform was blindly tested in n = 5 clinical bifur

How I do it: re-do surgery for recurrent trigeminal neuralgia.

BACKGROUND: When trigeminal neuralgia recurs after microvascular decompression surgery, re-operation may be indicated if trigeminal nerve compression remains. However, surgery for recurrent trigeminal neuralgia is more difficult because of adhesions between the prosthesis, such as the Teflon sling, and the surrounding structures, including the blood vessels, in addition to common post-open surgery adhesions. METHOD: A case of a patient with recurrent trigeminal neuralgia is presented. CONCLUSION: Preoperative evaluation of the compression of the trigeminal nerve is important. The pre-existin

The Current State of Physician-Modified Endovascular Grafts in Complex Abdominal Aortic Aneurysms.

Physician-modified endografts (PMEGs) for the treatment of complex abdominal aortic aneurysms and thoracoabdominal aortic aneurysms are a viable option. Other endovascular options include custom and off-the-shelf devices for fenestrated and branched endovascular aortic repair, parallel grafts, and in situ laser fenestration. The limitations of these devices include time to development, strict anatomic criteria, and durability regarding parallel grafts. PMEGs fill this void with perioperative and long-term outcomes similar to custom-made devices. Postdissection aneurysms also present a unique r

Update on Trials & Devices for Endovascular Management of the Ascending Aorta and Arch.

Thoracic endovascular aortic repair (TEVAR) may treat a variety of acute and chronic aortic diseases as described in several articles in this issue of TVIR. A major challenge to endovascular treatment in the thoracic aorta is disease involving or in close proximity to the aortic valve, coronary arteries, or aortic arch branch vessels. Disease involving the ascending aorta in particular has significant limitations based on its distance from the aortic root. The left subclavian artery (LSA) can be covered in the emergent setting to ensure an adequate landing zone, but patients may require later

Endovascular aneurysm sealing addresses several limitations of conventional endovascular aneurysm repair.

Endovascular aneurysm repair has enabled a broad population of patients with infrarenal abdominal aortic aneurysm to be treated by a less-invasive technique. However, endovascular aneurysm repair has therapeutic limitations, including the need for lifelong surveillance and a higher rate of secondary interventions than open repair. These outcomes can promote patient dissatisfaction and result in increased health care costs and associated morbidity and mortality. The primary reason for secondary interventions is continued abdominal aortic aneurysm sac enlargement due to endoleaks. Conventional e
