Cold Urticaria With Negative Standard Cold Stimulation Testing: Results From the COLD-CE Study.

BACKGROUND: Cold urticaria (ColdU) is defined by recurrent wheals and/or angioedema after cold exposure. Typical ColdU is confirmed by positive standard cold stimulation testing (SCST), defined as whealing within 10 minutes over the stimulated area on the volar forearm after provocation with an ice cube and/or TempTest. SCST-negative ColdU remains insufficiently characterized. OBJECTIVE: To characterize SCST-negative ColdU and identify factors independently associated with it. METHODS: We analyzed 364 patients with ColdU from the international, cross-sectional Comprehensive Evaluation of Col…

Cold Urticaria Preventing Clearance For Scientific Diving.

CASE DESCRIPTION: We present a case of a 39-year-old healthy female scientific diver who developed cold urticaria (CU) 8 months prior, when immediately postpartum. She had extensive diving experience but discontinued diving during pregnancy and sought to resume diving. Before our consultation, she had seen a dermatologist and allergist for evaluation for an underlying etiology and management. INTERVENTION: Initial management included diphenhydramine and cetirizine with topical triamcinolone based on dermatology recommendations. Her allergist later advised discontinuing diphenhydramine and rem…

Cold urticaria associated with acute serologic toxoplasmosis.

Cold urticaria is defined as a urticarial and/or angioedematous reaction of the skin to contact with cold objects, water or air. Types of urticaria associated with infectious diseases, such as mononucleosis, rubeola, varicella, syphilis, hepatitis, and HIV infection have been reported. We present the case of a patient who developed cold urticaria associated with acute serologic toxoplasmosis. The patient was a 34-year-old man who for the previous 2 months had presented cutaneous pruritus accompanied by several papular lesions in parts of the skin exposed to cold as well as those in contact wit…

Combined cold- and heat-induced cholinergic urticaria.

A 36-year-old white woman with a 20-year history of cutaneous, respiratory, and cardiovascular symptoms triggered by physical activity and by exposure to either heat or cold was evaluated. A routine evaluation for the cause of her condition was positive only for certain physical factors. Cutaneous testing for dermatographism, ice-cube challenge, and exposure to ultraviolet A and ultraviolet B light were negative. A methacholine skin test was positive. Sitting in a cold room (4 degrees C) induced micropapular wheals on exposed areas similar to those classically associated with cholinergic urtic

Cold urticaria. Clinical findings in 220 patients.

Patients with cold urticaria, a total of 220, were studied in Finland. Sixty-three percent of the patients were female. The diagnosis was based on a positive ice cube test in 90% of cases, and the other cold tests were needed to certify the diagnosis for the remainder of patients. The mean age at the onset of the disease was 25.1 years (range, 1-74), and the mean duration of symptoms was 6.3 years (range, 3 weeks to 37 years). Cold urticaria symptoms had disappeared in fifty-three patients (24%), but there was a recurrence of the disease in twelve. Idiopathic (primary acquired) cold urticaria …

Cold Agglutinins and Cryoglobulins Associate With Clinical and Laboratory Parameters of Cold Urticaria.

Mast cell-activating signals in cold urticaria are not yet well defined and are likely to be heterogeneous. Cold agglutinins and cryoglobulins have been described as factors possibly associated with cold urticaria, but their relevance has not been explained. We performed a single-center prospective cohort study of 35 cold urticaria patients. Cold agglutinin and cryoglobulin test results, demographics, detailed history data, cold stimulation test results, complete blood count values, C-reactive protein, total immunoglobulin E levels, and basal serum tryptase levels were analyzed. Forty six perc…

Cold-induced urticaria and angioedema: diagnosis and management.

The physical urticarias compromise a heterogeneous group of disorders characterized by their propensity to form urticarial lesions after exposure to certain physical stimuli, such as heat, vibration, or cold. Although generally benign, systemic hypotension and angioedematous airway compromise are known life-threatening complications. We present the case of a man who experienced benign angioedematous changes in response to cold exposure, and discuss the clinical course, diagnosis, and management of cold urticaria.

A Case of Cold Urticaria Diagnosed in the Emergency Department.

Cold urticaria can be primary (idiopathic) or secondary due to underlying hematologic or infectious diseases. Here, we present the case of a 19-year-old female patient with no past medical history who was diagnosed with cold urticaria in the emergency department (ED) setting using a cold stimulation test. Most cases are idiopathic. The reaction can be triggered in individual cases by exposure to cold objects or to generalized cold ambient temperatures, as was the case in the patient presented here. The physical response is most commonly pruritic wheals (urticaria). However, more severe symptom…

Cold contact urticaria triggered by a permanent tattoo.

Cold contact urticaria is an inducible urticaria triggered by exposure to cold air, liquids, or objects. Symptoms may range from mild, localized urticaria to genuine anaphylaxis particularly after extensive contact of large skin areas to cold. Treatment mainly requires a strict avoidance of cold exposure and the use of second-generation H1 antihistamine. In most cases, it is not associated with any specific etiology although in rare cases it can be secondary to various associated diseases or triggered by a number of external factors. We present the observation of a 38-year-old woman for whom c

Cold urticaria: a case report and review of the literature.

Cold urticaria represents a form of physical urticaria. The disorder is uncommon, and patients with the condition are at risk for systemic reactions and thus must be identified, counseled, and treated accordingly. Diagnosis principally is clinical and is confirmed by the results of cold stimulation tests such as placing an ice cube on the patient's forearm. Treatment primarily consists of preventive counseling, epinephrine autoinjections, and antihistamines. We present the case of a 9-year-old girl with acquired cold urticaria and review the literature.
