[Fractography of the long tubular bones of the lower extremities in trauma by blunt objects].

New data on micromorphological signs of fracture of long tubular bones in normal distribution and transformation of deforming forces are presented. Fractographic signs of bone for each destructive stage, signs of repeated trauma are singled out. Interrelationship between bone microstructure and conditions of its destruction is stated.

Functional fracture-bracing of long-bone fractures of the lower extremity in children.

Fifty-six tibial fractures in fifty-five children, sixteen years old or younger, and seventeen femoral fractures in seventeen children in the same age group, seven of them with associated tibial fractures, were treated by fracture-bracing. The tibial fractures healed in an average of 13.2 weeks with no complications that could be attributed to failure of the method. The femoral fractures healed in an average of seventeen weeks. There was an angular femoral deformity of more than 5 degrees in seven of seventeen patients, all but one in fractures of the middle third of the femoral shaft. Of nine

[Plate-assisted bone segment transport for bone defects of the lower extremities : Possibilities and limitations of treatment].

Large bone defects of the lower extremities are challenging for both patients and the treating orthopedic surgeons. The treatment is determined by the size and location of the defect; however, patient-specific factors, such as the soft tissue situation and the presence of systemic comorbidities must be taken into consideration in the treatment strategy. Osteodistraction is an excellent technique especially for large bone defects exceeding 3 cm; however, it is time-consuming and required external fixation prior to the development of motorized distraction nails. This article describes the proced

Treatment of lower extremity long bone nonunion with expandable intramedullary nailing and autologous bone grafting.

BACKGROUND: Nonunion of long bones in lower limbs is a common complication of orthopedic trauma that can be extremely debilitating. This retrospective study describes our experience using expandable intramedullary nails and autologous bone grafting in treating lower limb long bone nonunion with bone defects. METHODS: Nineteen patients (mean age 38.9 years, range 18-61) with lower limb long bone nonunion and defects caused by femoral or tibial fracture types were as follows: A2 (3 femoral, 1 tibial), A3 (1 femoral, 2 tibial), B2 (3 femoral, 4 tibial), and B3 (1 femoral, 4 tibial). Palliative percutaneous stabilization of lower extremity for bone metastasis using flexible nails and bone cement.

OBJECTIVE: Percutaneous stabilization (PS; percutaneous flexible nailing and intramedullary bone cement injection) was performed at lower extremity long bones in patients with multiple bone metastases with short life expectancy to get mechanical stability and local tumor control. We evaluated the usefulness of PS by clinical status, F-18-FDG PET-CT and bone scintigraphy (BS). METHODS: Patients comprised 15 patients (total 20 sites) who had undergone PS for the metastatic bone tumors of lower extremity long bones (femur and tibia). After percutaneous flexible nailing, bone cement was injected

Comparison of vascularised iliac crest and vascularised fibula transfer for reconstruction of segmental and partial bone defects in long bones of the lower extremity.

The results and complications of lower extremity bone reconstruction using microvascular fibula transfer were retrospectively compared with reconstruction using microvascular iliac crest transfer. Seventeen patients matched as much as possible in regard to location of defect (femur, tibia, ankle) and aetiology (traumatic defect, osteomyelitis) were studied in each group. Data collected included number of prior operative procedures, length of bony defect, presence or absence of associated soft tissue loss, the final clinical and radiographic result, technique of bone immobilisation and duration

Salvage of a lower extremity by microsurgical transfer of tibial bone from the contralateral extremity traumatically amputated at the ankle level.

The authors present a case of free vascularized transfer of the tibial bone as an osteomyocutaneous flap based on the posterior tibial vessels. A 42-year-old man presented with severe crush injury to bilateral legs. The left tibial bone and soft tissue defect required reconstruction with an osteomyocutaneous free flap. Since the contralateral leg was traumatically amputated at the level of the ankle, a decision was made to harvest a free tibial osteomyocutaneous flap with below-knee-amputation completed. The transfer was achieved successfully with complete survival of the flap and bony union.

Segmental transports for posttraumatic lower extremity bone defects: are femoral bone transports safer than tibial?

BACKGROUND: The long-term outcomes following femoral and tibial segment transports are not well documented. Purpose of the study is to compare the complication rates and life quality scores of femoral and tibial transports in order to find what are the complication rates of femoral and tibial monorail bone transports and if they are different? METHODS: We retrospectively analyzed the medical records of 8 femoral and 14 tibial consecutive segment transports performed with the monorail technique between 2001 and 2008 in our institution. Mean follow-up was 5.1 ± 2.1 years with a minimum follow-u

Impact of Fluoroscopically Guided Bone Biopsy on Antibiotic Management of Osteomyelitis in the Lower Extremity.

This study evaluated the impact of fluoroscopically guided percutaneous bone biopsy on altering antibiotic regimens in lower extremity osteomyelitis. Eighty-eight patients who received fluoroscopically guided bone biopsies were identified. There was bacterial growth in 28% of bone biopsies overall. The rate of positive culture was decreased in patients started on empiric antibiotics before biopsy (23%) compare with patients without empiric antibiotics (44%). Antibiotic regimens were changed in 24% of patients overall in response to culture data. The majority of positive biopsy cultures (76%) b

[Lower extremity nerves injuries in bone fractures and joint dislocations--etiopathogenesis, treatment].

Lower extremity nerve injuries accompanying bone fractures and joint dislocations lead to serious disturbances at the lower limbs and caused significant prolongation of the treatment time. Clinical material consisted of 73 patients treated surgically with lower extremity nerve injuries due to bone fractures and joint dislocation. Based on this material we analysed the most frequent localisations of the nerves lesions and the mechanism of injury. The indications to surgical treatment were also described.

[Therapy of bone metastases of the lower extremity with the KMFTR modular tumor endoprosthesis system].

During the years 1975 through 1989, 189 patients have been operated because of metastases within bones of the lower extremity. Thereof, 34 patients received an endoprosthesis of the KMFTR (Kotz Modular Femur-Tibia reconstruction system) type. In detail, the following reconstruction elements had been used: proximal femur (n = 20), distal femur (n = 7), complete femur (n = 2), proximal tibia (n = 5). At the time of follow-up examination 30 patients had died. Mean survival time after resection of metastases was 18.6 months (range 8-44 months). According to histologic judgement, in 23 cases wide o

Angular deformity of the lower extremity: evaluation with quantitative bone scintigraphy. Work in progress.

A technique for quantitative analysis of growth plates in varus or valgus deformity about the knee has been developed. Computer-generated regions of interest are placed on magnification scintigraphs to divide distal femoral and proximal tibial physes into four equal segments. The ratio of counts in the medial half to counts in the lateral half was studied in 34 children (23 healthy, 11 with angular deformity). Normal ratios were 0.74-0.98 for femurs and 0.98-1.20 for tibias. An above-normal ratio correlated with the presence of valgus deformity and a below-normal ratio with varus deformity. Th

Lower extremity loads stimulate bone formation in the vertebral column: implications for osteoporosis.

Senile osteoporosis is a growing national health problem. The purpose of this study was to determine whether loads applied through the lower extremity would stimulate bone formation in the vertebral column, the site of most osteoporotic fractures. Female white New Zealand rabbits were divided into experimental (EG) and control (CG) groups. The right hind leg of the EG was repetitively loaded to 1.5 X body weight, 20 min/day for three weeks; the CG was "sham loaded". Bone formation in both groups was labeled using tetracycline. Labeled osteons, resorption spaces, surface bone formation, and por

Updates on Residual Limb Management in Lower Extremity Amputation From Nerve to Bone.

Traumatic extremity amputation can be devastating for patients' functional and psychological health. Challenges of initial management and considerations for limb salvage versus amputation for mangled lower extremities are well documented. However, literature geared toward orthopedic surgeons highlighting management considerations for the residual limb of an amputation is scarce. This article reviews recent literature on management of the residual limb in the perioperative and rehabilitation phases of treatment.

Bone-based versus soft-tissue free flaps in lower extremity reconstruction.

BACKGROUND: Most lower extremity defects and minor bone defect wounds requiring a free flap are treatable with soft-tissue flaps, whereas large bone defect wounds are treated with bone-based flaps. This study aimed to compare bone-based and soft-tissue-free flaps in terms of operative procedures and postoperative complications, including long-term outcomes of lower extremity reconstruction. METHODS: This two-center retrospective cohort study collected data from all lower-extremity reconstructions with free flaps performed between March 2014 and February 2022; the level of evidence is consider
