Disorders of calcium and phosphate metabolism in horses.

Calcium and phosphate have structural and nonstructural functions, and their concentrations in the extracellular compartment are affected by the physiologic status of the animal as well as diseases. Important progress in understanding calcium and phosphorus metabolism in healthy and diseased horses and foals has been made in recent years. For example, several studies have confirmed that hypocalcemia is frequent in horses with gastrointestinal disease and that calcium endocrine dysregulation is associated with survival in foals. One critical point in the homeostasis of these minerals is their i

Applications of Metabolomics in Calcium Metabolism Disorders in Humans.

The pathogenesis of the disorders of calcium metabolism is not fully understood. This review discusses the studies in which metabolomics was applied in this area. Indeed, metabolomics could play an essential role in discovering biomarkers and elucidating pathological mechanisms. Despite the limited bibliography, the present review highlights the potential of metabolomics in identifying the biomarkers of some of the most common endocrine disorders, such as primary hyperparathyroidism (PHPT), secondary hyperparathyroidism (SHPT), calcium deficiency, osteoporosis and vitamin D supplementation. Calcium and bone metabolism disorders during pregnancy and lactation.

Pregnancy and lactation cause a substantial increase in demand for calcium that is met by different maternal adaptations within each period. Intestinal calcium absorption more than doubles during pregnancy, whereas the maternal skeleton resorbs to provide most of the calcium content of breast milk during lactation. These maternal adaptations also affect the presentation, diagnosis, and management of disorders of calcium and bone metabolism. Although some women may experience fragility fractures as a consequence of pregnancy or lactation, for most women, parity and lactation do not affect the l

[Disorders of calcium metabolism during pregnancy and breast feeding].

Hypoparathyroidism following thyroidectomy or due to autoimmune process is the most common cause of hypocalcaemia. If untreated, maternal hypocalcaemia may stimulate fetal parathyroids which results in bone demineralization. Calcium supplementation may also reduce the risk of hypertension. Moreover, hypocalcaemia may inhibit the synthesis of calcium gene-related peptide (CGRP), which, in turn, decreases blood pressure and uterus contractions. In women of reproductive age hypercalcaemia is mostly due to hyperparathyroidism. The complications include toxemia, vomiting, and hypercalcaemic crisis.

[Disorders of calcium and bone metabolism in glucocorticoid treatment].

The effects of glucocorticoids on calcium and bone metabolism were investigated in 11 children (aged 6 months to 13 years) who were treated with dexamethasone, prednisolone and depot-ACTH because of different disorders. Alkaline phosphatase activity and osteocalcin in serum, representing indices of osteoblastic bone synthesis, and urinary hydroxyproline in relation to creatinine in morning fasting urine specimens, an index of osteoclastic bone degradation, decreased by 53-61% from baseline (P less than 0.01), with a highly significant relationship of all 3 indices to each other. Factors affecting calcium metabolism in disorders of the kidney.

The ionized calcium concentration in blood is maintained within narrow limits by a complex hormonal system that includes parathyroid hormone (PTH) and vitamin D. The kidney plays a pivotal role in the physiologic action of PTH, as this peptide hormone increases tubular calcium reabsorption, decreases tubular phosphate reabsorption, and stimulates the renal 25-hydroxy-1-hydroxylase to convert 25-hydroxyvitamin D to 1,25-dihydroxyvitamin D, the hormonal form of the vitamin that stimulates intestinal calcium absorption. Inherited and acquired disorders of tubular function and acute and chronic re

Calcium-sensing receptor (CaSR) mutations and disorders of calcium, electrolyte and water metabolism.

The extracellular calcium-sensing receptor (CaSR) is a family C G-protein-coupled receptor (GPCR) that is expressed at multiple sites, including the parathyroids and kidneys. The human CASR gene, located on chromosome 3q21.1, encodes a 1078 amino acid protein. More than 230 different disease-causing mutations of the CaSR have been reported. Loss-of-function mutations lead to three hypercalcemic disorders, which are familial hypocalciuric hypercalcemia (FHH), neonatal severe hyperparathyroidism and primary hyperparathyroidism. Gain-of-function mutations, on the other hand, result in the hypocal

Genetics of monogenic disorders of calcium and bone metabolism.

Disorders of calcium homeostasis are the most frequent metabolic bone and mineral disease encountered by endocrinologists. These disorders usually manifest as primary hyperparathyroidism (PHPT) or hypoparathyroidism (HP), which have a monogenic aetiology in 5%-10% of cases, and may occur as an isolated endocrinopathy, or as part of a complex syndrome. The recognition and diagnosis of these disorders is important to facilitate the most appropriate management of the patient, with regard to both the calcium-related phenotype and any associated clinical features, and also to allow the identificati

[Disorders of calcium metabolism and ossification caused by anticonvulsants].

Disturbances of Calcium-metabolism and of ossification are well known and well documented side-effects of anticonvulsant drugs since the first description in 1967. Elevated alkaline phosphatase and hypocalcemia till generalised osteoporosis and vitamin-D-sensitive rickets can be seen. The primary cause of these dose-related disturbances is an accelerated elimination of vitamin D because of induction of microsomal enzymes in the liver by anticonvulsant drugs; a suboptimal vitamin-D-intake and reduced sunlight exposure in institutionalised patients are often combined causative factors. In all pa

[Electromyographic findings in disorders of calcium and potassium metabolism].

The electromyographic findings in disturbances of the calcium and potassium metabolism are discussed. Particularly in the tetanic syndrome the authors deal with the difficulty of the interpretation of the findings. Finally a casuistic report on relapsing paralyses of the muscles in hypopotassiaemia is given.

[Disordered calcium metabolism in Reiter's disease].

Cellular (erythrocytic) and extracellular (blood serum) calcium levels have been measured in patients with Reiter's disease and with urogenital chlamydiosis over the course of treatment. Calcium metabolism disorders depending on the disease severity have been detected in chlamydiosis patients. Calcium homeostasis imbalance manifests as a significant rise of the red cell calcium level in the presence of normal Ca ion concentration in the blood serum, which fact may result from impaired Ca ion transport through the plasma membrane. Combined therapy, including erythromycin, resulted, besides Chla…

[Cognitive Function and Calcium. The link between dementia and bone and calcium metabolism disorders].

Bone and calcium metabolism disorders are closely linked with dementia. Screening for dementia is important since chronic hypercalcemia and hypocalcemia resulting from parathyroid function abnormalities can become a cause of dementia onset. In recent years, it has become clear that vitamin D deficiencies inducing cardiovascular disease and other factors are involved in the pathogenesis of various diseases that in turn become risk factors in dementia, especially Alzheimer's disease. Moreover, osteoporosis and dementia both commonly occur among the elderly. Treating dementia patients for osteopo…
