Fatal, penicillin-induced, generalized, postinflammatory elastolysis (cutis laxa).

A 13-year-old boy received penicillin for influenza and otitis media. Within days of taking this medication, he developed recurrent edema of the face and a generalized urticarial eruption which waxed and waned. The salient and unusual features of this person's disease were: 1) A senile appearance of his face with flaccid folds and sagging of the skin. Histopathologic findings were dense dermal inflammatory-cell infiltrates with numerous eosinophils and destruction of elastic fibers. Findings by electron microscopy were fragmentation and clumping of elastic fibers and accumulation of granular m

Congenital cutis laxa associated with exocrine pancreatic insufficiency.

We report a patient who has congenital cutis laxa and exocrine pancreatic insufficiency. Collagen analysis has provided evidence for defective elastin cross linking, possibly related to a mild copper deficiency. Connective tissue anomalies, in the presence of poor growth and/or symptoms of malabsorption should alert the physician to investigate pancreatic function. Adequate replacement of pancreatic enzymes will result in normal growth and development.

A case of congenital cutis laxa (generalized elastolysis).

Cutis laxa (generalized elastolysis) is characterized by sagging folds of loose skin on the face and trunk. Although grouped with hyperelastic skin conditions such as Ehlers-Danlos syndrome, cutis laxa shows an almost total lack of skin elasticity. Because it is a generalized state, it may present as an abnormality in the connective tissue of more than one organ system. The authors report a patient who had end-stage renal failure related to heavy chain disease, a recognized association of cutis laxa. Although the patient's vascular abnormalities were significant, she died of respiratory failur…

Cutis laxa with ultrastructural abnormalities of elastic fiber.

A case of a congenital, autosomal recessive form of generalized cutis laxa is reported. The patient was a 27-month-old boy with generalized flaccid skin and short stature. Radiologic examination revealed that the age of the bones of the wrist was compatible with a chronologic age of only 1 year. Elastic fibers were diminished throughout the dermis, and results of electron microscopic study showed globular and unstained elastin and relatively large amounts of the microfibrillar components of elastic fibers.

Cutis laxa: autosomal dominant inheritance in five generations.

Cutis laxa is described in three cases: a 17-year-old man, his mother and his maternal grandmother. The onset of skin symptoms occurred from puberty to early adulthood. The skin was loose-hanging, wrinkled and without elasticity. X-ray examination showed numerous gastrointestinal diverticulae in the two older patients, and both had been operated on for abdominal hernia and genital prolapse. There were no cardiopulmonary symptoms. Histopathological investigation showed a reduction in the amount of elastic tissue in the dermis, but normally localized and ultrastructurally normal components. The …

Ocular Manifestations in Congenital Cutis Laxa: A Case Series.

PURPOSE: Congenital cutis laxa (CL) syndromes encompass a diverse group of inherited connective tissue disorders characterized by redundant, wrinkled, and inelastic skin, often with systemic involvement. Ocular involvement in CL has sporadically been reported albeit without systematic, comprehensive ocular assessment. The purpose of this study was to evaluate the ocular phenotype in an established cohort of patients with CL with a particular emphasis on corneal involvement. METHODS: A prospective cross-sectional study was conducted at Ghent University Hospital, Belgium, involving 13 genetical…

Neonatal Cutis Laxa and Hypertrichosis Lanuginosa in Sotos Syndrome.

We report a case of transient neonatal cutis laxa and hypertrichosis lanuginosa as an initial presentation in Sotos syndrome. Little is known about skin involvement in Sotos syndrome. Our observation highlights that Sotos syndrome is a rare cause of cutis laxa and suggests that it is a useful neonatal skin clue to the diagnosis of overgrowth syndromes.

[Did the Queen of Pount suffer from cutis laxa?].

A bas-relief of the temple of Deir el-Bahari, Luxor, Egypt, discovered in 1867 by Mariette, shows a queen (Queen of Pount) with wide cutaneous folds on arms, abdomen and legs, hyperlordosis, a steatopygia and abnormally short lower limbs. Various assumptions about the diagnosis (steatopygia, partial achondroplasia, neurofibromatosis) were proposed at the beginning of the XXth century, neither taking into account all morphological abnormalities of the queen. We propose a novel hypothesis taking into account all signs: a Cutis Laxa.

Mutations in PYCR1 cause cutis laxa with progeroid features.

Autosomal recessive cutis laxa (ARCL) describes a group of syndromal disorders that are often associated with a progeroid appearance, lax and wrinkled skin, osteopenia and mental retardation. Homozygosity mapping in several kindreds with ARCL identified a candidate region on chromosome 17q25. By high-throughput sequencing of the entire candidate region, we detected disease-causing mutations in the gene PYCR1. We found that the gene product, an enzyme involved in proline metabolism, localizes to mitochondria. Altered mitochondrial morphology, membrane potential and increased apoptosis rate upon…

Congenital cutis laxa syndrome: type II autosomal recessive inheritance.

Cutis laxa is a term that refers to markedly loose skin that is not hyperelastic. It is regarded as a genetically heterogeneous group of diseases and is presently divided into five types. We report a male patient with type II autosomal recessive disease. The patient was the third child of first-cousin consanguineous, healthy parents. His two siblings died a few hours after birth. One of the siblings also had similar features and wrinkled skin. Our case had markedly loose and wrinkled skin especially over the dorsum of the hands and feet, and on the face and abdomen, dolichocephaly, hypertelori…

Beyond the genome: a rare case report of cutis laxa.

BACKGROUND: Cutis laxa is a heterogeneous group of rare connective tissue pathologies characterized by dermal flaccidity and diminished cutaneous elasticity, often associated with phenotypic features of premature aging. The inherited variants of cutis laxa have various modes of genetic transmission and phenotypic heterogeneity and are categorized into three main categories based on inheritance patterns: autosomal dominant, autosomal recessive, and X-linked recessive forms. Autosomal recessive cutis laxa type 1B (ARCL1B) is associated with the gene EGF-containing fibulin-like extracellular matr…
