Spastic dysphonia.

130 cases of spastic dysphonia are reviewed with special emphasis on the etiology of the disorder. An analysis of the circumstances surrounding the onset of the disease points strongly to a psychogenic origin of the disease in, at least, the vast majority of the cases.

Differential diagnosis of muscle tension dysphonia and spasmodic dysphonia.

PURPOSE OF REVIEW: Muscle tension dysphonia (MTD) can mimic the voice features of adductor spasmodic dysphonia (ADSD) leading to diagnostic confusion. Researchers have begun to compare characteristics of MTD and ADSD to determine whether there are markers which reliably distinguish the two disorders and lead to improved differential diagnosis. RECENT FINDINGS: Differences between MTD and ADSD have been identified during fiberoptic laryngoscopy, phonatory airflow measurement, acoustic analysis, and variable sign expression based upon phonatory task. In general, evidence of task-dependent sign …

Hereditary whispering dysphonia.

An Australian family group is described where at least twenty members have inherited torsion dystonia and two siblings with an affected mother have similar clinical manifestations, but have also the biochemical and pathological changes found in Wilson's disease. Whispering dysphonia was the commonest presenting symptom, and a diagnosis of hysteria was invariably made if the family history was not known. This group emphasises the enormously varied ways in which torsion dystonia may be manifested in one family, and raises the possibility of a disturbance in copper transport in diseases of the ba…

Task specificity in adductor spasmodic dysphonia versus muscle tension dysphonia.

OBJECTIVES: Adductor spasmodic dysphonia (ADSD) has been characterized as a "task specific" laryngeal dystonia, meaning that the severity of dysphonia varies depending on the demands of the vocal task. Voice produced in connected speech as compared with sustained vowels is said to provoke more frequent and severe laryngeal spasms. This study examined the diagnostic value of "task specificity" as a marker of ADSD and its potential to differentiate ADSD from muscle tension dysphonia (MTD), a functional voice disorder that can often masquerade as ADSD. STUDY DESIGN: Case-control study. METHODS:…

Dysphonia in infants.

The definition of infant dysphonia is difficult because the physiological phonation itself may be extremely variable in newborns. In a wider sense, all voice utterings can be interpreted as dysphonia which deviate continuously in any of the parameters (timbre, pitch, intensity, or noise) from the normal. We differentiated 20 types of such pathological phonation. The most characteristic are: the hoarse (caused by inflammation or tumor of the vocal cords); the hollow (indicative of tracheal stenosis); the shrill (suggesting a CNS damage); the bleating (pathognostic of Down syndrome); the faint (…

[Dysphonia in children].

Numerous cases of early dysphonia in children are due to congenital lesions of the vocal cord. Others, developing later, appear within a context of maltreatment of the vocal cords, and become part of the child's psychological and psychomotor developmental disorder. Hearing and seeing dysphonia are the two parts of our diagnostic stage, assisted by the use of optic fibres and audiovisual methods. Treatment must be global, putting dysphonia back into its proper context, leaving a substantial part to vocal re-education, as laryngeal microsurgery only comes into play when the result is inadequate,…

Phonatory air flow characteristics of adductor spasmodic dysphonia and muscle tension dysphonia.

The purpose of this study was to determine if phonatory air flow characteristics differed among women with adductor spasmodic dysphonia (AdSD), muscle tension dysphonia (MTD), and normal phonation. Phonatory air flow signals were gathered during [pa] syllable repetitions. Mean phonatory air flow, coefficients of variation, and the presence of large air flow perturbations (75 ml/s or more) were examined for the three groups of speakers. There was no significant difference in mean phonatory air flow across groups, and very large intersubject variation in mean phonatory air flow occurred for both…
