Advances in Transcatheter Electrosurgery for Treating Valvular Heart Disease.

Delivery of electrosurgery energy through catheters and guidewires enables interventionists to 'cut' through obstructive intravascular lesions or across cardiac chambers. A novel application of transcatheter electrosurgery is to make controlled lacerations in heart valve leaflets. This review describes three applications of transcatheter electrosurgery of aortic and mitral valve leaflets to enable transcatheter heart valve implantation. Intentional laceration of the anterior mitral leaflet to prevent left ventricular outflow obstruction splits and splays the anterior mitral valve and enables t…

Removal of lymphangiectasis using high-frequency radio wave electrosurgery.

PURPOSE: To describe a novel technique using high-frequency radio wave electrosurgery (Ellman Surgitron Dual Frequency RF; Ellman International, Inc) for the management of lymphangiectasis. METHODS: Ablations were performed at the lowest power setting of 1 (of 100) in the cutting mode, producing the least amount of lateral heat. To prevent conjunctival hemorrhage, ablation was initiated immediately before introducing the tip of a needle electrode into the subconjunctival tissue under the target area. After the tip of the electrode reached the target area, ablation was maintained for 1 to 2 se…

Understanding submucosal electrosurgery for the treatment of nasal turbinate enlargement.

The surgical management of inferior turbinate enlargement is controversial. Submucosal electrosurgical techniques for turbinate reduction include conventional diathermy, radiofrequency tissue reduction and coblation. All electrosurgical techniques use radiofrequency electricity to damage turbinate tissue but differ in the control and delivery of energy. This review will examine the history of submucosal electrosurgery and clarify the various techniques. This review will also examine the evidence for the efficacy and safety of electrosurgery for the treatment of nasal turbinate enlargement, and…

Comparison of carbon dioxide laser and electrosurgery in the treatment of rhinophyma.

Cosmetic results of rhinophymas treated with carbon dioxide laser or electrosurgery were compared in six patients 2-5 years after surgery. Although the results were comparably good the use of the carbon dioxide laser proved to be more time-consuming and less convenient. Scar formation was observed in both patient groups equally and was related to the depth of tissue removal, independent of the instrument used.

Electrosurgery-induced ventricular fibrillation during pacemaker replacement--a unique mechanism.

Arrhythmias and pacemaker malfunction are known to occur from the use of an electrosurgical device. The present case report describes a patient with sick sinus syndrome who experienced ventricular fibrillation while undergoing surgery. During replacement of his non-functioning cardiac pacemaker under general anesthesia, electrosurgery was used to ensure hemostasis. Electric current may have stimulated myocardial leads present in the surrounding tissue, leading to ventricular fibrillation. The patient was resuscitated from the episode without any residual sequelae. Microshock and possible mecha…

Non-parametric frequency response function tissue modeling in bipolar electrosurgery.

High-frequency radio energy is applied to tissue therapeutically in a number of different medical applications. The ability to model the effects of RF energy on the collagen, elastin, and liquid content of the target tissue would allow for the refinement of the control of the energy in order to improve outcomes and reduce negative side-effects. In this paper, we study the time-varying impedance spectra of the circuit. It is expected that the collagen/elastin ratio does not change over time such that the time-varying impedance is a function of the liquid content. We apply a non-parametric model

Electrosurgery using insulated needles: treatment of axillary bromhidrosis and hyperhidrosis.

This paper deals with operative techniques and clinical aspects of electrically treating axillary bromhidrosis and hyperhidrosis using insulated needles. Most patients were treated 5 or 6 times using 2 types of needles, one for electroelimination of sweat glands and the other mainly for epilation. The results for most patients were satisfactory, with scarring occurring only in the early developmental stages of the technique.

Acute and subacute effects of the ultrasonic blade and electrosurgery on nerve physiology.

Ultrasonic blades have been shown to cause less acute electrophysiological damage when applied near nerves than monopolar electrosurgery (ES). This study was performed to determine whether the acute nerve damage observed for ES, as well as the relative lack of damage observed for ultrasonic dissection, extends through a subacute timeframe. Muscle incisions were made in rat with the Harmonic(®) Blade (HB) and ES at a distance of 2 mm from the sciatic nerve. Sham surgery was also performed which consisted of similar exposure of the sciatic nerve without use of an energized device. Electrophysiol…
