Brain stem abscesses in childhood.

BACKGROUND: Solitary brain stem abscess is a rare condition with high mortality and morbidity. These lesions were considered to be invariably fatal before 1974 when advanced diagnostic tools were not available. Recently, the diagnosis and prognosis of brain stem abscesses have been modified by the introduction of modern radiological tools, and several patients with a favorable outcome have been reported since then. Because the pons is the most common site of the abscesses, involvement of the sixth and seventh nerves is frequent. The midbrain is the second most likely location, and medullary ab…

Brain stem tuberculosis in children.

Tubercular meningitis and intracranial tuberculomas are the two frequent manifestations of neurotuberculosis with their variable incidence in different countries. Brain stem tuberculomas are even more unusual, accounting for 2.5-8% of all intracranial tuberculomas. We present here six paediatric cases of brain stem tuberculosis, where well-formed tuberculomas were demonstrated in five symptomatic cases; however, only a hypodense ill-defined lesion was found in the brain stem of one child who presented with features of tubercular meningitis. Three of these children had other associated tubercul…

[Brain stem encephalitis in childhood].

OBJECTIVE: To present two cases of post-infectious encephalitis of the brain stem (ETC) in infancy, which is very infrequent at this age. CLINICAL CASES: Two patients aged 4 months and 9 months respectively had a previous history of a catarrhal illness a few days before the onset of encephalitis. The clinical condition was of subacute onset and torpid course, characterized by ataxia, reduced level of consciousness, involvement of the pyramidal tracts and paralysis of the cranial nerves. No significant information for the diagnosis of either case was obtained from CT. MR showed lesions at the …

[Prognosis in brain stem infarction].

UNLABELLED: A retrospective study of patients who suffered cerebrovascular accidents (CVA particularly brainstem damage) was conducted to look for risk factors which, present on admission, indicated the chance of survival. The evolution of 100 patients, hospitalized in the Neurology Department, University Hospital Lausanne, after having had their first CVA was studied. The risk factors chosen were age, sex, hypertension, smoker, late onset diabetes, hypercholesterolemia as well as the conscious state, the location of the lesion and whether it was bi- or unilateral. Analysis of the data was per

The effects of experimental brain-stem ischaemia on brain-stem auditory evoked potentials in primates.

We related intracranial auditory brain-stem evoked potentials (BAEPs) to the surface BAEP using a model of focal brain-stem ischaemia. In 17 baboons anaesthetised with alpha-chloralose, BAEPs were recorded bilaterally at the mastoids and in the caudal lateral lemniscus (LL) and inferior colliculus (IC), in response to monaural click stimulation. Electrodes at these sites were each connected to the positive input of a differential amplifier, and one other electrode, placed at the vertex, was connected to all the negative inputs. Measurements of local cerebral blood flow (CBF) by hydrogen cleara

Computed tomography of the brain stem with intrathecal metrizamide. Part I: the normal brain stem.

In summary, the anatomic details of the brain stem are exquisitely demonstrated with metrizamide CT. The obex is the zone of transition from the closed medulla, which has a conical pear-shaped appearance to the quadriconcave open medulla. The pyramids and inferior olivary nuclei characterize the anterior surface of the open medulla, whereas the median sulcus and the cranial nerve nuclei constitute the floor of the fourth ventricle at the same level. The mushroomlike appearance of the pontomedullary junction is easily reproducible with the restiform bodies forming the posteriorly diverging bipe…

Serial observations of brain stem function by auditory brain stem responses in central transtentorial herniation.

Changes in auditory brain stem responses were serially investigated in a patient with downward transtentorial herniation due to acute obstructive hydrocephalus to correlate the neurological signs with the results of computerized tomography. Neurological deterioration correlated highly with disruption of auditory brain stem responses, especially with that of the components of waves V to VII. A noninvasive technique, the measurement of auditory brain stem responses is thought to be useful in detecting the severity of downward transtentorial herniation and in estimating the recovery of brain stem…

Metabonomics Analysis of Brain Stem Tissue in Rats with Primary Brain Stem Injury Caused Death.

OBJECTIVES: To explore the potential biomarkers for the diagnosis of primary brain stem injury (PBSI) by using metabonomics method to observe the changes of metabolites in rats with PBSI caused death. METHODS: PBSI, non-brain stem brain injury and decapitation rat models were established, and metabolic maps of brain stem were obtained by LC-MS metabonomics method and annotated to the HMDB database. Partial least square-discriminant analysis (PLS-DA) and random forest methods were used to screen potential biomarkers associated with PBSI diagnosis. RESULTS: Eighty-six potential metabolic marke…

Brain-stem auditory evoked potentials in supratentorial brain tumors.

A group of 42 adult patients with supratentorial brain tumors were studied with computed tomography (CT or/and NMR) and brain-stem evoked potentials (BAEPs). 12 patients had meningiomas and 30 had malignant tumors (gliomas and metastases). The latencies of wave V and of the interpeak latency I-III, III-V and I-V were evaluated both on the lesion side and the opposite side. In meningiomas mean latencies did not show significant differences from normal, while mean latencies in malignant tumors were significantly prolonged, except for the I-III interpeak latency. The results of this study demonst

Auditory brain-stem response in determination of brain death.

A diagnosis of brain death requires evidence of irreversible destruction (or dysfunction) of neurons in the brain stem and cerebrum. The physical examination is not a valid index of brain integrity in patients who are drug-intoxicated or receiving therapeutic paralyzing agents or high-dose barbiturates. We evaluated the use of the auditory brain-stem response (ABR) as an ancillary test in the determination of brain death. A total of 100 combined ABR and nuclear cerebral blood flow studies were completed at bedside with 81 acute, severely brain-injured adults. The ABR and cerebral blood flow ou…

Application of brain stem response in brain-injured children.

The early components of the auditory evoked potential within 10 msec following an auditory stimulus are attributed to the brain stem auditory nuclei and pathways. In pediatric neurology the auditory brain stem response (ABR) can be applied to: 1) differential diagnosis of hearing impairment in young children including objective threshold measurement of hearing, 2) electrophysiological evaluation of maturation of the auditory pathways, 3) diagnosis of the site and/or extent of neurlogical diseases affecting the brain stem and 4) observation of a degenerating process of degenerative diseases in …

[Benign encephalitis of the brain stem].

INTRODUCTION: The secondary encephalitis disorders are due to an immunological mechanism which causes demyelinating lesions of the central and peripheral nervous systems, with very variable clinical features. The pathogenesis and localization of benign encephalitis of the brain stem and the Miller Fisher syndrome (MFS) are still subject to debate. It is suggested that they may both belong to different extremes of the same nosological spectrum known as the ophthalmoplegia-ataxia-areflexia syndrome. CLINICAL CASE: We report the case of an 11 year old boy with encephalitis of the brain stem who …

Infectious lesions of the brain stem.

Infectious lesions of the brain stem are rare and include primarily abscess and encephalitis. The most common etiologic agents for abscess formation are Streptococcus sp., Staphylococcus sp., and M. tuberculosis. Encephalitis is associated most often with L. monocytogenes and herpes simplex virus infection. Classical brain stem syndromes are uncommon with brain stem infections, and CSF obstruction can be seen with neurocysticercosis. The diagnosis of these lesions has been greatly aided by CT and MR imaging. Microsurgery and stereotaxis are both appropriate techniques for the treatment of brai…

Cavernous malformations of the brain stem.

Once they become symptomatic, cavernous malformations of the brain stem appear to cause progressive morbidity from repetitive hemorrhage, and can even be fatal. Twenty-four patients with long-tract and/or cranial nerve findings from their cavernous malformations of the brain stem were seen for initial evaluation or surgical consultation and thereafter received either surgical or continued conservative treatment. The decision to operate was based on the proximity of the cavernous malformation to the pial surface of the brain stem, the patient's neurological status, and the number of symptomatic…
