Dantrolene inhibits human erythrocyte glutathione reductase.

Dantrolene, a drug used to treat malignant hyperthermia, inhibits abnormal Ca2+ release from the sarcoplasmic reticulum. Glutathione reductase (Glutathione: NADP+ oxidoreductase, EC 1.8.1.7), a member of the pyridine-nucleotide disulfide oxidoreductase family of flavoenzymes, catalyzes the reduction of glutathione disulfide (GSSG) to reduced form (GSH) in the presence of nicotinamide adenine dinucleotide phosphate (NADPH). In the present study, the in vitro effects of dantrolene on human erythrocyte glutathione reductase were investigated. For this purpose, initially, human erythrocyte glutath…

Clinical effects of intravenously administered dantrolene.

OBJECTIVE: To compare side effects after intravenous administration of dantrolene sodium in subjects susceptible to and those nonsusceptible to malignant hyperthermia (MHS and MHN, respectively). DESIGN: We studied two groups, six patients thought to be MHS and six assumed MHN subjects, and analyzed their responses to intravenously administered dantrolene. MATERIAL AND METHODS: Dantrolene (3 mg/kg) was administered slowly into an antecubital vein, and blood samples were withdrawn from the other arm at 5 and 25 minutes after infusion. Shortly thereafter, all subjects underwent a clinical neur…

Postoperative malignant hyperthermia and dantrolene therapy.

A case of malignant hyperthermia (MH) in a three-year eight-month-old girl is presented. Definite symptoms of MH developed in the awake patient 30 min after termination of anaesthesia which had lasted five hours. This postoperative MH-episode resolved promptly following intravenous administration of dantrolene (2.5 mg X kg-1 initially, followed by 5 mg X kg-1 over 12 hours). Results of serial serum samples revealed a steady increase in creatine kinase (CK) concentration with the highest value being observed at the second day, despite dantrolene therapy. The unusual occurrence of MH in the post…

Current clinical application of dantrolene sodium.

Dantrolene sodium (DS) was first introduced as an oral antispasmodic drug. However, in 1975, DS was demonstrated to be effective for managing malignant hyperthermia (MH) and was adopted as the primary therapeutic drug after intravenous administration. However, it is difficult to administer DS intravenously to manage MH. MH is life-threatening, pharmacogenomically related, and induced by depolarizing neuromuscular blocking agents or inhalational anesthetics. All anesthesiologists should know the pharmacology of DS. DS suppresses Ca2+ release from ryanodine receptors (RyRs). RyRs are expressed i…

Dantrolene reduces CaMKIIδC-mediated atrial arrhythmias.

AIMS: In atrial fibrillation (AF), an increased diastolic Ca2+ leak from the sarcoplasmic reticulum (SR) mediated by calcium/calmodulin-dependent-protein-kinaseIIδC (CaMKII) can serve as a substrate for arrhythmia induction and persistence. Dantrolene has been shown to stabilize the cardiac ryanodine-receptor. This study investigated the effects of dantrolene on arrhythmogenesis in human and mouse atria with enhanced CaMKII activity. METHODS AND RESULTS: Human atrial cardiomyocytes (CMs) were isolated from patients with AF. To investigate CaMKII-mediated arrhythmogenesis, atrial CMs from mice…

[Dantrolene-induced pleurisy: a case report].

We report the case of a patient who developed dantrolene-induced pleurisy. Dantrolene (Dantrium) is a muscle relaxing agent used for the treatment of spastic neurological manifestations which has known liver toxicity. Lung toxicity is rarely reported. Six cases of dantrolene-induced pleurisy occurring after chronic administration (> 60 days) have been described in the literature. The pleurisy is associated with pleural and peripheral eosinophilia. Spontaneous regression a few days after withdrawal and radiological cure a few months later is the rule. The precise mechanism of this drug-induced …

Is dantrolene effective in heat stroke patients?

Dantrolene (2.45 mg/kg body weight, range 2 to 4) was administered iv in eight heat stroke (HS) patients and compared with a control group of 12 patients of similar age, weight, and temperature range (41.9 degrees to 44 degrees C). Body surface cooling was conducted in air conditioned rooms at temperatures of 18 degrees to 23 degrees C. Mean cooling time in the dantrolene group was 49.7 +/- 4.4 (SEM) min, whereas cooling time in the control group was 69.2 +/- 4.8 min. The decrease in temperature was significantly greater in the dantrolene group, for whom the cooling time was decreased by about…

[Cardiac failure caused by an overdose of dantrolene].

A case is reported of cardiac failure with repeated pulmonary oedema in a female patient suffering from renal failure and having been given incremental doses of dantrolene sodium. On withdrawal of the drug, all symptoms disappeared. This muscle relaxant was considered not to have any depressive myocardial side-effects. However, recent studies on isolated myocardial fibres showed the existence of a constant dose-dependent negative inotropic effect of dantrolene sodium, only noticeable with high concentrations of the drug.

Efficacy of dantrolene sodium in the treatment of spasticity.

Dantrolene sodium was effective in controlling spasticity and improving function in 11 of 13 patients taking part in a 14-week double-blind crossover study. Clonus and lower-extremity tendon jerks were reduced in almost all patients (10 and 11 respectively) during dantrolene sodium therapy at 200 to 400 mg/day. Activities of daily living showed marked improvement in 3 cases and moderate improvement in 8. All 13 patients reported decreases in the frequency of intermittent painful spasm. Side effects were minimal and consisted mainly of an initial 3- to 4-day period of weakness and lightheadedne…
