In its function of separating pulmonary and systemic arterial blood flow, the ductus arteriosus, which connects both circuits, either closes permanently at a certain stage in development or attains a capacity to close and reopen depending on the physiological needs in certain species. In air-breathing vertebrates varying from lungfish to mammals, the ductus arteriosus derives from the sixth pharyngeal arch artery, and in preparation for its specific task, undergoes its own unique differentiation programme, starting early in development. To date, the mechanisms involved in defining this unique

Difficult laryngoscopy in syndromic children is usually attributed to mandibular or cervical abnormalities; however, supraglottic malformations may also impair laryngeal exposure. We describe three syndromic pediatric cases in which difficult laryngoscopy was associated with abnormal epiglottic morphology. In the index neonate with suspected left-sided branchial arch hypoplasia, bronchoscopy demonstrated ipsilateral hypoplasia of the epiglottis and arytenoid region, with the epiglottis arising unusually deep from the tongue base. These findings suggest that supraglottic malformation should be

Branchial sinuses are one of the most common congenital anomalies present. They are usually unilateral; bilateral cases are present but are rare. The presentation of bilateral branchial sinus anomalies along with bilateral first arch anomalies is very rare. Here, we present a case of bilateral first arch anomalies co-existing with bilateral second arch anomalies in a patient with no related family history and no associated syndrome.

Our aim in this review is to discuss currently known mechanisms associated with three important syndromes of the first and second pharyngeal arches: Treacher Collins syndrome (TCS), Oculo-auriculo-vertebral syndrome (AOVS) and Auriculo-Condylar syndrome (ACS) or question mark ear syndrome. TCS and ACS are autosomal dominant diseases, with nearly complete penetrance and wide spectrum of clinical variability. The phenotype of the latter has several overlapping features with OAVS, but OAVS may exist in both sporadic and autosomal dominant forms. Mutations in the TCOF1 gene are predicted to cause

It is difficult to teach students about the embryological transformation of the branchial arteries. In mammals, six pairs of branchial arteries develop, but all are not present at the same embryological stage. The first, second, fifth and a part of right sixth branchial artery disappear early in embryological development. The remaining third, fourth and sixth branchial arteries mainly constitute the arterial system of the breast. The aorta and pulmonary trunk are derived from the truncus arteriosus. Because of the complexity of this developmental process, we have devised a user-friendly model

A Type II first cleft anomaly may lie superficial to, deep to, or between the branches of the facial nerve. The embryologic basis of this finding depends on the variable relationship between the migrating facial musculature, as it passes forward from the second branchial arch to the face, and the epithelial elements which persist in the region of fusion between the inferior portions of the mandibular and hyoid arches.

PURPOSE: Branchial cleft anomalies (BCAs) are developmental malformations of the head and neck region. Their histogenesis has been the subject of controversy and is not fully understood. This study aimed to test all present developmental theories ("branchial apparatus," "precervical sinus," "thymopharyngeal," and "inclusion" theories) on a sample of 48 BCAs from a single institution.
METHODS: We performed a retrospective analysis of clinical-epidemiological and anatomical-pathological characteristics of BCAs treated over a 12-year period in our hospital.
RESULTS: Overall, 46 patients (24 [52

First branchial cleft anomaly is a rare disease of the head and neck. It accounts for less than 8% of all branchial abnormalities. It is classified into type I, which is thought to arise from the duplication of the membranous external ear canal and are composed of ectoderm only, and type II that have ectoderm and mesoderm. Because of its rarity, first branchial cleft anomaly is often misdiagnosed and results in inappropriate management. A 9-year-old girl presented to us with fistula in the submandibular region and discharge in the external ear. Under general anesthesia, complete surgical excis

Cystic diseases of the salivary gland include mucous cysts and plunging ranula; cysts in the parotid region are rare. In this report, we describe a case of a first branchial cyst in the parotid region. The cyst extended into the parapharyngeal space and was repeatedly infected. The patient was a 35-year-old woman who presented to our hospital with a mass on the left lower ear. Imaging findings revealed a cystic lesion in the parotid region that extended to the parapharyngeal space. A yellowish slurry was aspirated on a percutaneous fine-needle aspiration biopsy. Cytology revealed a class II tu

The branchial arches are meristic vertebrate structures, being metameric both between each other within the rostrocaudal series along the ventrocephalic surface of the embryonic head and within each individual arch: thus, just as each branchial arch must acquire a unique identity along the rostrocaudal axis, each structure within the proximodistal axis of an arch must also acquire a unique identity. It is believed that regional specification of metameric structures is controlled by the nested expression of related genes resulting in a regional code, a principal that is though to be demonstrate

Masses of the parotid gland region can be caused by a variety of different conditions, most commonly neoplasms of a benign or malignant nature. A rare cause for parotid tumors is the embryonologic remnant of first branchial cleft cysts. We reviewed branchial cysts for a 10-year period at the three teaching hospitals in Akron, OH. First branchial cleft cysts, or parotid lympho-epithelial cysts, were found to be a very rare condition presenting as parotid tumors. The surgeon should always be aware of these cysts as a possibility in the differential diagnosis of parotid masses in the infant and a

Branchial cleft cyst was diagnosed in a 6-month-old Angus bull with a large swelling in the distal ventral neck region. A definitive diagnosis could not be attained from results of the clinical examination, radiography, and ultrasonography. Diagnosis was made from histologic examination of the surgically removed mass. Branchial cleft cysts are remnants of the branchial apparatus and are considered rare in domestic animals. The differential diagnosis should include the thymic form of bovine viral leukosis, thymoma, abscess, goiter, and thyroid gland tumors as well as other rare cysts that can d

Branchial cysts are relatively rare lesions with lymphoid tissue in the underlying epithelium of the cyst wall. The present study describes the case of a branchial cyst with keratinization and calcification that occurred in the right submandibular region, along with a review of the literature. A 49-year-old female patient presented with a complaint of swelling in the right submandibular region. Computed tomography revealed a well-defined, cystic lesion located anterior to the sternocleidomastoid muscle, outside the hyoid bone, and in front of the submandibular gland. The cystic cavity presente
