Regarding the question of an adequate depth of anesthesia, over the past decade anesthesiologists have focused on the prevention of intraoperative consciousness in combination with explicit memory. Recent studies approached the topic from a different way postulating that deep anesthesia, quantified as time with a bispectral index (BIS)< 45, is associated with increased postoperative mortality and four out of the five published studies revealed such a correlation. However, the finding is limited by a suboptimal study design, e.g. none of the studies presented randomized data. Furthermore, it is

BACKGROUND: Several studies have suggested that monitoring the depth of anaesthesia might prevent the development of postoperative cognitive decline. We aimed to conduct a meta-analysis to investigate the effects of bispectral index (BIS) monitoring in anaesthesia.
METHODS: We searched in six major electronic databases. Trials were included if they discussed anaesthesia with and without BIS monitoring or low (<50) and high (≥50) BIS levels and which measured the risk of postoperative delirium (POD) and/or postoperative cognitive dysfunction (POCD).
RESULTS: We included fourteen studies in th

BACKGROUND Our study aimed to test the predictive value of the bispectral index (BIS) for the post-operational consciousness recovery in patients undergone hematoma evacuation due to spontaneous intracerebral hemorrhage (ICH). MATERIAL AND METHODS In this prospective cohort study, we enrolled adult spontaneous ICH patients after surgical hematoma evacuation who did not recover consciousness on the first postoperative day. After patient enrollment, the BIS was continuously monitored for 12 hours, and the motor response on the Glasgow Coma Scale (GCS-M) was evaluated. The patients were followed

BACKGROUND: Low bispectral index (BIS) and low mean arterial pressure (MAP) are associated with worse outcomes after surgery. We tested the hypothesis that a combination of these risk factors, a 'double low', is associated with death and major complications after cardiac surgery.
METHODS: We used data from 8239 cardiac surgical patients from two US hospitals. The primary outcomes were 30-day mortality and a composite of in-hospital mortality and morbidity. We examined whether patients who had a case-averaged double low, defined as time-weighted average BIS and MAP (calculated over an entire c

Anaesthesia awareness (AA) is postoperative recall of events experienced under general anaesthesia. Most frequently patients remember an auditory perception, the feeling of motor function lost, pain, helplessness, anxiety, panic, impending death. The prevalence of awareness in nonobstetric and noncardiac surgical cases is 0.1%-0.2%. The prevalence is higher in cardiac surgery, obstetric and major trauma cases. According to the results of many studies light anaesthesia is the most common cause of the AA. Posttraumatic stress disorder appears in 33%-56% of patients who experienced awareness duri

Spravato and other drugs with consciousness-altering effects show significant promise for treating various mental health disorders. However, the effects of these treatments necessitate a substantial degree of patient monitoring which can be burdensome to healthcare providers and may make these treatments less accessible for prospective patients. Continuous passive monitoring via digital devices may be useful in reducing this burden. This proof-of-concept study tested the MindMed Session Monitoring System™ (MSMS™), a continuous passive monitoring system intended for use during treatment session

OBJECTIVE: Whether consciousness has a causal role in voluntary movements is not clear. Backward masking blocks a stimulus from becoming conscious, but it can trigger movement in a reaction time paradigm. We hypothesize that if backward masking is used in a choice reaction time paradigm, when the visible stimulus (S2) differs from the masked stimulus (S1), the movement will often differ from conscious intent. We did such a study employing electroencephalography (EEG) to explore the brain activity associated with this effect.
METHODS: Twenty healthy adults participated in a choice reaction tim

BACKGROUND: Fospropofol, a water-soluble prodrug of propofol, is metabolized into propofol by alkaline phosphatase after administration. This study aimed to develop a pharmacokinetic-pharmacodynamic (PK-PD) model that correlates the propofol concentration in exhaled air (Ce-pro-f) with its anesthetic effects, as measured by the bispectral index (BIS) in beagles.
METHODS: Beagles receiving a single intravenous infusion of fospropofol at varying doses were divided into three groups (n = 6): the DBL-fospro group (15 mg/kg), the DBM-fospro group (30 mg/kg), and the DBH-fospro group (60 mg/kg). Pr

Bispectral index with density spectral array (BIS-DSA) monitoring during hyperbaric oxygen therapy of a case with inner ear and cerebral decompression sickness is described. During the initial treatment, a particular DSA pattern was found, which resolved after four treatments. Clinical resolution of the symptoms accompanied this improvement. The particular BIS-DSA pattern described in this case is concordant with a potential hypo-perfusion of the cortex related to decompression stress. This case suggests that BIS-DSA monitoring may be an easy, cost-effective, and viable form of neuro-monitorin

INTRODUCTION: The Bispectral Index (BIS) provides an objective measure of the level of sedation and general anesthesia. We performed this meta-analysis and trial sequential analysis (TSA) of randomized clinical trials to clarify whether BIS monitoring is helpful in enhancing intraprocedual safety, shortening procedure duration or promoting recovery during sedation for endoscopic procedures.
EVIDENCE ACQUISITION: PubMed, EMBASE, CINAHL, the Cochrane Central Register of Controlled Trials [CENTRAL]) databases, reference lists of articles as well as relevant articles from "Google Scholar" were se

BACKGROUND: Closed-loop systems for automated hypnotic delivery integrate continuous bispectral index (BIS) monitoring with controller algorithms to adjust infusion rates in real time. Unlike earlier syntheses that focused on aggregate control metrics, we frame anaesthetic depth as an asymmetric risk problem by separating exposure to potentially inadequate anaesthesia from exposure to excessive depth of anaesthesia.
METHODS: This PROSPERO-registered systematic review (CRD420251208039) searched PubMed, Embase, Cochrane CENTRAL, and Web of Science through November 2025. Randomised trials compar

INTRODUCTION: Whether anesthesia depth affects postoperative mortality remains uncertain.
MEASUREMENTS: Several databases were systematically searched to identify all articles studying the relationship between depth of anesthesia and postoperative mortality. Post hoc subgroup analyses were conducted for follow-up period (30 days vs. longer than 90 days) and type of surgery.
MAIN RESULTS: The analysis included 38,722 patients from nine studies. We observed a significant relationship between low bispectral index (BIS) and mortality (pooled aHR, 1.22;95% CI, 1.08 to 1.38; P = 0.001; I2 = 85.4%)

The bispectral (BIS) monitor uses brain electroencephalographic data to measure the depth of sedation and pharmacological response during anaesthetic procedures. In this case, the BIS monitor was used for another purpose, to demonstrate postoperatively to the nursing staff that a patient with history of locked-in syndrome (LIS), who underwent pressure ulcer debridement, had periods of wakefulness and apparent sensation, even with his eyes closed. Furthermore, as patients with LIS can feel pain, despite being unable to move, local block or general anaesthesia should be provided for sharp surgic

Background and Objectives: No processed electroencephalographic index is a gold standard for anesthetic depth. We compared the Cortical Activity Index (CAI) with the bispectral index (BIS) as a pragmatic, widely studied clinical comparator, without assuming that BIS measures the true hypnotic state or that CAI is superior. Materials and Methods: This single-center prospective observational study enrolled 100 adult surgical patients; seven cases with acquisition-device data loss were excluded according to the final research-results report, leaving 93 participants for analysis. Locked-in syndrome is an extremely rare neurological state caused by injury of the ventral pons. The syndrome is characterized by quadriplegia and anarthria with concomitant preservation of cortical function. When a reversible underlying pathological abnormality is identified and managed aggressively, meaningful recovery is possible. Because patients retain consciousness throughout their illness, a dependable method for titrating sedation may improve their quality of life. The case presented suggests that bispectral index monitoring may be a cost-effective and reliable method for managing seda

Diagnosis of ictal discharges during epileptic activity should be based on raw electroencephalography (EEG). The apparatus necessary for such a diagnosis would typically require time to properly set up and operate. The bispectral index (BIS), an EEG-derived parameter that is easy to establish and easy to use, has been shown to correlate with numerous EEG conditions and may represent a useful tool. Acute encephalitis with refractory, repetitive, partial seizures (AERRPS) is an epileptic syndrome first described by Sakuma in Japan in 2001. We assessed the utility of using BIS monitoring as a dia

The neural substrates of consciousness remain elusive. Competing theories that attempt to explain consciousness disagree on the contribution of frontal versus posterior cortex and omit subcortical influences. This lack of understanding impedes the ability to monitor consciousness, which can lead to adverse clinical consequences. To test substrates and measures of consciousness, we recorded simultaneously from frontal cortex, parietal cortex, and subcortical structures, the striatum and thalamus, in awake, sleeping, and anesthetized macaques. We manipulated consciousness on a finer scale using

BACKGROUND: Processed electroencephalography (EEG) monitors such as the bispectral index (BIS) and patient state index (PSI), are used clinically to estimate anesthetic depth, yet their algorithmic design obscures how closely these indices reflect underlying neural complexity. Entropy-based analyses, grounded in information theory, provide a quantitative framework for characterizing EEG signal irregularity and have been proposed as physiologically interpretable alternatives. However, the relationship between these commercial indices and theoretical entropy measures remains unclear.
