Ceramic on Ceramic or Ceramic-on-polyethylene for Total Hip Arthroplasty: A Systemic Review and Meta-analysis of Prospective Randomized Studies.

BACKGROUND: Wear debris of polyethylene has become a restraining factor of the durability for total hip arthroplasty (THA). Ceramic on ceramic (COC) has better wear resistance while the squeaking sound and prosthesis fracture are of concern. It is still a controversy that bearing couples are better for THA. METHODS: We performed a systematic review of all English articles identified from PubMed (1966-), Embase (1980-) and the Cochrane Library. Clinical outcomes, complications, revision rates, and radiographic outcomes of COC-THA and ceramic on polyethylene (COP)-THA were compared and evaluated

Primary ceramic-on-ceramic total hip arthroplasty using a 32-mm ceramic head with a titanium-alloy sleeve.

BACKGROUND: Modern ceramic-on-ceramic bearings have become attractive alternatives to conventional polyethylene in total hip arthroplasty (THA) as a result of their low wear and minimal particle production. However, 28-mm heads in ceramic-on-ceramic bearing couples have been associated with ceramic fracture. To address these issues, 32-mm and larger ceramic heads with a titanium-alloy sleeve have been introduced, although limited data are available on their durability and clinical outcomes. QUESTIONS/PURPOSES: We determined (1) the survivorship of the primary ceramic-on-ceramic THA using a 32

Comparison of surgical outcomes and implant wear between ceramic-ceramic and ceramic-polyethylene articulations in total hip arthroplasty.

The results of a prospective multicenter trial comparing 357 hips randomized to total hip arthroplasty with either ceramic-ceramic or ceramic-polyethylene couplings are presented. No statistically significant difference in clinical outcomes scores between the ceramic-ceramic and ceramic-polyethylene groups was observed at any time interval. The mean linear rate was statistically lower (P < .001) in the ceramic-ceramic group (30.5 μm/year) when compared with the ceramic-polyethylene group (218.2 μm/year). The rates of ceramic implant fracture (2.6%) and audible component-related noise (3.1%) we

Prospective randomized trial comparing alumina ceramic-on-ceramic with ceramic-on-conventional polyethylene bearings in total hip arthroplasty.

This prospective randomized study aims to compare the outcome between an alumina ceramic-on-ceramic (CC) articulation with a ceramic on ultra-high-molecular-weight polyethylene articulation (CP). Fifty-six hips in 55 patients with mean age 42.2 (range, 19-56) each received uncemented components, a 28-mm alumina head with randomization of acetabular liner. Mean St Michael's outcome score for each group with up to 10 years follow-up (median, 8 years; range, 1-10) was 22.8 and 22.9, respectively (P = .819). Wear was identified in all but 1 CP replacement, but only 12 of the 23 CC. Mean wear in th

Overcoming the performance ceiling of textured piezoelectric ceramics.

Textured piezoelectric ceramics could deliver the ultrahigh piezoelectricity of single crystals while retaining the mechanical robustness and cost-effectiveness of conventional ceramics, but after decades of effort, their piezoelectricity has not approached that of single crystals. We exploited compositional flexibility of textured ceramics, a critical advantage over single-crystal counterparts, using a machine-learning approach. Our samarium (Sm)-doped Pb(In1/2Nb1/2)O3-Pb(Sc1/2Nb1/2)O3-PbTiO3 textured ceramics exhibited an ultrahigh piezoelectric coefficient (d33) of 1720 picocoulombs per new

Meta-analysis of bonding effectiveness to zirconia ceramics.

Dental zirconia can no longer be considered un-bondable to tooth tissue. In literature, an increasing number of papers indeed report on the bonding effectiveness of different luting techniques to zirconia. We aimed to disclose general trends in bonding to zirconia by systematically collecting zirconia bond-strength data. A search in PubMed and EMBASE revealed 1,371 bond-strength tests reported on in 144 papers. A macro-shear bond-strength protocol was most frequently used; it revealed significantly lower bond strengths and was less discriminative than the other test methods. Regarding luting t

BiFeO3 Ceramics: Structure-Property Relationships and Doping Strategies.

Bismuth ferrite (BiFeO3, BFO) is a lead-free perovskite that combines robust ferroelectricity and antiferromagnetism at room temperature, making it one of the most extensively studied room-temperature multiferroics for spintronics, sensors, and multifunctional devices. In bulk ceramics, however, Bi volatility, oxygen-vacancy-driven leakage, and parasitic phases complicate synthesis and obscure intrinsic ferroic response. This review critically revisits the fundamental physical properties of bulk BiFeO3 ceramics by elucidating the relationships between crystal structure, defect chemistry, proce

Interconnected porous hydroxyapatite ceramics for bone tissue engineering.

Several porous calcium hydroxyapatite (HA) ceramics have been used clinically as bone substitutes, but most of them possessed few interpore connections, resulting in pathological fracture probably due to poor bone formation within the substitute. We recently developed a fully interconnected porous HA ceramic (IP-CHA) by adopting the 'foam-gel' technique. The IP-CHA had a three-dimensional structure with spherical pores of uniform size (average 150 microm, porosity 75%), which were interconnected by window-like holes (average diameter 40 microm), and also demonstrated adequate compression stren

[Interaction between orthodontic tooth movement and hydroxyapatite ceramics].

A pilot study examined the clinical and histological effects of orthodontic tooth movement in hydroxyapatite-augmented ridges in animal models. Responses observed at the interface between the clinical root and the ceramo-osseous regeneration were negative: impairment of periodontal function had led to immobility and root resorption. On the basis of this experience one must advice against using this approach in humans at the present time. Further studies will be conducted to elucidate the relevant clinical questions.

Ceramic-on-ceramic versus ceramic-on-polyethylene in total hip arthroplasty: a comparative study at a minimum of 13 years follow-up.

BACKGROUND: Nowadays hip replacement is one of the most successful surgery in terms of clinical outcome and patient's satisfaction. Therefore, the choice of biomaterials in hip replacement is increasingly important with the aim of obtaining a long-term satisfaction of patient and a greater survivorship of the implants. Ceramic-on-polyethylene (COP) and ceramic-on-ceramic (CoC) bearings are two common coupling used in total hip arthroplasty. The aim of this retrospective study was to compare clinical and radiological outcomes between patients treated using CoC and CoP THA at a mean follow-up of

Study of New Glass-Ceramic and Dense Ceramic Containing Biogenic Hydroxyapatite.

A novel bioactive glass-ceramic was developed using biogenic hydroxyapatite (BHA) synthesized from Rapana venosa (Black Sea) shells and monocalcium phosphate monohydrate [Ca(H2PO4)2·H2O] via solid-state synthesis. The prepared batches were obtained by combining BHA with SiO2, B2O3, and Na2O, melted at 1200 °C and melt-quenched in water to form glass-ceramic materials. Dense biogenic hydroxyapatite-based ceramics were successfully sintered at 1200 °C (2 h hold) using a 25 mass % sintering additive composed of 35 mass % B2O3, 45 mass % SiO2, 10 mass % Al2O3, and 10 mass % Na2O. Midterm Results of Fourth-Generation Ceramic-On-Ceramic Total Hip Arthroplasty.

PURPOSE: There are limited reports for the results of the fourth-generation ceramic-on-ceramic (CoC) articulation total hip arthroplasty (THA). And, throughout the surgical experience, we encountered some cases of liner pulling-out phenomenon after liner fixation and femoral preparation. The objective of this study was to evaluate the incidence, risk factors of delta ceramic liner or head fractures, and also the clinical and radiological results of using the fourth-generation CoC articulation in THA. PATIENTS AND METHODS: We retrospectively reviewed 242 patients (263 hips) who underwent prima

Ceramic-on-ceramic THA Implants in Patients Younger Than 20 Years.

BACKGROUND: Poor survival of THA implants in very young patients has been attributed to use of cemented implants, wear of conventional polyethylene, and the presence of morphologic deformities in the proximal femur or in the acetabulum. Few studies have reported the long-term results of ceramic-on-ceramic implants in THAs in patients younger than 20 years. QUESTION/PURPOSES: We determined: (1) the proportion of patients who experienced complications related to the ceramic bearing (squeaking, fracture); (2) the survivorship free from loosening and free from revision for any reason; (3) whether

Ceramic-on-ceramic bearing decreases the cumulative long-term risk of dislocation.

BACKGROUND: It is unclear whether late THA dislocations are related to mechanical impingement or to a biological mechanism that decreases the stability provided by the capsule (eg, inflammation secondary to osteolysis). It is also unknown if alumina-on-alumina bearing couples decrease the risk of late dislocation as a result of the absence of wear and osteolysis. QUESTION/PURPOSES: We asked (1) whether the cumulative number of dislocations differed with alumina-on-alumina (AL/AL) or alumina-on-polyethylene bearings (AL/PE); (2) whether patient factors (age, sex, and diseases) affect risk of l

Alumina ceramic-on-ceramic total hip replacement with a layered acetabular component.

A modular layered acetabular component (metal-polyethylene-ceramic) was developed in Japan for use in alumina ceramic-on-ceramic total hip replacement. Between May 1999 and July 2000, we performed 35 alumina ceramic-on-ceramic total hip replacements in 30 consecutive patients, using this layered component and evaluated the clinical and radiological results over a mean follow-up of 5.8 years (5 to 6.5). A total of six hips underwent revision, one for infection, two for dislocation with loosening of the acetabular component, two for alumina liner fractures and one for component dissociation with

Two- to 9-year clinical results of alumina ceramic-on-ceramic THA.

From June 1997 to June 2003 we performed 194 total hip arthroplasties on 173 patients using alumina ceramic-on-ceramic bearings as part of a prospective United States Food and Drug Administration/Investigational Device Exemption study. The average patient age at surgery was 49.9 years. Minimum followup time was 2 years (mean 4.3 years, range 2-9 years). We evaluated survival rate, implant- and nonimplant-related complications. Clinical outcomes included the Merle d'Aubigné score. We assessed radiographs for signs of osteolysis, component loosening, and implant wear. No patients had osteolysis

Pseudotumor After Ceramic-on-Ceramic Total Hip Arthroplasty: A Case Report.

Pseudotumors, typically associated with metal-on-metal (MoM) bearings in total hip arthroplasty (THA), have rarely been reported in ceramic-on-ceramic (CoC) THA. This case details the development of a pseudotumor in a 70-year-old female six years after an uneventful CoC THA for osteoarthritis. The patient presented with groin pain and swelling, and imaging revealed a fluid-filled mass at the hip. Aspiration provided temporary relief, but the cyst recurred. Surgical intervention showed no macroscopic wear on the bearing surfaces, but 40cc of fluid was evacuated from the acetabulum after cup rem
