PURPOSE: To document the development of corneal perforation after using gatifloxacin to treat inflamed pterygium.
METHODS: A 30-year-old man developed corneal perforation after using gatifloxacin to treat inflamed pterygium.
RESULTS: After treatment with gatifloxacin was stopped, the perforation was managed by applying cyanoacrylate glue, followed by pterygium excision.
CONCLUSIONS: If used in the management of inflamed pterygium, gatifloxacin may predispose patients to corneal perforation.

An 18-year-old white man with severe staphylococcal blepharokeratoconjunctivitis of several years' duration developed phylctenules intermittently. At age 22 an active corneal phlyctenule caused perforation of the cornea. Seven days after this occurrence, the perforation closed spontaneously but perforated again ten days later. This occurrence may have been caused by an increase in the patient's hypersensitivity to the Staphylococcus as a result of concurrent viral infection, or it may have been due to the patient's failure to return for treatment at the scheduled time. The authors present a case of severe gonococcal conjunctivitis associated with corneal perforation of the right eye in a 25-year-old homosexual man. Inpatient management and corneal grafting were required. The authors demonstrate that Neisseria gonorrhoea should be considered in the presence of purulent conjunctival discharge with a white patch on the cornea or reduced vision. Regardless of whether a patient has genital symptoms, they should be referred urgently to an ophthalmologist to ensure adequate treatment of this rare but sight-threatening complication.

Eighty patients with either corneal perforation or impending perforation were treated with the application of tissue adhesive that remained in place on the average of 50 days. Mitomycin intravascular chemoembolization (MICE) is a potential treatment for corneal neovascularization (NV) with encouraging early outcomes. This apparently is the first reported case of corneal perforation due to a mixed infection of Helminthosporium species and Mima polymorpha. These 2 organisms separately rarely cause ocular infection and when present together could produce serious damage to the eye. In long-standing diseases of the cornea, mixed infection should be suspected when the infection fails to respond to conventional medical therapy. Thorough laboratory evaluation should be done. Selection of appropriate antibiotics should be based on clinical impression, initial Gram stain findings and final cultural results.
