Mitomycin Intravascular Chemoembolization for Corneal Neovascularization.

PURPOSE: The purpose of the study was to evaluate the efficacy of mitomycin C intravascular chemoembolization (MICE) for corneal neovascularization (CNV). METHODS: This is a prospective, nonrandomized, interventional study. Patients with stable CNV irrespective of the underlying etiology were enrolled in the study. 0.1% mitomycin C was injected intravascularly in CNV at the slit lamp under topical anesthesia. To evaluate the regression of vessels, the ImageJ software was used to trace and quantify vasculature by a pixel count in standardized clinical slit-lamp photographs. RESULTS: Eight eye…

[Imaging and quantification of corneal neovascularization].

Corneal neovascularization is defined as the invasion of vessels into the normally avascular clear corneal stroma, secondary to acute or chronic tissue injury. In addition to decreasing visual acuity, vascularity introduces circulating immune cells, reducing corneal immune privilege and the graft survival of subsequent keratoplasty. Thus, reducing neovascularization has become a recent therapeutic target in order to increase the success of corneal transplantation. Comparing the effects of antiangiogenic drugs assumes that we are able to quantify corneal neovascularization before and after trea…

Treatments for corneal neovascularization: a review.

Corneal neovascularization (CNV) may be a physiological response to various stimuli, but a chronic and persistent upregulation of neoangiogenesis can result in pathological CNV. Pathological blood vessels are immature and lack structural integrity, predisposing the cornea to lipid exudation, inflammation, and scarring. CNV can therefore become a potentially blinding condition. In this review, we frame CNV in an epidemiological perspective, consider risk factors for CNV, provide an overview of CNV pathogenesis, and consider the impact of CNV on corneal transplantation. We consider treatments th…

Anti-VEGF Treatment of Corneal Neovascularization.

Vascular endothelial growth factor (VEGF) is an angiogenic factor shown to be a critical secreted cytokine in tumorigenesis and retinal neovascularization (NV). Currently, there are two anti-VEGF agents, pegaptanib and ranizumab, approved by the United States Food and Drug Administration (FDA) for intravitreal use in the treatment of wet age-related macular degeneration (AMD). Bevacizumab is FDA-approved for intravenous administration in the treatment of several cancers and is in widespread use, off-label, as an intravitreal injection to treat a variety of retinal pathologies. Animal studies d

[Mechanisms and regulation of corneal neovascularization].

The cornea is a transparent tissue and its transparency is dependent on many factors including avascularity. However, in response to a stimulation such as inflammation, neovasculature from the limbal plexus can invade the transparent cornea. Neovascularization in the corneal tissue induces unwanted opacification, which can result in significant reduction in visual function. Furthermore, corneal neovascularization is the main risk factor for rejection after keratoplasty. Although anti-angiogenic therapy is a promising approach to reduce corneal opacity and immune rejection secondary to keratopl…

Subconjunctival bevacizumab injection for corneal neovascularization.

PURPOSE: To report on the clinical use of subconjunctival bevacizumab in patients with corneal neovascularization. METHODS: The charts of 10 consecutive patients with corneal neovascularization who received subconjunctival injections of bevacizumab (2.5 mg/0.1 mL) were reviewed. Digital photographs of the cornea were graded by 2 masked observers for density, extent, and centricity of corneal vascularization. Image analysis was used to determine the area of cornea covered by neovascularization as a percentage of the total corneal area. RESULTS: No significant ocular or systemic adverse events…

Suppression of corneal neovascularization with cyclosporine.

We sought to determine if cyclosporine, which has been shown to suppress corneal allograft rejection, could also suppress corneal neovascularization induced by interleukin 2. Thirty A/J mice were treated with daily intramuscular injections of cyclosporine (25 mg/kg in olive oil) for 3 days before and 2 weeks following the intrastromal injection of 0.5 microL (5 IU) of recombinant mouse interleukin 2. Controls received intramuscular injections of olive oil. The mean area of corneal neovascularization 4, 8, and 12 weeks after injection was 9.2, 9.1, and 9.2 mm2, respectively, in controls, and 5.…

Corneal stromal stem cell-derived extracellular vesicles inhibit corneal neovascularization.

PURPOSE: Corneal neovascularization (CNV) threatens corneal transparency and is a leading cause of vision loss. This study evaluates the anti-angiogenic effects of extracellular vesicles (EVs) derived from human corneal stromal stem cells (CSSCs). METHODS: Cultured human umbilical vein endothelial cells (HUVECs) were treated with CSSC-EVs to examine the influence on cell growth and modulation of angiogenesis using xCELLigence and tube formation assays. In vivo, a mouse model of CNV was induced by silver nitrate (AgNO₃) cauterization, and the injured corneas were treated with CSSC-EV eye drops…

The effect of corneal trephination on neovascularization.

In order to investigate a neural influence on corneal neovascularization, we attempted to induce new vessel growth within a denervated zone produced by trephination. The stimulus for vascularization was cauterization central to the trephined margin. We demonstrated that in corneas denervated and stimulated in this manner neovascularization did not occur. This finding supports our hypothesis of a neural influence on neovascularization.

Corneal melting following intrastromal injection of bevacizumab for corneal neovascularization.

The aim is to report corneal melting following intrastromal injection of antivascular endothelial growth factor (anti-VEGF), bevacizumab, for pre-existing corneal neovascularization. A 68-year-old male with a history of failed bilateral penetrating keratoplasties due to scarring from childhood measles and trachoma underwent right eye superficial keratectomy, intrastromal bevacizumab, and amniotic membrane transplant to control the neovascularization in an overseas center. A week later developed corneal melting. In conclusion, anti-VEGF can be used in controlling corneal neovascularization, alb…

Delayed neovascularization in inflammation-induced corneal neovascularization in interleukin-10-deficient mice.

PURPOSE: To investigate the potential modulatory role of interleukin-10 (IL-10) in the suture model for corneal neovascularization. METHODS: Neovascularized areas were measured on corneal flat-mounts in IL-10(-/-) and wild-type C57BL6 mice. The inflammatory cellular response was characterized with immunohistochemistry. Gene expression was measured by real-time polymerase chain reaction. RESULTS: IL-10(-/-) mice showed a delayed neovascular response compared to wild-type animals at day 6 after suture, when approximately half of the cornea was neovascularized. No apparent differences in inflam…
