In five normal women, the expected chlorpromazine-induced rise in serum prolactin was inhibited by pretreatment with a serotonin antagonist, cyproheptadine. This indicated that the serotoninergic system has an excitatory effect on the release of prolactin. The effect of cyproheptadine was tested on the neurotransmitter-induced release of corticotrophin releasing factor (CRF) from the rat hypothalamus in vitro. It was found that cyproheptadine inhibited both acetylcholine and 5-hydroxytryptamine-induced release of CRF. Cyproheptadine did not interfere with the ACTH releasing ability of CRF. These results suggest that cyproheptadine reduces the high circulating levels of ACTH in some patients suffering from Cushing's disease and Nelson's syndrome by inhibiting CRF secretion.

Cyproheptadine is a drug with direct antimuscarinic, antihistaminic (H1) and antiseronergic (5HT2) effects. In spite of its use as an orexigenic agent in normal or anorectic subjects, little is known about its effect in affective, anxiety or psychotic disorders. We present the case of a 14-year patient beginning with a psychotic syndrome after the sudden withdrawal cyproheptadine, achieving a total control of the symptoms with olanzapine at low dose, besides a good family, school and social adaptation.

A retrospective review of the psychiatric records of nine patients with posttraumatic stress disorder (PTSD) was conducted to determine the efficacy of cyproheptadine in relieving nightmares. The treatment dose was 4 to 12 mg at bedtime. The response varied from complete remission to a decrease in the intensity and frequency of nightmares.

These results in a small group of patients seem quite clear for the benefit of cyproheptadine in patients with BEB, especially where botulinum A injections fail, are too expensive, or are refused. The apparent memory improvement requires further study. A search for more effective serotonin antagonists without the anticholinergic and antihistaminic side effects of cyproheptadine is needed and is in process.
