Motor evoked potentials following cervical electrical stimulation in brachial plexus lesions.

The diagnostic relevance of recording motor evoked potentials (MEPs) after electrical stimulation of the cervical region, as compared with conventional needle electromyography (EMG), was evaluated in 26 patients with brachial plexus (BP) damage of different aetiology, severity and topography. MEP abnormalities (absence or latency increase) were observed in at least one muscle of all the patients, with a global incidence of 61.5% of the muscles examined. Neurogenic EMG signs were present in all but one patient with an incidence of 62.2% of the muscles examined. Combining the two methods, the gl

Bilateral vocal cord palsy during carotid endarterectomy under cervical plexus block.

We describe a case of vocal cord palsy leading to respiratory obstruction during carotid endarterectomy under cervical plexus block in a patient who had preexisting contralateral vocal cord paralysis subsequent to a previous thyroidectomy. The patient required immediate tracheal intubation and subsequent tracheostomy to maintain the airway postoperatively. Care must be given to avoid contralateral vocal cord paralysis in the presence of a preexisting vocal cord palsy.

Functional hemitongue reconstruction using innervated supraclavicular fasciocutaneous island flaps with the cervical plexus and reinnervated supraclavicular fasciocutaneous island flaps with neurorrhaphy of the cervical plexus and lingual nerve.

BACKGROUND: The purpose of this study was to evaluate tongue function in patients with tongue cancer after reconstruction with innervated supraclavicular fasciocutaneous island flaps (iSFIFs) with the cervical plexus (iSFIFs-SPN) or reinnervated SFIFs with neurorrhaphy of the cervical plexus and lingual nerve (rSFIFs-SPN-LN). METHODS: Forty-two tongue defects were reconstructed using iSFIFs-SPN and rSFIFs-SPN-LN. Two-point discrimination tests revealed a significant difference on the dorsal and ventral hemitongue reconstructed using rSFIFs-SPN-LN from control values. RESULTS: No statistical …

Superior sulcus tumor disguised as cervical radiculopathy with metastasis to brachial plexus.

A 74-year-old man presented to an orthopedic spine surgeon with signs and symptoms consistent with cervical radiculopathy. Investigation revealed metastasis of a Pancoast tumor to the patient's brachial plexus. A year after initial diagnosis, the patient achieved full neurological function of his left arm. Pancoast tumors are rare in the orthopedic population. Thorough physical examination, imaging, and nerve conduction studies ultimately led to the patient's diagnosis.

Three-tesla magnetic resonance neurography of the brachial plexus in cervical radiculopathy.

INTRODUCTION: There have been no reports of the use of 3-Tesla magnetic resonance neurography (3T MRN) to characterize cervical radiculopathy. In particular, there are no reports of MRN of brachial plexus involvement in patients with cervical radiculopathy. METHODS: We reviewed retrospectively 12 consecutive patients with cervical radiculopathy who underwent 3T MRN. RESULTS: The median age was 54.5 years. Eleven of 12 patients were men. The distribution of nerve-root signal abnormality was correlated with intervertebral foraminal stenosis and the presence of muscles that exhibited weakness a

[Evaluation of safety and anesthetic effect for ultrasound-guided cervical plexus block].

OBJECTIVE: To observe the influences upon the degree of diaphragmatic excursion during deep cervical plexus block at the third cervical vertebra (C3) and compare the safety and anesthetic effect of modified cervical plexus block by ultrasonic guidance and blocking of cervical plexus at one point. METHODS: Part I: 30 patients of ASA (American society of anesthesiologists) I-II scheduled for thyroid surgery were selected for bilateral cervical plexus block at C3 and bilateral skin nerve branches via ultrasonic guidance. Diaphragmatic excursion was recorded. Part II: 80 patients of ASAI-II sched…

Congenital dilation of the cervical epidural venous plexus: neuroradiology and endovenous management.

We report a case of a 15-year-old girl suffering from cervicobrachialgia who was admitted to our service due to an enlarged neural foramen suspicious for a neurinoma. The cervical phlebography, however, revealed a space-occupying dilated epidural vein with increased blood supply from the suboccipital venous plexus. Lesions like this are absolutely rare, presumably of congenital origin and have not been described before. The lesion was treated by feeder occlusion applying platinum coils and enbucrilate via the internal jugular vein.

Digital nerve sensory potentials in lesions of cervical roots and brachial plexus.

Normal values for the amplitudes of digital sensory nerve action potentials were obtained from 190 control subjects, using an orthodromic surface-recording technique. Lower limits were calculated, taking into account laterality, age and sex; the ratios of amplitudes between different digits were also estimated. The results in control subjects were compared with those of 20 cases of trauma involving root avulsion or extraspinal injury, 38 patients with intraspinal disease, 6 patients with extraspinal disorder and 9 cases of neuralgic amyotrophy. The variability of the dermatomes is discussed an

An Uncommon, Extensive Involvement of Hydatid Cyst-Cervical Spine to Brachial Plexus.

INTRODUCTION: Hydatid cyst usually involves liver and lung. But it has been reported in uncommon locations like the spine and brain. There are few reports of cervical spine to brachial plexus. This report is about such extensively involved hydatid cyst management. CASE REPORT: A 32-year-old male had complaints of neck pain, left-hand weakness, and atrophy. MRI of cervical spine and thorax revealed a multicystic lesion involving the cervical spine, vertebrae, brachial plexus, and lung apex. The lesion was removed in two stages. DISCUSSION: Due to scarcity of literature, dense adhesions with d

Surgical Approach to Injuries of the Cervical Plexus and Its Peripheral Nerve Branches.

BACKGROUND: Located in the neck beneath the sternocleidomastoid muscle, the cervical plexus comprises a coalition of nerves originating from C1 through C4, which provide input to four cutaneous, seven motor, and three cranial nerves and the sympathetic trunk. Sporadic instances of injury to these superficial nerves have been reported. Nevertheless, this specific anatomical cause of neurogenic pain remains incompletely described and underrecognized. METHODS: Twelve patients presented with pain and were diagnosed with various combinations of injury to the lesser occipital, great auricular, tran…

Cervical laminectomy and micro resection of the posterior venous plexus in Hirayama disease.

INTRODUCTION: Hirayama disease is a rare cervical myelopathy predominantly affecting young adults and mainly found in Asia. It results in a pure motor distal lesion of the upper limbs with slow progression. Dynamic magnetic resonance imaging (MRI), which allows the diagnosis to be made, shows a typical appearance of anterior compression of the cervical spinal cord associated with enlargement of the posterior epidural spaces due to a dilated venous plexus. Surgery is considered when conservative treatment has failed. However, the type of surgery is not well standardized in this compressive myel

Demonstration of the spread of injectate with deep cervical plexus block: a case series.

BACKGROUND AND OBJECTIVES: The authors conducted a scanographic study in order to characterize the local anesthetic spread of injectate resulting from a single-injection technique of deep cervical plexus block. METHODS: Six consecutive American Society of Anesthesiologists II and III patients scheduled for elective carotid endarterectomy under regional anesthesia were enrolled. Deep cervical plexus block was placed via an anterolateral approach using a nerve stimulator to guide the injection on contact with the levator scapulae nerve. With specific contractions evoked at a stimulating current…

[Origin of shoulder pain after "neck dissection". Importance of the cervical plexus].

BACKGROUND: The role of sensitive parts of the cervical plexus for the development of neuropathic pain is not yet clear. Our study investigated the correlation between shoulder pain and cervical plexus damage after different types of neck dissection (ND). MATERIAL AND METHODS: The sensitivity for warm/cold and sharp/blunt was tested in the dermatomes of C2, C3, C4, and the minor occipital nerve. Shoulder pain was measured semiquantitatively by a rating scale. RESULTS: Motion-dependent shoulder pain was observed 6 months postoperatively in 50% after resection and in 29.2% after preservation o…

[Block of the cervical plexus for the unipolar exclusion of a gastroplasty after bronchial fistula].

A 46 year old man was operated on for an oesophageal carcinoma. Total oesophagectomy and gastroplasty were carried out. Postoperatively, a fistula developed between the gastroplasty and the right main bronchus. This led to respiratory impairment, with pulmonary infection, pleural effusion and hypoxia; the patient could not remain supine. A cervical oesophagostomy was decided on; to carry it out, a cervical plexus block was preferred to a cervical epidural anaesthesia because of the lesser respiratory and haemodynamic effects of the former technique. The procedure was well supported by the pati…
