Photoallergic Contact Dermatitis: No Fun in the Sun.

Photoallergic contact dermatitis (PACD) is a form of allergic contact dermatitis that occurs due to the interaction between a topically applied chemical and exposure to UV radiation. It can be difficult to identify and requires photopatch testing (PPT) for definitive diagnosis. In this article, we provide an overview of PACD, including clinical features, the most common photoallergens, and why cases may go undiagnosed.

Allergic and photoallergic contact dermatitis to chlorpromazine.

Chlorpromazine is known to produce both systemic phototoxic and photoallergic reactions. However, it may also cause photoallergic contact dermatitis and, albeit exceptionally, allergic contact dermatitis (ACD). We present a series of photoallergic contact dermatitis and ACD to chlorpromazine diagnosed at a tertiary centre cutaneous allergy unit between 1980 and 2019.

Photoallergic contact dermatitis from benzydamine presenting mainly as lip dermatitis.

BACKGROUND: Benzydamine, a non-steroidal anti-inflammatory drug (NSAID) in use for more than four decades, has been reported to cause photosensitivity. OBJECTIVES: To study the results of photopatch testing to benzydamine and the clinical features of the dermatitis during a 3-year period (2006-2008). PATIENTS AND METHODS: During this period, 74 patients with photodermatoses were photopatch tested with an extended baseline series of allergens including benzydamine and in suspicious cases, with drugs that contain it. Test sites were irradiated on D2 with 5 J/cm(2) and readings were performed o

Photoallergic contact dermatitis due to mineral oil.

A 39-year-old metal polishing mechanic had had an acute-on-chronic eczematous eruption on sun-exposed skin for 2 years. It improved in winter. He had been using an insoluble oil, CRT 20, as a cutting oil for 20 years. The cutting oil itself and mineral oil, which was one of its ingredients, showed positive reactions on photopatch testing. The difference action spectrum with or without mineral oil application, assessed at 48 h, ranged from 300 to 350 nm, with a peak at 320 nm. This is the 1st report of photoallergic contact dermatitis due to mineral oil.

Photoallergic and allergic contact dermatitis from dibenzoylmethanes.

Dibenzoylmethanes are widely used in Europe in UVA-absorbing sunscreens. We found that 2 of these, 4-isopropyl-dibenzoylmethane and 4-tert.butyl-4'-methoxy-dibenzoylmethane caused allergic or photoallergic contact dermatitis. This is the first report on allergic and photoallergic reactions caused by dibenzoylmethanes.

Experimental photoallergic contact dermatitis: a mouse model.

We have induced photoallergic contact dermatitis in mice to 3,3',4',5 tetrachlorosalicylanilide (TCSA), chlorpromazine and 6-methylcoumarin. These compounds are known to produce photoallergic contact dermatitis in humans. The photoallergic contact dermatitis reaction in the mouse is immunologically specific viz. mice photosensitized to TCSA react, by photochallenge, to that compound and not to chlorpromazine, and conversely. The reaction requires UVA at both sensitization and challenge. It appears to be T-cell mediated in that it can be passively transferred to syngeneic mice by lymph node cel

Facial Sweet syndrome mimicking photoallergic contact dermatitis.

Sweet syndrome (acute febrile neutrophilic dermatosis) usually presents with painful erythematous plaques, malaise, and fever. Histologically skin infiltration with neutrophils is characteristic. The etiology of the disease is unknown. Sweet syndrome can be subdivided into the classical form, which is usually idiopathic and is associated with some kind of inflammation, the paraneoplastic form and the drug-induced one. We present the unusual case of an 83-year-old woman who appeared to have facial photoallergic contact dermatitis but turned out to have facial Sweet syndrome.

Contact urticaria, allergic contact dermatitis, and photoallergic contact dermatitis from oxybenzone.

There is little literature regarding conventional patch tests and photopatch tests to oxybenzone resulting in both immediate- and delayed-type hypersensitivity reactions. A patient was patch-tested and photopatch-tested to various sunscreen chemicals. Both immediate- and delayed-type hypersensitivity reactions were observed with oxybenzone. The positive patch tests were also photoaccentuated. Oxybenzone, a common sunscreen allergen, can result in both contact urticaria and delayed-type hypersensitivity on both conventional patch testing and photopatch testing. Occupational photoallergic contact dermatitis to thiourea in a firefighter.

Thioureas are a relatively uncommon cause of allergic contact dermatitis and a rare cause of photoallergic contact dermatitis. They are primarily used as vulcanization accelerators in the production of neoprene, and common sources of exposure include gloves, shoe insoles, wet suits, and orthopedic braces. We report a multifactorial case of severe episodic photoallergic contact dermatitis to thiourea in a 29-year-old firefighter. Multiple sources of neoprene exposure were identified, including a firefighting mask, continuous positive airway pressure strap, and hockey helmet.

Immunological and molecular mechanisms of photoallergic contact dermatitis.

Photocontact dermatitis is one of the major occupational skin diseases and an undesirable adverse effect produced by chemicals and drugs in our environment. This dermatitis is caused by skin application of photosensitive agents plus ultraviolet light irradiation. There exist both primary irritant and allergic mechanisms in this dermatosis. While the former is induced by the phototoxic reactions, the latter allergic type is mediated by Langerhans cells, T cells, various cytokines and chemokines, and thus occurs via a well-orchestrated and well-elucidated immunological mechanism. The photoallerg

[Photoallergic contact dermatitis by benzophenone containing sunscreening preparations].

The case of a 42-year-old woman with sunlight-provoked eczematous eruptions recurrent over several years is reported. Photopatch-testing with cosmetic products and sunscreen preparations used by the patient as well as the active ingredient of the sunscreen formulations revealed photosensitivity to the UV-blocking agent 2-hydroxy-4-methoxybenzophenone. The active wavelengths were shown to be in the UV-A range.
