Risk factor for colorectal neoplasms: obstructive sleep apnea or nocturnal hypoxemia?

OBJECTIVE: The objective of this study is to determine whether nocturnal hypoxemia, as compared to obstructive sleep apnea (OSA), serves as … hypertension and smoking habits. CONCLUSIONS: Nocturnal hypoxemia is associated with occurrence of colorectal neoplasms, especially moderate and severe hypoxemia. Across various subgroups, hypoxemia still remained as an independent adverse predictor for colorectal neoplasm, among individuals aged 40 and older, without overweight, hypertension and smoking habits.

Association between Clostridium septicum and Streptococcus gallolyticus bacteremia and colorectal neoplasms.

PURPOSE: Colorectal cancer (CRC) is associated with Clostridium septicum (CS), Streptococcus gallolyticus (SG), and Fusobacterium nucleatum (FN) bacteremia. Further investigation of the mechanism of translocation and the time interval between bacteremia and a CRC diagnosis is required. METHODS: Patients with bacteremia caused by the three organisms were studied. The organisms were identified using MALDI-TOF mass spectrometry. RESULTS: Fourteen patients (CS [n = 1], SG [n = 11], and FN [n = 2]) were included among 2866 patients with bacteremia. Colorectal neoplasms were detected in two patien…

Endoscopic Submucosal Dissection for Colorectal Neoplasms: Is the West Catching Up?

Endoscopic submucosal dissection (ESD) has revolutionized the management of superficial colorectal neoplasms, offering superior en bloc resection rates compared with conventional endoscopic mucosal resection (EMR). While ESD has been the standard of care in East Asian countries for over two decades, its adoption in Western countries has been considerably slower, hampered by the steep learning curve, prolonged procedural times, limited training infrastructure, and differences in disease epidemiology. However, recent years have witnessed a paradigm shift, with growing evidence from Western multi…

Transabdominal Ultrasound Colonography for Detection of Colorectal Neoplasms: Initial Clinical Experience.

We investigated the feasibility of using ultrasound colonography (USC) to visualize the healthy colon and rectum and detect colorectal polyps. Eight healthy volunteers underwent USC after standard bowel preparation. The feasibility and image quality of USC in different segments were evaluated. Then, USC was conducted on eight patients with known colonic neoplasms using colonoscopy as the reference standard. For volunteers, USC examinations were successfully performed on four (50.0%) ascending, three (37.5%) transverse and eight (100%) descending colons, as well as all sigmoid colons and rectum

Bowel preparation and colonoscopy technique to detect non-polypoid colorectal neoplasms.

Colonoscopy is considered the gold standard for colon cancer screening. In a recent study, however, 0.3% to 0.9% patients developed colorectal cancer within 3 years after removal of adenomas. Some reasons for the development of interval colorectal cancers include missed or incompletely removed lesions during the initial colonoscopy. Non-polypoid colorectal neoplasms are a potential contributor to the pool of missed lesions because they can be easily missed as a result of inadequate colon preparation or examination technique. This article discusses the methods that are useful to improve the qua…

Expression of CuZn- and Mn-superoxide dismutase in human colorectal neoplasms.

Decreased intracellular SOD protein levels and activity have been related with malignancy in the past. To investigate their relevance in the carcinogenetic process in the colon, we determined quantitatively CuZn-SOD and Mn-SOD levels and total SOD activity by histochemical means in human normal colorectal mucosa, adenomas, and carcinomas. Protein levels and activity were significantly decreased in carcinomas. CuZn-SOD protein levels, but not Mn-SOD levels or total SOD activity were related with differentiation grade and to a lesser extent with Dukes stage. Moderately differentiated carcinomas

Role of robotic natural orifice specimen extraction surgery in colorectal neoplasms.

Natural orifice specimen extraction surgery (NOSES) is especially suitable for colorectal surgery. Until now, most of the reports published were about laparoscopic NOSES, the reports about robotic NOSES are extremely rare. This study aims to explore the safety and feasibility of robotic NOSES for colorectal neoplasms. All patients underwent robotic NOSES from March 2016 to October 2019 in our hospital were enrolled for retrospective analysis. Clinicopathological data including patient characteristics, perioperative information and pathological information were collected and analyzed. According…

Risk factors for bleeding after endoscopic submucosal dissection of colorectal neoplasms.

AIM: To investigate the risk factors for delayed bleeding following endoscopic submucosal dissection (ESD) treatment for colorectal neoplasms. METHODS: We retrospectively reviewed the medical records of 317 consecutive patients with 325 lesions who underwent ESD for superficial colorectal neoplasms at our hospital from January 2009 to June 2013. Delayed post-ESD bleeding was defined as bleeding that resulted in overt hematochezia 6 h to 30 d after ESD and the observation of bleeding spots as confirmed by repeat colonoscopy or a required blood transfusion. We analyzed the relationship between …

Endoscopic mucosal resection of superficial colorectal neoplasms: review of 140 procedures.

INTRODUCTION AND AIMS: Endoscopic mucosal resection (EMR) has been shown to be useful in the removal of large colorectal sessile and flat lesions, avoiding the need for surgical resection. The aim of this study was to evaluate the efficacy and safety of EMR in colorectal lesions using the inject-and-cut technique. MATERIAL AND METHODS: Based on the review of colonoscopy reports, performed from February 2007 and February 2010, resected lesions = 10 mm in diameter were selected for the study. The endoscopic and histologic characteristics, complications, follow-up and surgical need were recorded

Association between non-diminutive hyperplastic polyps and synchronous advanced colorectal neoplasms.

BACKGROUND/AIMS: Many studies have suggested that hyperplastic polyps (HPs) are associated with adenomas and even malignancy. We investigated whether or not HPs are associated with the presence of synchronous advanced colorectal neoplasms as a function of their size and location. METHODS: A total of 2,482 asymptomatic patients who underwent screening colonoscopies were classified into the following two groups: cases with advanced neoplasms and controls without advanced neoplasms. The proximal colon was defined as all segments proximal to the splenic flexure. We defined diminutive HPs as <6 mm…

Safety and reliability of tattooing colorectal neoplasms prior to laparoscopic resection.

Accurate tumor localization is critical to performing minimally invasive colorectal resection. This study reviews the safety and reliability of tattooing colorectal neoplasms prior to laparoscopic resection. We retrospectively reviewed 50 consecutive patients with colorectal neoplasms who underwent endoscopic tattooing prior to laparoscopic resection. Data were obtained from medical charts, endoscopy records, and pathology reports. No complications related to endoscopy or tattooing were incurred. Five neoplasms (10%) were in the ascending colon, five (10%) were in the transverse colon, eight (…

The comparison of risk factors for colorectal neoplasms at different anatomical sites.

AIM: Both the clinical manifestation and molecular characteristics of colorectal cancer (CRC) vary according to the anatomical site. We explored the risk factors for four groups of colorectal neoplasms (CRN) at different anatomical sites. METHODS: We extracted data from the database of Tianjin Colorectal Cancer Screening Program from 2010 to 2020. According to the CRN anatomical sites, patients were divided into four groups: the proximal colon group, the distal colon group, the rectum group, and the multiple colorectal sites. Binary logistic regression analysis was used to explore the differe…

Nonalcoholic steatohepatitis is associated with a higher risk of advanced colorectal neoplasm.

BACKGROUND & AIMS: Nonalcoholic fatty liver disease (NAFLD) is known to increase the risk of adenomatous colonic polyps. However, the role of screening colonoscopy in patients with biopsy-proven NAFLD in detecting advanced colorectal neoplasm is not clearly evidence-based. Therefore, we investigated whether the histological severity of NAFLD is associated with advanced colorectal neoplasm. METHODS: This study included patients ≥18 years old who underwent screening colonoscopy between 2013 and 2018 within a biopsy-evaluated prospective NAFLD cohort. Advanced colorectal neoplasm was defined as

Metabolic factors affect the occurrence of colorectal neoplasm on surveillance colonoscopies.

BACKGROUND AND AIM: Although metabolic factors such as obesity and hyperlipidemia were reported to be associated with high prevalence of colorectal neoplasm (CRN), their influence on the occurrence of CRN at surveillance colonoscopy has not been clarified. The purpose of this study was to analyze the association between metabolic factors and the risk of CRN at the time of surveillance colonoscopy. METHODS: We reviewed the medical records of 1792 asymptomatic subjects (average 52.1 years, 1233 male) who underwent screening and follow-up surveillance colonoscopies. Fasting glucose level, fastin

Klebsiella Pneumoniae Liver Abscess as the Herald of Underlying Colorectal Neoplasm.

BACKGROUND: Klebsiella pneumoniae liver abscesses (KPLA) remain endemic to Taiwan. Hypervirulent strains in the gut can translocate across colonic mucosal defects into the portal circulation, causing subsequent liver abscesses. Occult colorectal cancer in patients with KPLA has been reported in a case series. Herein, we determined whether the diagnosis of KPLA serves as a harbinger of colorectal neoplasms. METHODS: Consecutive patients with KPLA treated at Taipei Veterans General Hospital in Taiwan between April 2011 and January 2024 were retrospectively enrolled. Hypervirulent K. pneumoniae …

Vitamin D status and colorectal neoplasm risk in Lynch syndrome.

BACKGROUND: Higher circulating vitamin D concentrations have been associated with a decreased colorectal neoplasm (CRN) risk in the general population. However, this association has not yet been investigated in individuals with Lynch syndrome (LS), an inherited predisposition to colorectal cancer. METHODS: The study population included 2,351 LS carriers from the GEOLynch and the Colon Cancer Family Registry cohorts. Total plasma 25-hydroxyvitamin D (25(OH)D2 and 25(OH)D3) concentrations were measured using isotope-dilution Liquid Chromatography-tandem Mass Spectrometry. CRNs included adenoma,
