Management of gallbladder perforation by percutaneous cholecystostomy.

Percutaneous cholecystostomy was employed successfully as an alternative to cholecystectomy for the treatment of acute cholecystitis with perforation in an elderly woman. The procedure, performed portably in the cardiac care unit, was selected because progressive cardiac and renal decompensation made the patient an extremely poor surgical risk.

Cholecystostomy: a place in modern biliary surgery?

Cholecystostomy retains a place in the general surgical armamentarium. In this series of 115 patients undergoing cholecystostomy between 1967 and 1977, 68 percent had acute cholecystitis, whereas in the remaining patients biliary drainage was undertaken as part of another procedure. The in-hospital mortality rate was 6 percent for the group with acute cholecystitis and 37 percent for the other patients. Forty-five patients subsequently had elective cholecystectomy, 29 of these for radiographically documented retained calculi. Thirty-four patients without retained calculi remained asymptomatic …

Cholecystostomy--an old dog with new tricks.

A large minority of patients having cholecystostomy show cystic duct or common duct stones on postoperative tube cholangiograms. The use of mechanical (Dormia stone basket) and chemical (heparin and sodium cholate) means to remove gallstones via the cholecystostomy fistula reduces the need for additional surgery (ie, cholecystectomy), which is particularly important in poor-risk elderly patients.

[Percutaneous cholecystostomy in non-surgical patients].

PURPOSE: To assess the efficacy and complications of percutaneous cholecystostomy (PC) in the treatment of acute cholecystitis in non-surgical patients. MATERIALS AND METHODS: Retrospective study of 25 cases (16 males and 9 females) of PC. The average age was 82 years (range: 59-95). Eight had acute acalculous cholecystitis (AAC) and 17 had acute calculous cholecystitis (ACC). US-guided percutaneous cholecystostomy was performed in most cases; CT-guidance was required in 5 cases. RESULTS: One technical failure and one complication (abdominal wall hematoma) occurred. PC was successful for sep…

Percutaneous cholecystostomy in critically ill patients.

Sixteen critically ill patients underwent percutaneous cholecystostomy because of suspected acute cholecystitis. The procedure was technically successful, although 11 of 16 patients died subsequently because of various complications of their underlying primary disorders. We reviewed this series to reassess the value of percutaneous cholecystostomy. Four of 11 patients with definite acute cholecystitis (group 1) were cured by this technique, but three required surgery because of gallbladder wall necrosis. Two of these were among four cases which had demonstrated pericholecystic fluid collection…

Image-guided percutaneous cholecystostomy: challenging the norms.

Image-guided percutaneous cholecystostomy (IGPC) is a widely recognized and regularly employed procedure in numerous institutions, serving as an indispensable cornerstone in the management of patients with acute cholecystitis. The most up-to-date literature has found that the transperitoneal route is at least as safe as the transhepatic route and that both the trocar and Seldinger techniques are equally safe and effective. The above novel insights may offer reassurance and alleviate concerns among operators performing IGPC by dispelling the fixation on previously established beliefs and thus p…

Patterns of care after cholecystostomy tube placement.

BACKGROUND: The use of cholecystostomy (c-tube) in acute cholecystitis (AC) has increased yet there is limited evidence to guide surgical decision-making after placement. As a result, there is variability in the use and timing of cholecystectomy after c-tube. We aimed to describe patient characteristics, outcomes, and biliary-related utilization in those who did and did not have cholecystectomy after c-tube. METHODS: This is a retrospective cohort study (2007-2017) using the MarketScan® claims database of patients (18-63 years) with at least 3 months of follow-up (or death). ICD-9/10 and CPT …

Percutaneous Cholecystostomy: Evidence-Based Current Clinical Practice.

The role of percutaneous cholecystostomy (PC) in the management of acute cholecystitis and cholangitis is outlined in the revised 2013 Tokyo Guidelines. These two emergencies constitute the vast majority of PC performed today for therapeutic purposes, and research has repeatedly shown the utility of PC in these conditions. PC is typically employed in the management of critically ill patients who are not surgical candidates. Indications and contraindications to PC are reviewed. Additional innovative applications of PC have been developed since it was first described in 1980. These include bilia…

Irrigation removal of retained gallstone following cholecystostomy.

A case is presented in which a patient, treated for acute cholecystitis by cholesystostomy, has a retained gallbladder stone removed by saline irrigation. A brief discussion is given on the indication for cholecystostomy and its use as definitive versus temporizing therapy. In performing cholecystostomy, a catheter of diameter equal to or greater than the diameter of any stone which may be left is recommended.

Outcomes of Percutaneous Cholecystostomy for Acute Cholecystitis.

BACKGROUND: Acute cholecystitis (AC) is an important cause of emergency admissions among the elderly. The use of percutaneous cholecystostomy (PC) as bridging therapy among high-risk patients is widely accepted. However, the use of PC as definitive treatment is controversial. AIM: To determine the characteristics, clinical outcomes and predictors of recurrence of AC among patients who underwent PC. METHODS: A retrospective case series of 71 consecutive patients [73 (38-96) years, 43/71 males] treated with PC for AC at a tertiary hospital from 2007 to 2013, with data collected from case recor…

Outcomes of Percutaneous Cholecystostomy in the Presence of Ascites.

PURPOSE: To evaluate whether the presence of ascites increases complications following placement of percutaneous cholecystostomy tubes (PCTs). MATERIALS AND METHODS: Retrospective review of all transhepatic PCTs placed between January 2005 and June 2014 was performed: 255 patients were included (median age of 65 y; range, 20-95 y). Of these patients, 97 had ascites and 158 had no ascites or only pelvic fluid. In all, 115 patients had calculous cholecystitis (45%), 127 had acalculous cholecystitis (50%), and 13 had common bile duct obstruction (5%). The primary outcome of interest was all comp…

Percutaneous cholecystostomy in the management of acute cholecystitis.

BACKGROUND: Percutaneous cholecystostomy (PC) has been used in managing acute cholecystitis in the setting of a patient with severe comorbidities where emergency cholecystectomy would carry significant mortality. The present study aims to assess the role, efficacy and complications of PC in acute cholecystitis. METHODS: Retrospective review of case notes of patients who underwent PC at Box Hill Hospital, Melbourne, Australia between July 1997 and December 2002. RESULTS: Sixteen patients (mean age 75 years; range 50-96) underwent PC. Indications for PC were significant comorbidities (n = 6), …

Percutaneous cholecystostomy in the management of acute cholecystitis.

BACKGROUND: The mainstay of therapy for acute cholecystitis is cholecystectomy, which has a mortality of 14-30% in high risk patients. An alternative approach in patients suffering from acute cholecystitis with contraindications to emergency surgery is percutaneous cholecystostomy. OBJECTIVE: To evaluate the efficacy and safety of percutaneous cholecystostomy as the initial treatment of acute cholecystitis in high risk patients. METHODS: Eighty consecutive patients (42 men, 38 women) underwent ultrasound-guided percutaneous cholecystostomy over a 5 year period. Sixty-five patients suffered f…

Percutaneous cholecystostomy: safety of the transperitoneal route.

PURPOSE: The authors analyzed the safety and efficacy of transperitoneal percutaneous cholecystostomy (PC) in patients with acute cholecystitis. PATIENTS AND METHODS: Thirty-four consecutive patients underwent transperitoneal PC for treatment of acute cholecystitis. A trocar technique was used for placement of the drainage catheter. RESULTS: PC was technically successful in all patients. In 27 (79%) of 34 patients, symptoms of acute cholecystitis resolved within 7 days. Minor complications occurred in two patients (5%). Both patients developed a hematoma adjacent to the gallbladder; these re…

[The treatment of acute cholecystitis by percutaneous cholecystostomy].

The authors report on their personal experience with percutaneous cholecystostomy (PC) in 36 cases of acute cholecystitis (AC). PC was successful in 34 cases, improving the clinical condition of the patients within the first hour following treatment and normalizing the laboratory data within 24-48 hours. Twenty-seven patients with calculous AC underwent cholecystectomy within 4-15 days. In 6 cases (3 calculous and 3 acalculous ACs) PC was the only treatment performed; in one case (calculous AC) PC was combined with lithotomy, the stone fragments being extracted via the cholecystostomy. Accordi…

Techniques and clinical utility for percutaneous cholecystostomy.

Therapeutic gallbladder procedures offer new options for treatment of gallbladder disease. These procedures may be life-saving, may be easier than other available techniques, or may merely provide an alternative form of therapy. Further use of these techniques will likely result in improvements in PC, as well as providing an ongoing definition of the role of these percutaneous procedures.
