A quantitative assessment of pre-dental antibiotic prophylaxis for patients with mitral-valve prolapse.

Because antibiotic prophylaxis for dental procedures in patients with mitral-valve prolapse (MVP) has been controversial, we performed a decision analysis to assess the costs and effects of the oral and parenteral penicillin regimens currently recommended for patients at high risk for bacterial endocarditis. The analysis suggests that there is a very small risk of post-dental endocarditis in MVP (4.1 cases per 10(6) procedures) which is outweighed by a greater risk of fatal reactions to parenteral penicillin (15 deaths per 10(6) courses). Parenteral penicillin prophylaxis therefore causes a ne

Antibiotic Prophylaxis for Prosthetic Joint Patients Undergoing Invasive Dental Procedures: Time for a Rethink?

BACKGROUND: In the United States, it has been common practice to recommend that dentists provide antibiotic prophylaxis (AP) before invasive dental procedures (IDPs) to prevent late periprosthetic joint infections (LPJIs) in patients who have prosthetic arthroplasties despite lack of evidence for a causal relationship between IDP and LPJI and a lack of evidence for AP efficacy. METHODS: A recent study quantified the IDP incidence over the 15-month period prior to LPJI hospital admissions in the United Kingdom for which dental records were available. A case-crossover analysis compared IDP inci

Assessing Variances in Dentist's Interpretations of Antibiotic Prophylaxis Guidelines: A Survey of Dental School Faculty.

Purpose: A recent review by the American Heart Association (AHA) noted a decrease in the prescription of antibiotic prophylaxis (AP) for infective endocarditis (IE) following the release of their guidelines in 2007. However, studies indicate dentists may still face challenges in identifying which procedures require AP and which procedures are considered to involve gingival manipulation (GM) requiring the need for AP. Methods: A sample of dental school faculty was surveyed to assess their likelihood of prescribing AP for various dental procedures when treating individuals at high risk for IE an

Elimination of bacteraemia after dental extraction: comparison of erythromycin and clindamycin for prophylaxis of infective endocarditis.

Erythromycin and clindamycin are currently recommended for antibiotic prophylaxis of infective endocarditis in predisposed patients allergic to penicillin undergoing oral invasive procedures. Thirty-eight healthy patients were randomized to receive either erythromycin (1 g) or clindamycin (0.6 g) orally 1.5 h prior to dental extraction. Blood samples for microbiological investigation were collected before, during and 10 min after surgery and were processed by lysis filtration under anaerobic conditions. The incidence of bacteraemia with viridans streptococci was 79% in the erythromycin group a

Antibiotic Prophylaxis Is Not Necessary for Invasive Dental Procedures in Existing Total Knee Arthroplasty Implants.

Antibiotic prophylaxis before an invasive dental procedure (IDP) for patients with preexisting knee implants has been suggested, but its use has yet to reach consensus. Therefore, we sought to examine antibiotic prophylaxis before an IDP for patients who had undergone a total knee arthroplasty (TKA). We specifically assessed 90-day to 1-year (1) periprosthetic joint infection (PJI) and (2) revision. We queried a national, all-payer database for patients undergoing primary TKA between 2010 and 2020 (n=1,952,917). We identified IDP, as defined by any procedure that involves gingival manipulation

Factors associated with use of antibiotic prophylaxis following dental procedures in total joint replacement recipients.

BACKGROUND: Total joint replacement recipients (TJR) are at risk for prosthetic joint infection (PJI), particularly those with comorbidities such as diabetes mellitus (DM) and rheumatoid arthritis (RA). METHODS: We mailed surveys to 1078 subjects undergoing primary TJR between 2011 and 2016 at an academic center. The survey asked about medical, dental, and orthopedic history and use of antibiotics prior to dental appointments. We generated adjusted relative risks (aRR) for using antibiotic prophylaxis less than always using Poisson regression, adjusting for demographic, clinical, and behavio

Prosthetic Joint Infection After Dental Work: Is the Correct Prophylaxis Being Prescribed? A Systematic Review.

BACKGROUND: Prosthetic joint infection (PJI) of total hip (THA) or total knee arthroplasty (TKA) after dental procedures is uncommon, and antibiotic prophylaxis remains controversial. For high-risk patients, the American Academy of Orthopedic Surgeons recommends amoxicillin prophylaxis. However, no systematic review of the literature of PJIs associated with dental procedures explores if amoxicillin is suitable for the reported organisms. METHODS: A librarian-assisted search of the major databases (PubMed, Medline, Embase, Scopus) identified 954 articles. Only case reports, case series, and re

Interventions for replacing missing teeth: antibiotic prophylaxis for dental implant placement without bone augmentation procedures.

RATIONALE: Some dental implant failures may result from bacterial contamination at implant insertion, and biomaterial-related infections are difficult to treat. Antibiotic prophylaxis is generally indicated for patients at risk of infective endocarditis or with reduced host response, surgery at infected sites, extensive or prolonged procedures, or implantation of large foreign materials. Various prophylactic regimens, including short-term protocols, have been proposed to reduce post-implant infections. However, concerns about adverse events and antibiotic resistance make prophylactic antibioti

Antibiotic Prophylaxis for Dental Procedures Following Total Joint Arthroplasty: A Systematic Review and Meta-Analysis.

BACKGROUND: Periprosthetic joint infection (PJI) is a serious complication following total joint arthroplasty (TJA). Hematogenous spread, including from dental procedures, is a proposed mechanism for PJI. While prophylactic antibiotics (PAs) have traditionally been recommended before dental procedures in patients with joint prostheses, recent guidelines question their effectiveness. This study aimed to evaluate the association between dental antibiotic prophylaxis and PJI risk through a systematic review and meta-analysis. METHODS: A systematic literature search was conducted using PubMed, Em

Acceptance among and impact on dental practitioners and patients of American Heart Association recommendations for antibiotic prophylaxis.

BACKGROUND: The 2007 American Heart Association (AHA) guidelines for the prevention of infective endocarditis (IE) called for a major reduction in the number of patients recommended for antibiotic prophylaxis (AP) and redefined the dental procedures considered to put these patients at risk of acquiring the infection. The purpose of the authors' study was to determine the acceptance of these changes among and the impact of the changes on dentists and their patients. METHODS: The authors sent a survey to a random sample of 5,500 dentists in the United States. RESULTS: Ninety-five percent of th

Dental Students' Knowledge and Opinion on Optimizing Antibiotic Prophylaxis Guidelines: A Cross-Sectional Survey in India.

BACKGROUND: Antibiotic prophylaxis (AP) prescription is recommended to aid the body's defense in clearing tissues of microbial pathogens in patients at high risk of adverse outcomes, bacteremia, or infection undergoing invasive oral and dental procedures. A variety of influences impact dentists' decisions regarding AP prescribing. There is a paucity of data on diverse prescribing patterns of antibiotics as prophylaxis among dental students and professionals. Despite accumulating antimicrobial resistance (AMR), knowledge gaps regarding optimal prophylaxis practices persist among dental students
