Cryptorchidism and infertility.

Although the maldescended or dystopic testis is one of the most common congenital anomalies in man, proper treatment remains a subject of much discussion and confusion. Certainly, in light of the previously described findings, the future fertility of the patient with unilateral cryptorchidism, regardless of age of correction, should be a realistic concern. Hormonal treatment should be recognized as a good diagnostic test for defining the retractile testes and should not be considered therapy. The pediatrician or family practitioner should be encouraged to examine the scrotal contents thoroughl…

Laparoscopy for cryptorchidism.

Laparoscopy is suggested as an accurate way to localize the non-palpable testicle or to diagnose anorchia without an abdominal exploration. From our early experience laparoscopy will aid greatly in the management of the intra-abdominal testicle.

Unilateral abdominal cryptorchidism.

The unilateral non-palpable undescended testis is considered separately from other forms of cryptorchidism. It is less likely to be fertile, it is more prone to malignancy and it is more difficult to place. Removal rather than orchiopexy often is in the best interests of the child.

Pathogenesis of cryptorchidism.

Cryptorchidism was induced experimentally by treating pregnant mice on the 14th day of pregnancy with 5 mg estrogen. Testes from cryptorchid and control newborn and adult mice were investigated with radioimmunoassay and electron microscopy. It was concluded that a normal Leydig cell function plays a decisive role in testicular descent. In cryptorchidism, Leydig cells at birth are atrophic. Testicular testosterone content is diminished as compared to controls. Ultrastructural alterations of Leydig cells observed in our experiments closely resemble those found in biopsies of cryptorchid patients…

Hypogonadism and Cryptorchidism.

Congenital cryptorchidism (undescended testis) is one of the most common congenital urogenital malformations in boys. Prevalence of cryptorchidism at birth among boys born with normal birth weight ranges from 1.8 to 8.4%. Cryptorchidism is associated with a risk of low semen quality and an increased risk of testicular germ cell tumors. Testicular hormones, androgens and insulin-like peptide 3 (INSL3), have an essential role in the process of testicular descent from intra-abdominal position into the scrotum in fetal life. This explains the increased prevalence of cryptorchidism among boys with …

[Cryptorchidism and fertility].

BACKGROUND: Through the completion of the first year of life, cryptorchidism is the most common genital malformation in pediatric urology; unilateral cryptorchidism is detected in about 1% of full-term male infants and a fifth (20%) of children are bilaterally affected. DISCUSSION: It must be understood that azoospemia affects about 10% of men with unilateral and about 32% of men with bilateral cryptorchidism and that approximately 10-20% of men, in whom infertility has been diagnosed, have a history of orchiopexy. AIM: What factors influencing the fate of postpubertal spermatogenesis are de…

Cryptorchidism and Fertility.

Congenital cryptorchidism, that is, undescended testis, is one of the most common urogenital abnormalities observed in newborn boys. In addition to the congenital form, there is also acquired form of cryptorchidism. Fertility potential of patients with cryptorchidism has been evaluated by testicular histology and volume, semen quality, reproductive hormone levels, time to conception, and paternity rates. Cryptorchidism is associated with abnormalities in testicular development, and early treatment is recommended to optimize the fertility potential of the patients.

Cryptorchidism and Fertility.

Cryptorchidism, the failure of one or both testes to descend into the scrotum prenatally, occurs in 2.4%-5% of newborns. Many of these testes will descend spontaneously shortly after birth, but ~23% will remain undescended unless surgery is performed. Bilaterally cryptorchid men have a six times greater risk of being infertile when compared with unilaterally cryptorchid men and the general male population. Approximately 10% of infertile men have a history of cryptorchidism and orchidopexy. The main reasons for infertility in men with a history of cryptorchidism treated by orchidopexy are malde…

[Cryptorchidism in adolescence].

INTRODUCTION: An early investigation and management op cryptorchidism are essential for the diagnosis of associated pathologies, because intra-abdominal testis are associated with infertility and neoplasic risk. The authors describe a case of late presentation of cryptorchidism associated with genetic disorder. CASE REPORT: Adolescent male with 17 years old, referred to Adolescent Clinic for short stature and bilateral cryptorchidism. Hypertension, short stature and bilateral cryptorchidism were detected in clinical evaluation. Bone age is coincident with chronological age and testosterone wa…

Cryptorchidism. Current concepts.

Cryptorchidism is the most common genitourinary disorder of childhood. Even though its incidence has changed only slightly over the years, the number of operations for cryptorchid testes has tripled. Better understanding of the natural history of cryptorchidism, as well as changes that occur in testicular histology both in the cryptorchid and the contralateral descended testis very early in life, are the cause. This experience has led us to advocate early orchiopexy as the optimum means of treatment.

[Cryptorchidism: current views].

Cryptorchidism is a pathological condition which affects up to 6% of newborns. Main etiopathogenetic hypotheses are the hormonal and the dysgenetic one. Ultrasonography is useful in locating testis in the inguinal canal, while in the management of intraabdominal testis, laparoscopy is considered the best diagnostic technique and, in many cases, can be coupled with surgical management. Medical treatment with LH-RH or with hCG or, better, combined (LH-RH+hCG) is recommended before the second year. Impairment of fertility is a complication mainly in subjects with a history of bilateral cryptorchi…
