Bronchogenic cyst presenting as dysphagia.

We report an adult patient who masqueraded as a case of gastroesophageal reflux disease for over 6 years. On close questioning he was found to have severe constricting pain in the retrosternal region and non progressive dysphagia for 6 years. Cineradiography of the oesophagus and CT scan chest revealed a large bronchogenic cyst compressing the oesophagus and the trachea. Excision of the cyst relieved the symptoms.

Kinematic evaluation of atlantoaxial joint instability: an in vivo cineradiographic investigation.

Although range of motion has been considered the best parameter to quantify atlantoaxial instability, no other kinematic parameters have been determined for dynamic quantification. The objectives of this study were to investigate the kinematics of the normal and pathologic atlantoaxial joints by cineradiography and to determine the in-vivo kinematic parameters, if any, for the quantification of atlantoaxial instability. Sagittal plane motion of the atlantoaxial joints was analyzed by cineradiography in 12 healthy volunteers and 15 patients with atlantoaxial subluxation. In both flexion and ext…

Nonsurgical correction for thrombosis of a St. Jude Medical tricuspid prosthesis: report of a case.

A 43-year-old woman with rheumatic heart disease underwent replacement of the aortic, mitral, and tricuspid valves using three SJM prostheses. Despite adequate warfarin therapy, routine cineradiography performed on the 40th postoperative day showed one of the leaflets of the tricuspid prosthesis to be "stuck" in the semiclosing position. A thrombosis of the tricuspid prosthesis was successfully treated with a urokinase infusion and mechanical thrombolysis using a pacemaker, following which normal valve function was restored.

Cricopharyngeal myotomy in pharyngeal paralysis. Cineradiographic and manometric indications.

A brief outline of the causes and symptoms of hypopharyngeal paresis is followed by a discussion of the diagnostic possibilities of manometry and cineradiography. It was found that the pharyngoesophageal sphincter mechanism remains intact in the case of complete paralysis of the hypopharynx. When there are severe symptoms of dysphagia, myotomy of the pharyngoesophageal sphincter is to be considered on the basis of these diagnostic findings. This procedure can lead to substantial improvement of the symptoms. Sphincterotomy yielded very favorable results in five out of nine patients, two of whom…

Leaflet movement of the ATS valve in the aortic position: unique behavior observed in 19-mm valves.

BACKGROUND: Leaflet opening angles of ATS valves in vivo tend to be less than those reported by the manufacturer, and there is a wide variation in opening angles even among valves of the same size. METHODS: Leaflet movement of aortic ATS valves was evaluated by cineradiography in 77 patients. The mean period from valve replacement to cineradiography was 40.2 months, and during that period, the ATS valve was replaced due to prosthetic valve obstruction in 1 of the 77 patients. RESULTS: In 76 patients with a normally functioning ATS valve, the mean opening angle of 19-mm valves (66.3 degrees +…

[Valve thrombosis of St. Jude Medical prosthesis; report of three cases].

Three cases with valve thrombosis of St. Jude Medical prosthesis (SJM valve) are reported. Incidence of valve thrombosis of SJM valve was 0.15%/patient-year. Cineradiography was useful to make the diagnosis. SJM valves were implanted with the hinges anatomically oriented in mitral position, and in both aortic and tricuspid positions with the leaflet opening parallel to the ventricular septum. Therefore, we recommend that SJM valve should be placed with the hinge antianatomically oriented in mitral position, and in both aortic and tricuspid positions with the leaflet opening to be perpendicular…

[Significance of roentgen cinematography in the diagnosis of functional disorders of the pharyngo-esophageal junction].

Dysfunction of the pharyngo-oesophageal sphincter may escape detection by clinical examination, endoscopy, and routine barium studies. Cineradiographic examination of 300 patients with unexplained dysphagia revealed 57 cases of pharyngo-oesophageal dysfunction. In 25 cases an underlying disorder could be found, and 32 cases were considered idiopathic. Radiologically 3 types of dysfunction may be distinguished, namely late opening, incomplete relaxation, and early contraction of the pharyngo-oesophageal sphincter. Cineradiography was found to be an easy and reliable method of detecting pharyngo…

ACTION trial: a prospective study on diagnostic Accuracy of 4D CT for diagnosing Instable ScaphOlunate DissociatioN.

BACKGROUND: Early detection of scapholunate ligament (SLL) tears is essential after minor and major trauma to the wrist. The differentiation … hand surgeons to radiological reports. A historical cohort of 80 patients which was diagnosed using cineradiography before implementation of 4D CT at the study site will serve as a comparative group. DISCUSSION: Static imaging lacks the ability to detect instable SLL tears after wrist trauma. Dynamic methods such as cineradiography and dynamic magnetic resonance …

The width of the pharyngo-esophageal junction area.

In order to elucidate the pharyngo-esophageal segment in normal and abnormal conditions the width of the pharyngo-esophageal junction area on three different levels: hypopharynx, infracricoid segment of the cervical esophagus (ICCO) and cervical esophagus was measured in 150 non-dysphagial volunteers and in 252 dysphagial patients examined with cineradiography during barium swallowing. The result indicates that a narrow ICCO in absolute figures reflects the size of the swallowed bolus and is not a predictor of pharyngeal abnormality, per se. However, when the width of the ICCO in a.p. projecti…

Treatment of scaphoid fractures with carpal instability.

Since 1981, I treated 81 scaphoid fractures: 14 wrists (group A) with acute unstable fracture, 37 (B) with ununited fracture accompanied by … cept for patients who did hard manual labor, in half of whom the score was poor when DISI remained. Cineradiography of ununited fractures was done before and after surgery. For three wrists in group B, plain X-ray films showed that DISI improved, but cineradiography showed that DISI was present; even without static DISI, there may be dynamic DISI. Treatment of ac …

[A new technique of gene transposition in the surgical treatment of sleep apnea syndrome].

Pharyngotomy appears to be insufficient for the treatment of severe sleep apnea. For patients who refuse or abandon mechanical ventilatory assistance, surgery can be used to widen the retrobasilingual space adapting it to the cephalometric profile defined by cineradiography and MRI in combination with velopharyngeal plasty. For patients with no maxillomandibular or lingual malformation, the pharyngotomy can be combined with an anterior transposition of the genial insertions of the tongue without rotation. A method different from that described by Riley and Powel is proposed. The details of the…

Acquired tracheobronchomalacia.

Acquired tracheobronchomalacia is seen in middle-aged and elderly people. Weakness of the tracheal and bronchial walls allows the posterior and anterior walls to draw nearer together during expiration and coughing, producing a varying obstruction. The main symptoms are dyspnoea, cough, phlegm and haemoptysis. Tracheobronchomalacia has often been diagnosed as chronic bronchitis, and the dyspnoea has also been treated as asthma, without success. Bronchoscopy, cineradiography, spirometry and intrabronchial pressure measurements are the diagnostic methods used. Tracheobronchomalacia is a progressi…

A cineradiographic investigation of velar movement variables in two normals.

The purpose of this investigation was to study certain aspects of velar dynamics among normals. Two adult subjects, one male and one female, were filmed using high-speed (100 fps) lateral-view cineradiography. Small radiopaque cups were attached to the surface of the soft palate and other articulators for tracking purposes. Trajectories of the velar fleshpoint in the midsagittal plane were demonstrated. Measures of velar velocity, velar displacement, and duration of transitional movements were studied in relation to the effects of phonetic context and speaking rate variation and were compared …

Acquired tracheomalacia: etiology and differential diagnosis.

The noninvasive diagnosis of impaired mechanical integrity or compliance of the trachea is most accurately made by fluoroscopic observation recorded on video tape or cineradiography, with or without benefit of artificial contrast media (contrast tracheography). In particular, localized buckling, collapse or dilatation indicative of focal tracheomalacia may thus be diagnosed in vivo and correlated with diseases of the central airways. Congenital tracheomalacia is a rare, bu, well described entity. Cases of acquired tracheomalacia occur with increasing frequency, but are often not clearly recogn…

Topographic representation of the pharyngoesophageal segment during swallowing.

Motility of the pharyngoesophageal segment (PES) was monitored by cineradiography (50 frames a second) during barium swallow and analyzed by a computerized topographic mapping of sagittal wall motion. Through measurement of wall displacement on 20 consecutive levels of the PES, distention, contraction, and peristalsis were studied. Topographic mapping of three patients with normal PES motility, delayed opening of the cricopharyngeus, and incomplete opening of the cricopharyngeus, was performed. These topographic mappings show that "narrowing" at the level of the cricopharyngeus reflects expans…

Computed axial tomography in C1-C2 trauma.

Twenty patients with C1-C2 vertebral trauma underwent computed tomographic (CT) scans to evaluate prospectively its efficiency for diagnosis and treatment. It is suggested that if standard roentgenograms are inconclusive, CT scan is recommended for further evaluation of C1 fractures and C1-C2 rotatory subluxation in preference to tomography and cineradiography. CT scans do not assess dynamic stability and may miss minimally displaced odontoid fractures. It was also concluded that they add little in the treatment of hangman's fractures, although scanning did show that the pathology was often mo…

Esophageal function in progressive systemic sclerosis.

A prospective study was performed in 13 consecutive patients with systemic progressive sclerosis (PSS). For the diagnosis of impaired esophageal peristalsis cineradiography and manometry are equally useful. Esophageal suction biopsy allows the diagnosis of esophagitis but not of scleroderma. Mild to severe esophageal involvement was observed in 12 patients. In only one patient the esophagus was virtually normal. Dysfunction of the esophageal body may occur early in the course of the disease while incompetence of the lower esophageal sphincter is observed on an average after 7 to 8 years. Both …

The snapping hip syndrome.

The snapping hip syndrome is a symptom complex characterized by hip pain and an audible snapping of the hip with exercise typically seen in young individuals. "External" and "internal" etiologies have been described, although the "internal" etiology is poorly understood. A clinical, radiographic, and anatomical study of eight patients with this disorder, secondary to an internal etiology, was undertaken to aid in the diagnosis and surgical treatment. Iliopsoas bursography with cineradiography revealed subluxation of the iliopsoas tendon to be an apparent cause of the snapping hip. The anatomy …
