Spontaneous and reflex movements have been described in brain death and these unusual movements might cause uncertainties in diagnosis. In this study we evaluated the presence of spinal reflexes in patients who fulfilled the criteria for brain death. Thirty-two (22 %) of 144 patients presented unexpected motor movements spontaneously or during examinations. Hence, in the third generation of brain death criteria, the disappearance of evoked potentials is, in our opinion, the best of the new tests. There are, therefore, three tests alternatively available: ECS, EP-abolition and CCA, which permit a shortening of the observation time, before the determination of brain death. This is not valid for newborns. In these tests the difference between supratentorial and infratentorial primary brain lesions has to be considered carefully. In infratentorial lesions the apnea can precede the ECS by several hours.

The authors report on the behavior of the intraocular pressure of 20 patients who had sustained severe head injury. The 8 patients who retained normal intraocular pressure all survived. The 12 patients who developed intraocular hypotony all suffered brain death. Although optic disc edema was not noted in any brain-dead patients, we did find signs of fundus ischemia in most.
