Penetrating corneal fish-hook injury.

A healthy 13-year-old female was struck in the left eye by a fish-hook while fishing. The fish-hook penetrated the cornea, passed the anterior chamber and exited from a second corneal site near the limbus. The fish-hook was removed by cutting its shank and pushing it out of the wounds. Visual acuity was 20/20 OS after surgery. Mild corneal scarring was present at the penetration sites. We would like to report this case because there is a limited number of patients with penetrating corneal fish-hook injuries and this case presents the only female patient in the ophthalmological literature who s

OcuPair, a Novel Photo-crosslinkable PAMAM Dendrimer-Hyaluronic Acid Hydrogel Bandage/Bioadhesive for Corneal Injuries and Temporary Corneal Repair.

Traumatic corneal injuries are a leading cause of blindness among military personnel. These injuries need immediate attention in the combat zone, but treatment options are limited as life-saving measures are often prioritized. To address this critical gap, we have developed OcuPairTM, a two-component hydrogel system that consists of i) an injectable viscoelastic filler that stabilizes the ocular cavity and prevents hypotony. ii) An in situ photo-curable adhesive hydrogel comprising of methacrylated, hydroxyl PAMAM dendrimer and hyaluronic acid engineered to form a transparent, flexible, and ro…

Physicochemical and safety evaluation of an antioxidant ophthalmic hydrogel for corneal injuries.

Corneal injuries are frequently associated with oxidative stress and standard treatment are only to prevent infection, reduce pain and inflammation. This work describes the development of antioxidant ophthalmic hydrogels containing Idebenone (IDB) and hydroxocobalamin (HCB) as a complement treatment for corneal injuries, using hydroxypropyl- β-cyclodextrin (HPβCD) to improve IDB aqueous solubility. Three hydrogels were prepared (hyaluronic acid, FAH; poloxamer 407, FP; and polyvinyl alcohol, FPVA) and characterized for physicochemical properties, mucoadhesion and safety. Phase-solubility assay…

[Low-intensity helium-neon laser irradiation in multimodal treatment of corneal injuries].

He-Ne laser stimulation was used in multiple-modality treatment of 512 patients with various corneal conditions: 217 with perforating wounds, 75 with chemical and thermal burns, 48 with ulcers of various origins, and 162 with endothelial-epithelial dystrophy resulting from cataract extraction with implantation of the intraocular lens. A course of treatment consisted of 6 to 10 daily 3 min exposures at radiation power density of 50-100 micro W/cm2 on the cornea. The effect achieved by laser stimulation was significantly superior to that of routine methods of treatment used in the reference grou

Corneal injury repair and the potential involvement of ZEB1.

The cornea, consisting of three cellular and two non-cellular layers, is the outermost part of the eyeball and frequently injured by external physical, chemical, and microbial insults. The epithelial-to-mesenchymal transition (EMT) plays a crucial role in the repair of corneal injuries. Zinc finger E-box binding homeobox 1 (ZEB1), an important transcription factor involved in EMT, is expressed in the corneal tissues. It regulates cell activities like migration, transformation, and proliferation, and thereby affects tissue inflammation, fibrosis, tumor metastasis, and necrosis by mediating vari…

Corneal bee sting injury: a clinical conundrum.

This study outlines the clinical features and comprehensive management of three cases of corneal injury by a bee sting. The stinger removal was attempted, and in two cases, the stinger was successfully removed. In one case, a portion of the stinger broke and was left in situ without inciting any further inflammation. In all the cases, the surrounding infiltrate was scraped, and after microbiological evaluation was found to be negative, topical corticosteroids were started. The management of corneal bee sting injuries should include appropriate surgical intervention along with the judicious use

The corneal fibrosis response to epithelial-stromal injury.

The corneal wound healing response, including the development of stromal opacity in some eyes, is a process that often leads to scarring that occurs after injury, surgery or infection to the cornea. Immediately after epithelial and stromal injury, a complex sequence of processes contributes to wound repair and regeneration of normal corneal structure and function. In some corneas, however, often depending on the type and extent of injury, the response may also lead to the development of mature vimentin+ α-smooth muscle actin+ desmin+ myofibroblasts. Myofibroblasts are specialized fibroblastic

Corneal higher-order aberrations in corneal endothelial decompensation secondary to obstetric forceps injury.

Forceps corneal injuries during infant delivery cause Descemet membrane (DM) breaks, that cause corneal astigmatism and corneal endothelial decompensation. The aim of this study is to characterise corneal higher-order aberrations (HOAs) and corneal topographic patterns in corneal endothelial decompensation due to obstetric forceps injury. This retrospective study included 23 eyes of 21 patients (54.0 ± 9.0 years old) with forceps corneal injury, and 18 healthy controls. HOAs and coma aberrations were significantly larger in forceps injury (1.05 [0.76-1.98] μm, and 0.83 [0.58-1.69], respectivel…

Dry eye predisposes to corneal neovascularization and lymphangiogenesis after corneal injury in a murine model.

PURPOSE: Dry eye disease is becoming recognized as an immune-inflammation mediated disorder. Surgical insults such as corneal incision or suture can aggravate dry eye. We sought to determine whether underlying dry eye aggravates corneal inflammatory infiltration, hemangiogenesis, and lymphangiogenesis (LY) induced by surgical injury in a murine model. METHODS: We used treatment arms; one, normal eye (non-dry eye) and the other, a scopolamine-induced dry eye model. We first compared the corneas of both groups on which no surgery was performed with confocal and fluorescent microscopy. In subgro

Non-penetrating corneal foreign body injuries: factors affecting delay in rehabilitation of patients.

A review of 783 patients with non-penetrating, superficial corneal foreign bodies (FBs), indicated that delay in rehabilitation was related to two factors: (1) the size of the abrasion following removal of the FB, larger abrasions requiring longer duration of antibiotic ointment, and (2) inadequate removal of corneal rust. Allergy to chloramphenicol 1% ointment (5.5 in 1000), commonly used in the management of corneal abrasions, is unpredictable and can also impair rapid rehabilitation.

Corneal manifestations in chemical injury with stannous chloride.

Chemical injuries are potentially devastating ocular accidents that can lead to permanent damage to the ocular surface and visual loss. The majority of serious chemical injuries occur in factory workers who are exposed to hazardous chemicals in their day-to-day life. The ocular effects of commonly encountered acids and alkalis are well documented in the literature. Here we report briefly on the corneal and other ocular effects of stannous chloride, which is sparsely reported in the currently available literature.
