Secondary root and distal thoraco-abdominal endovascular aortic re-interventions can be challenging after frozen elephant trunk. We obtained from the TERUMO Aortic custom-made platform a Thoraflex graft with a modified disposition of the arch branches that facilitate secondary proximal and distal reinterventions. Here we describe the graft and our first implant.

We describe a 67-year-old man who developed a dissection of the right common carotid arterial wall with intussusception two years following endarterectomy for atherosclerosis. We suggest that the previous surgical procedure resulted in a thinned and weakened media. With a disruption of the intima, a dissection into the inner third of the media formed, and blood in the false lumen buckled the intima into the lumen of the blood vessel. Hemodynamic forces then probably played a role in the propagation of the dissection and the formation of an intussusception. Complete occlusion by thrombus follow

OBJECTIVES: Our goal was to present our experience with a hybrid approach to the frozen elephant trunk (FET) technique for the treatment of patients with chronic type B aortic dissection.
METHODS: Between January 2013 and July 2019, 86 patients underwent the FET procedure at our centre. In 20 patients, the indication was chronic type B aortic dissection with a concomitant proximal aortic lesion. We evaluated the sites of proximal and distal entries, luminal communication and originating visceral branches in the computed tomography scan data. Primary end points were hospital deaths, complicati

OBJECTIVES: Conventional head and neck practice procedures fail to address several clinical issues. Anatomical structures were removed from the surface layers in that order, and the dissection was shifted to the deeper layer. This approach makes it difficult for medical and dental students to understand the relationships among various anatomical structures. Furthermore, it is difficult to determine the risk on the mandibular nerves and blood vessels during conventional mandible removal procedures. This study aims to develop an anatomical technique that maintains the skin, superficial facial mu

Due to the pathology of acute type A aortic dissection (AAD), including fragile dissected aortic wall and visceral malperfusions, surgical procedures are still in development. This commentary is a review of a report by Bin and Yang et al. published in the Journal of Cardiac Surgery that reported similar effectiveness of the open triple-branched stent graft for acute type AAD and total arch replacement with the frozen elephant trunk technique.

Despite advances in the development of endovascular techniques of revascularization of renal arteries, in certain clinical cases still remains the need to perform extra-anatomic renal bypass grafting. To such instances belong complicated atherosclerotic aortic lesions, technical difficulties occurring during open revascularization of the aorta and its branches, as well as aneurysms of the juxtarenal portion of the abdominal aorta. Presented herein is a clinical case concerning a patient subjected to non-standard restoration of blood flow in the right renal artery after thromboendarterectomy fr

PHACE syndrome is a rare disorder with posterior fossa brain malformations, segmental infantile haemangiomas, arterial anomalies, cardiac defects and eye anomalies. Cerebral and cervical arterial abnormalities occur commonly in these patients, predisposing subjects with PHACE syndrome to neurovascular complications including migraine-like headaches, moyamoya vasculopathy, arterial dissection and arterial ischaemia stroke. We leveraged institutional MRI protocols developed for adult neurovascular disease to better elucidate the pathogenesis of the arterial alternations observed in PHACE. Using

The aim of this study was to evaluate the arterial and venous blood flow in women who underwent upper limb axillary dissection surgery for the treatment of breast cancer. Sixty women were divided into two groups: group 1 (G1)--30 women who underwent breast surgery with axillary dissection level II or III (55.6 ± 8.6 years); group 2 (G2)--control, 30 women with no breast cancer (57.4 ± 7.0 years). Blood flow profile was evaluated by a continuous wave ultrasound Doppler device (Nicolet Vascular Versalab SE) with an 8 MHz probe. Total mesorectal excision (TME) is a precise dissection of the rectum and all pararectal lymph nodes within an oncologic package: the mesorectal envelope. This article is a brief description of the technical aspects of the dissection, illustrated by cadaver dissection. Here the TME dissection is organized into 6 steps to facilitate learning: (1) left retroperitoneum; (2) superior rectal and inferior mesenteric vessels; (3) upper mesorectum; (4/5) right and left mid-mesorectum; and (6) distal mesorectum and anorectal junction. The relationship of the autonomic nerves, blood vessels and adjacent

BACKGROUND: To date, it has been assumed that acute seizures which arise in the context of sudden, spontaneous, atraumatic, acute, arterial dissections (SAAADs) are downstream consequences of the dissections driven by syncope or focal brain lesions (FBLs). As this subject has not been formally investigated, likely due to its rarity, we reviewed published case reports (CRs) to examine the veracity of this assumption.
METHODS: We included CR describing patients diagnosed with both acute seizures and arterial dissections in order to ascertain the temporal sequence between acute seizures and typi

Despite the fact that recent decades have witnessed significant advances in treatment of patients with DeBakey type I acute aortic dissection, it still remains difficult to restore the aortic root when the dissection extends to the Valsalva's sinuses. Thinned aortic walls are vulnerable to traumatization on applying a vascular suture. We used in patients with this pathology the Florida sleeve technique in order to reinforce the weakened aortic root. After mobilization of the aortic root and coronary arteries, the transplant 'wraps' the sinuses from the outside, like the neoadventitia, in order

PERFORMANCE: Spontaneous dissections of the cerebral arteries are among the leading causes of stroke in young adults. They result from hemorrhage into the outer layers of the arterial wall, which can lead to stenosis or even complete vessel occlusion. Clinical presentations vary, ranging from localized pain to cerebral ischemic complications. Early recognition of dissection warning signs and diagnosis through imaging is crucial due to the high risk of recurrent transient ischemic attack (TIA) or stroke within the first two weeks following a dissection, necessitating immediate therapeutic or pr

A male patient in his 60s developed acute Stanford type B aortic dissection with malperfusion of the lower limbs in early June. Emergency surgery was performed. The surgery aimed to close the entry in the distal arch and involved a debranched thoracic endovascular aortic repair (TEVAR). Postoperatively, malperfusion improved, but a type 1a endoleak persisted, and the distal arch aneurysm enlarged. Therefore, an additional TEVAR was performed in early October. Compared to the previous procedure, this TEVAR was placed just after the brachiocephalic artery in the central side. One week after surg

INTRODUCTION: Endovascular treatment of type B aortic dissections (TBAD) has currently acquired a primary therapeutic role when anatomically feasible. The main issue with thoracic endovascular aortic repair (TEVAR) for aortic dissection is the actual nature of the aortic wall, which is structurally compromised and more fragile. Indeed, a wealth of data have shown that TEVAR for TBAD will lead, in a substantial proportion of cases, to a device-related adverse event named distal stent-graft induced new entry (dSINE).
AREAS COVERED: Currently available aortic stent-grafts have not been specifica

BACKGROUND: Frozen elephant trunk has in recent times become a mainstay for total arch replacement in aortovascular surgery and is indicated to treat a spectrum of complex aortic pathologies. However, despite associated excellent postoperative results it is incredibly important to recognize potential adverse complications such as negative aortic remodeling, endoleak, and distal stent-graft induced new entry so that outcomes can be further improved.
AIM OF THE STUDY: Below we provide commentary on a recent article in the Journal of Cardiac Surgery discussing the topic.
CONCLUSIONS: Despite th

BACKGROUND: The inferior mesenteric artery (IMA) is usually divided during the resection of sigmoid colon and rectal cancers. However, this sometimes results in a vein (SRA\V) insufficient blood supply to the anastomosis, leading to anastomotic leakage. We summarized the experience of laparoscopy surgery approach to perform the D3 lymph node dissection with preserving the superior rectal artery and vein.
METHODS: Our method involves peeling off the vascular sheath from the inferior mesenteric vessel to the superior rectal vessel and dissection of the lymph node around the IMA together with th
