A 56-year-old woman presented with changes in balance, handwriting, and thinking. Approximately 1 year before her first visit, the patient developed difficulty walking, which caused multiple falls without serious injury. She also developed bilateral upper-extremity tremors that worsened with movement. At the time of her visit, she could barely sign her name.

BACKGROUND: To investigate the predictive validity of the Medical College Admission Test (MCAT) for clinical reasoning skills upon completion of medical school.
METHOD: A total of 597 students (295 males, 49.4%; 302 females, 50.6%) participated from 1991 to 1999. Stepwise multiple regressions of the MCAT and premedical school GPA (independent variables) on the Part 1(declarative knowledge) and Part 2 (clinical reasoning) of the Medical Council of Canada Examinations (dependent variables) were employed.
RESULTS: For Part 1, the multiple regression revealed that three predictors (verbal reason…

A 74-year-old man presented with a 5-year history of progressive feet numbness followed by gait unsteadiness. Neurologic examination revealed absent tendon reflexes, markedly reduced vibration sense in the lower limbs with sensory ataxia, and preserved motor strength. The clinical picture was consistent with a chronic sensory-predominant neuropathy causing sensory ataxia. Nerve conduction studies demonstrated a sensory axonal polyneuropathy with absent sensory nerve action potentials and preserved compound motor action potentials. Extensive screening for potential acquired causes of polyneurop

We report a case of a 69-year-old woman who presented with acute-onset ophthalmoplegia. The patient continued to deteriorate over the next 10 days, developing upper and lower extremity weakness, dysphagia, and dyspnea, which eventually required intubation. This case report highlights the differential diagnosis associated with this constellation of symptoms and reviews current guidelines for treatment options related to the underlying diagnosis.

BACKGROUND: Difficulties experienced by novices in clinical reasoning have been well documented in many professions, especially medicine (Boshuizen and Schmidt 1992, 2000; Elstein, Shulman and Sprafka 1978; Patel and Groen 1986; Rikers, Loyens and Schmidt 2004). These studies have shown that novice clinicians have difficulties with both knowledge and strategy in clinical reasoning tasks. Speech and language therapy students must also learn to reason clinically, yet to date there is little evidence of how they learn to do so.
AIMS: In this paper, we report the clinical reasoning difficulties o…

INTRODUCTION: Clinical reasoning is essential for hospice and palliative medicine (HPM) fellows, yet structured assessment tools are lacking. The Assessment of Reasoning Tool (ART) has shown positive results for early learners but has not been applied to advanced learners.
METHODS: We conducted an 18-month mixed-methods study at a single academic center. Seven fellows and seven faculty participated. Faculty used the ART every day during clinical teaching rounds on a fellow's inpatient rotation. Pre- and post-intervention surveys assessed the ART via 4-point Likert scale and free-response ques…
