[Hyperglycemia of the dehydrated infant].

Hyperglycemia and glucosuria are frequently seen during the acute phase of dehydration secondary to gastroenteritis. In this paper, 33 patients in hypovolemic shock due to diarrhea were studied. Serum glucose resulted above 140 mg/100 ml. in 14 patients (41%); it was below 27 mg/100 ml. in 2 more patients (5.8%). Despite high glucose concentration, only 4 out of the 14 hyperglycemic patients had serum insulin concentration above that observed in normal fasting children. Plasma cortisol was significantly higher in the group with hyperglycemia (54 +/- 24 micrograms/100 ml.) in comparison with th…

Managing the dehydrated child.

Hypotonic fluid losses lead to hypertonic dehydration. Because the extracellular fluid compartment has the ability to draw fluid from the intracellular compartment, intravascular volume is better maintained in hypertonic dehydration than in isotonic or hypotonic dehydration. When the amount of weight loss is known, the fluid deficit can be easily calculated; otherwise, a clinical assessment of the percentage of dehydration is required.

Association Between Dehydration and Falls.

OBJECTIVE: To determine whether there is an association between dehydration and falls in adults 65 years and older. PATIENTS AND METHODS: We used University of Wisconsin Health electronic health records from October 1, 2011 to September 30, 2015 to conduct a retrospective cohort study of Midwestern patients 65 years and older and examined the association between dehydration at baseline (defined as serum urea nitrogen to creatinine ratio > 20, sodium level > 145 mg/dL, urine specific gravity > 1.030, or serum osmolality > 295 mOsm/kg) and falls within 3 years after baseline while accounting fo…

Transtympanic electrocochleography during glycerol dehydration.

Thirteen patients with Meniere's disorder were tested using transtympanic electrocochleography during glycerol dehydration. The most common finding was a decrease in the negative summating potential and this appeared to be a more sensitive indicator of changes occurring in the cochlea than pure tone audiometry and speech discrimination. A pathophysiological explanation for this observation is offered and the possibility of using this decrease in the negative summating potential as a pointer to the prognosis of endolymphatic sac surgery is discussed.

Extreme Hypernatremia due to Dehydration.

Hypernatremia is defined as having a serum sodium concentration exceeding 145 mmol/L (normal range is 136 - 145 mmol/L). When the serum sodium level surpasses 160 mmol/L and 190 mmol/L, it is classified as severe and extreme hypernatremia, respectively. Extreme hypernatremia is an uncommon occurrence and is associated with significant risks of mortality and morbidity. The most frequently reported causes of extreme hypernatremia are accidental or intentional salt ingestion and diabetes insipidus, while dehydration due to poor water intake as a trigger for extreme hypernatremia is rarely documen…

Malnutrition and dehydration after stroke.

By identifying malnutrition and dehydration in patients who have had a stroke, nurses can intervene to prevent significant complications developing and so improve patient outcomes. Poor intake of fluid and food may result from difficulties in swallowing, as well as other physical and functional problems that occur as a result of a stroke. The aim of this article is to encourage nurses to identify the frequency and causes of malnutrition and dehydration, to consider the complications this can cause and to be aware of optimum feeding strategies for stroke patients.
