The colovesical fistula in congenital pouch colon: a histologic study.

PURPOSE: The aim of this study was to perform a detailed histopathologic examination of the terminal colonic pouch and the colovesical fistula (CVF) excised during surgical management of male patients with the more severe forms (types I/II) of congenital pouch colon (CPC) associated with anorectal agenesis. METHODS: From January 2005 to December 2006, 25 male patients with types I/II CPC underwent abdominal exploration with dissection of the terminal portion of the colonic pouch and associated CVF till the bladder, division-ligation of the fistula, and excision of the colonic pouch. In 6 of t

Congenital pouch colon syndrome: a report of 17 cases.

BACKGROUND: Congenital pouch syndrome (CPC) is a rare condition seen in association with anorectal malformation that occurs almost exclusively in northern India. We reviewed cases seen in our institution to study aspects of clinical presentation, diagnosis, embryogenesis and management and raise awareness of this relatively infrequent entity. PATIENTS AND METHODS: From March 2002 to September 2004, 17 neonates/infants (11 males and 6 females) treated for CPC associated with anorectal malformations included 13 with type IV and 4 with type I CPC. Diagnosis was made by a single large air-fluid l

Congenital pouch colon with prune belly syndrome and megalourethra.

Congenital pouch colon (CPC), an unusual high anorectal malformation in which a varying length of colon is replaced by a dilated pouch, has appeared in a number of case reports and series published in the English literature. There is only one case report of its association with prune belly syndrome (also known as triad syndrome, Eagle-Barrett syndrome, and abdominal muscle deficiency syndrome). We report the description and management of this rare association in a male neonate with CPC, prune belly syndrome, and congenital megalourethra.

Colonic J pouch reconstruction of the radiation-damaged neorectum.

BACKGROUND: Postoperative radiotherapy after restorative resection for rectal carcinoma can cause fibrosis, stricture and proctitis. There are few accounts in the literature of the surgical approach to the radiation-damaged neorectum. METHODS: Reported are four patients who underwent resection of the radiation-damaged neorectum with colonic J pouch reconstruction between September 1996 and July 1997. The operative and postoperative course is described. Parameters of bowel function were assessed by standardized interview 6 months after ileostomy reversal. Ongoing follow up has occurred to the

Radiologic Evaluation of Uterovaginal Abnormalities in Girls with Congenital Pouch Colon.

BACKGROUND/PURPOSE: The radiologic investigations of 25 girls with congenital pouch colon (CPC), managed over 17 years, were retrospectively reviewed. In 13 girls who form the study group, the investigations provided information about the anomalous uterovaginal (UV) anatomy and these findings were studied. MATERIALS AND METHODS: Age at presentation was 2 months to 10 years. The subtypes of CPC were Type I (n = 2), Type II (n = 9), and not recorded (n = 2). All patients had a double vagina and a unicornuate uterus on each side in the pelvis. The radiologic studies, performed at varying periods

A female patient with congenital pouch colon (CPC): a case report.

Congenital short colon, more commonly known as pouch colon, is a rare anomaly that can be encountered with anorectal malformations (ARM). The colon is shorter than normal and the distal end is dilated like a pouch. We report the case of a newborn with a Type 2 pouch colon. A female newborn was brought to our clinic with a diagnosis of cloaca anomaly. Her physical examination revealed a single canal introitus and flat perineum. In the abdominal x-ray taken in the upright position, a prominent air sac was noticed at the left side. A laparotomy was performed and exploration demonstrated that the

Duplication of pouch colon associated with duplication of the lower genitourinary tract.

The authors report on a baby with imperforate anus associated with duplication of descending colon, double pouch colon type IV, duplication of the urinary bladder, and a bifid penis. The interesting presentation of this problem and its management is discussed with a brief review of the probable embryologic basis for such an anomaly. Such a duplication of pouch colon has not been described previously.

The embryology and management of congenital pouch colon associated with anorectal agenesis.

Forty-one infants with a pouch colon malformation accompanied by a high anorectal anomaly were treated between January 1986 and December 1990. The 41 cases constituted 9% of all anorectal malformations and 15.2% of high defects managed during this period. There were 32 boys and nine girls; three of the girls had an associated cloaca. Many of the babies presented in poor condition, with gross abdominal distension caused by the distended colonic pouch. The typical radiological feature was an enormously distended colonic shadow occupying more than 50% of the width of the abdomen. At the time of s

Laparoscopic Excision of Douglas Pouch Fibroma in Contact With the Sigmoid Colon.

Pelvic tumors, especially those in difficult locations such as tumors in the rectouterine pouch in contact with the sigmoid colon, pose therapeutic approach difficulties due to the increased risk of complications, such as intestinal perforation. In practice, open surgical technique is often used even though laparoscopy has more advantages, due to lack of familiarity with delicate endoscopic maneuvers. This article describes the laparoscopic excision of a pelvic mass (residual fibroid after a previous myomectomy) implanted in the rectouterine pouch and in contact with the sigmoid colon. The pat

Congenital Pouch Colon in Males: Exploring the Risk of Epididymo-orchitis.

AIM: Patients with anorectal malformations (ARMs) are known to have a higher association with epididymo-orchitis (EO). Congenital pouch colon (CPC), a rare subtype of ARM, presents with several predisposing factors that are presumed to be linked with EO. This study aims to investigate the occurrence of EO in this specific subgroup of ARM patients to enhance the understanding of its etiology. MATERIALS AND METHODS: An ambispective observational study was conducted by analyzing medical records of male patients with CPC. Out of 28 patients managed primarily at our center, 25 fulfilled the inclus

Management of congenital pouch colon based on the Saxena-Mathur classification.

BACKGROUND: Management guidelines with regard to congenital pouch colon (CPC) are not clearly defined with regard to the type of pouch present. The aim of this study was to outline the management strategy and surgical approach to CPC using the Saxena-Mathur classification based on anatomical morphology of the pouch. METHODS: During a 12-year period (1995-2007), 426 patients were surgically managed for anorectal malformations at the RNT Medical College, Udaipur. Congenital pouch colon was documented in 80 patients and categorized into 5 types according the anatomical morphology. The management

Rapid construction of right colon pouch: initial clinical experience.

Between December 1992 and July 1993, 7 patients who underwent urinary diversion after cystectomy or bladder augmentation had colon pouches constructed with an "absorbable" staple. Our "one-step" technique of colon pouch construction incorporates standard principles of bowel detubularization and refashioning using absorbable GIA staples. The technique is easily learned and decreases operative time and blood loss. Herein we report follow-up of the first 7 patients undergoing this modification.

Small colonic J-pouch improves colonic retention of liquids--randomized, controlled trial with scintigraphy.

PURPOSE: A small 6-cm colonic J-pouch improves stool frequency and continence, without stool evacuation problems. However, the reservoir function is not improved on physiologic studies. Hence, a scintigraphy technique was devised to study the transit of solid and liquid colonic contents in these patients. METHODS: Patients were randomly assigned to straight or colonic J-pouch anal anastomoses after ultralow anterior resection. At 1 year after surgery, they were studied by bowel questionnaire, anorectal manometry, and technetium TC 99m tin-colloid liquid test meal/I-131 microcapsule scintigrap

Colectomy Followed by J-Pouch Reconstruction to Correct Total Colonic Aganglionosis.

BACKGROUND: Patients suffering from complete colonic aganglionosis (TCA) require the best surgical care possible. Only a few studies reported J-Pouch repair as the primary reconstructive surgery in TCA patients. This study adds to the current literature a thorough clinical and functional outcomes group. METHODS: Between 2011 and 2021, medical records of Hirschsprung disease (HD) patients who underwent J-Pouch reconstruction during infancy (n = 12) were reviewed. In close follow-up, bowel function and satisfaction with operation results were evaluated. The median age at the time of J-Pouch rec
