Caloric function after endolymphatic sac surgery.

OBJECTIVE: To evaluate the effect of endolymphatic sac surgery on vestibular functions using caloric testing on electronystagmography (ENG). STUDY DESIGN: Retrospective chart review. METHODS: The medical records of 21 adult patients with unilateral Ménière's disease who underwent endolymphatic sac surgery between 1998 and 2004 were reviewed. With use of ENG, the absolute value of the caloric response of the operated ear (i.e., the cool + warm irrigation response) and the degree of reduced vestibular response (RVR) rates as indicators of caloric functions were compared before and after surger

Cholesterol granuloma surrounding the endolymphatic sac.

We report a unique case of cholesterol granuloma (CG) surrounding the endolymphatic sac (ES). A 49-year-old man presented with the left side of sensorineural hearing loss, tinnitus, and vertigo. Magnetic resonance and computed tomography imaging revealed a CG surrounding the left ES. The patient initially underwent left transmastoid surgical resection of the tumor. At the time of surgery, brown fluid was aspirated from the tumor, but no other tumors were found. Histopathological examination revealed that the tumor contained cholesterol crystals, confirming the diagnosis of CG. At his 12-month

Hearing results from endolymphatic sac surgery.

This report examines the effect of the endolymphatic shunt on hearing. The study group was drawn from 101 shunt operations for intractable vertigo between 1983 and 1987. Thirty ears met criteria for diagnosis, level of preoperative hearing impairment, and length of follow-up. The control group consisted of 30 ears with symptoms severe enough to prompt recommendations for shunt surgery, but the patients either opted against surgery or improved. The results were analyzed using the 1985 AAO-HNS reporting criteria. When looking at worst postoperative (or worst scores after 2 years of follow-up in

[Endolymphatic sac surgery for Menière's disease].

A series of fifteen patients with intractable vertigo from Meniere's disease were treated by endolymphatic sac surgery. Thirteen of the fifteen patients were followed up for more than 6 months with an average of 16 months. Eight of the 13 patients relieved from vertigo and 3 improved. While in half of the patients the tinnitus and pressure feeling were diminished or vanished, only one patient demonstrated a 13 dB hearing loss. The surgical indication for Ménière's disease, the location of the endolymphatic sac and the evaluation of the endolymphatic shunt operation were discussed.

Valved implants in endolymphatic sac surgery.

This study demonstrates the superiority of a valved implant (Denver-Arenberg valve) over simple sac decompression surgery for Meniere's disease. The valved implant has the advantage of better hearing results. Effectiveness in the relief of vertigo is equal for the two types of procedures. The phenomenon of enhanced high-frequency hearing gain has been observed only with valved implants. Rather spectacular hearing gains in a minority of patients have occurred primarily with the valved implant. Although the numbers are too small to draw any firm conclusions, it would seem that the valved implant

Endolymphatic sac surgery for Menière's disease.

An endolymphatic-mastoid Silastic shunt procedure was performed in 122 cases of Menière's disease having a mean follow-up period of three years. In accordance with American Academy of Ophthalmology and Otolaryngology 1972 criteria, there were 43% class A, 20% class B, 21% class C, and 17% class D results. Analysis of 35 recent cases having a mean follow-up period of nine months revealed 57% class A, 25% class B, 9% class C, and 9% class D results. Sixteen percent of the patients who experienced classes A, B or C results complained of other fluctuating symptoms which were not relieved by surger

The development of endolymphatic hydrops following CMV inoculation of the endolymphatic sac.

The effect of inoculation of cytomegalovirus (CMV) into the endolymphatic sac was examined in CMV-seronegative and seropositive animals. Seronegative animals developed hearing loss, infection in the epithelial cells of the endolymphatic sac and perisaccular connective tissue, and endolymphatic hydrops. Control animals inoculated with inactivated CMV showed no hearing loss, viral infection, or endolymphatic hydrops. Seropositive animals showed complete protection from hearing loss and viral infection, yet a monocytic infiltrate was seen surrounding the endolymphatic sac. Associated with this lo

Characterization of endolymphatic sac tumors and von Hippel-Lindau disease in the International Endolymphatic Sac Tumor Registry.

BACKGROUND: Endolymphatic sac tumors (ELSTs) are, with a prevalence of up to 16%, a component of von Hippel-Lindau (VHL) disease. Data from international registries regarding heritable fraction and characteristics, germline VHL mutation frequency, and prevalence are lacking. METHODS: Systematic registration of ELSTs from international centers of otorhinolaryngology and from multidisciplinary VHL centers' registries was performed. Molecular genetic analyses of the VHL gene were offered to all patients. RESULTS: Our population-based registry comprised 93 patients with ELST and 1789 patients wi

Endolymphatic Sac Tumors With Aberrant Immunophenotypes.

Endolymphatic sac tumor (ELST) is a rare, locally destructive, but non-metastasizing neoplasm arising from the intraosseous epithelium of the endolymphatic duct and sac. Morphologically and immunohistochemically, ELST overlaps with metastatic renal cell carcinoma (RCC), and both neoplasms occur with increased frequency in patients with von Hippel-Lindau (VHL) disease. The largest study to date comparing the immunophenotypes of ELST and RCC found that 100% of ELSTs (25/25) were positive for CA9, whereas all tested ELSTs were negative for CD10 (0/16) and RCC (0/15). They concluded that strong ex

Endolymphatic Sac Tumor in Two Dogs.

Endolymphatic sac tumors (ELSTs) are rare neoplasms of the inner and middle ear described in humans. Diagnosis of such neoplasms is difficult and largely dependent on a combination of histologic, immunohistochemical, and clinical findings. Although the neoplastic cells lack cellular features of malignancy, these are clinically aggressive tumors that often invade the surrounding temporal bone. Here, we describe 2 dogs with middle ear masses that share morphologic, immunohistochemical, and clinical similarities with human ELSTs. Advanced imaging of the masses revealed evidence of aggressive beha

Endolymphatic sac enhancement: reversal of pathogenesis.

As this article and many other publications have indicated, many thousands of patients with incapacitating intractable progressive Meniere's disease have had their lives restored through endolymphatic sac enhancement surgery. This is the only nondestructive procedure for Meniere's disease that has stood the test of time. Destructive procedures such as labyrinthectomy (chemical or physical) and vestibular nerve section are reserved for rare cases of failure or recurrence after sac enhancement, despite revision of the enhancement. There is much evidence to support the concept that endolymphatic

Audiometric results following endolymphatic sac surgery.

Decompression and drainage of the endolymphatic sac is a surgical procedure which was developed to relieve vertigo and hopefully to maintain or improve hearing in patients with Meniere's disease. In this study, 16 medically diagnosed Meniere's disease patients who underwent endolymphatic sac surgery were followed audiometrically for a period of up to three years. Pre- and postoperative audiometric and other symptomatic findings were reported. Results indicated that in the greatest majority of cases hearing remained essentially unchanged or was improved. Poorer hearing was found in 26.3% of the

The proteome of the human endolymphatic sac endolymph.

The endolymphatic sac (ES) is the third part of the inner ear, along with the cochlea and vestibular apparatus. A refined sampling technique was developed to analyse the proteomics of ES endolymph. With a tailored solid phase micro-extraction probe, five ES endolymph samples were collected, and six sac tissue biopsies were obtained in patients undergoing trans-labyrinthine surgery for sporadic vestibular schwannoma. The samples were analysed using nano-liquid chromatography-tandem mass spectrometry (nLC-MS/MS) to identify the total number of proteins. Pathway identification regarding molecular

Dissection of the Endolymphatic Sac from Mice.

The study of mutant mouse models of human hearing and balance disorders has unraveled many structural and functional changes which may contribute to the human phenotypes. Although important progress has been done in the understanding of the development and function of the neurosensory epithelia of the cochlea and vestibula, limited knowledge is available regarding the development, cellular composition, molecular pathways and functional characteristics of the endolymphatic sac. This is, in large part, due to the difficulty of visualizing and microdissecting this tissue, which is an epithelium c

Aquaporin-2 in the human endolymphatic sac.

OBJECTIVE: To detect and localize aquaporin-2 (AQP-2), a water channel regulated by the antidiuretic hormone, in human endolymphatic sac. MATERIAL AND METHODS: Human endolymphatic sacs were sampled during removal of vestibular schwannomas via a translabyrinthine approach. Samples were immediately fixed in 10% formalin (24 h) and embedded in paraffin; in situ hybridization and immunohistochemistry were performed with an AQP-2-specific probe and a polyclonal antibody. RESULTS: Both AQP-2 mRNA and protein were detected in the epithelium of the endolymphatic sac. AQP-2 immunostaining was mainly

Detection of viral antigen in the endolymphatic sac.

A study was devised to determine whether or not any immune defense mechanism is present when a virus invades the human endolymphatic sac (ES). The ES was removed from 14 fresh autopsy cases having no known pre-mortem diseases in the middle and inner ears. Specimens were then examined for viral antigens including herpes simplex (HSV) type 1 and 2, mumps and cytomegalovirus using immunohistochemical methods. DNA examination by in situ hybridization was also performed for HSV. HSV antigen and DNA were observed in 9 of the 14 cases studied. These findings suggest that the virus invades the ES but
