Cytarabine is a useful agent in the management of acute leukaemias, but is not thought to have activity in solid tumours. We describe the case history of a woman with advanced breast cancer, presenting with superior vena cava obstruction occurring after radiotherapy and chemotherapy, who had a durable remission following inadvertent dosing with cytarabine. A review of the literature reveals that this is the first documented case of a meaningful response to cytarabine in metastatic breast cancer.

The patient is a 36 year old female who presented breast cancer with leptomeningeal involvement. A systematic lumbar puncture was performed and sent to the laboratory for CSF analysis. CSF examination using wet mount preparation showed a large number of round spherules. After discussion with the ordering physician, we learnt that the patient had received intrathecal liposomal cytarabine injection 19 days earlier. Disseminated herpes zoster infection is frequently fatal. Cytarabine (cytosine arabinoside), a drug of variable efficacy, has offered some hope in the treatment of this condition. In this case of disseminated herpes zoster infection with pneumonia, there was remarkable recovery after low-dosage cytarabine infusion. The findings suggest that doses of the drug not exceeding 30 mg/sq m of body surface per 24 hours (30 mg/sq m/24 hr) may well be virucidal but not cytohematotoxic.
