Surgical bypass procedures to facilitate endovascular repair of aortic arch pathology.

Thoracic aortic aneurysms and other thoracic aortic lesions may become life-threatening conditions if they remain untreated. Conventional op … encouraging early and mid-term results, although the long-term durability of these hybrid surgical-endovascular procedures remains to be defined.

Endovascular experience at an academic office-based procedure center.

OBJECTIVE: The number of office-based procedure centers with the capability of performing a wide range of endovascular procedures has substantially increased over the past decade. This shift in practice settings has occurred faster in the private sector as compared to the academic environment. The purpose of our study was to evaluate the clinical outcomes of endovascular procedures performed at a dedicated academic outpatient procedural center. METHODS: We reviewed the clinical data of 400 patients who underwent 499 endovascular procedures in a university-based, academic outpatient procedure …

Iatrogenic pericallosal artery aneurysm after endovascular procedure.

Iatrogenic brain aneurysms are rare and are usually a result of direct trauma to the arterial wall during various neurosurgical or endovascular procedures. Single cases of pericallosal posttraumatic and postsurgical aneurysms were previously reported. Herein, we report the first confirmed case of iatrogenic pericallosal artery aneurysm caused by an arterial wall injury during the endovascular treatment of another aneurysm. A 50-year-old woman with ruptured anterior communicating artery aneurysm was initially treated with balloon-assisted coiling. During the procedure the tip of a microwire ins…

Hufnagel procedure reconverted by endovascular leaflet dislocation and extirpation.

Before the development of standard aortic valve replacement, Hufnagel treated aortic insufficiency by implanting a ball-valve prosthesis in the descending thoracic aorta. In a patient who, after four major thoracic procedures, ultimately received two mechanical bileaflet valves in series (one in the ascending and one in the descending aorta), the downstream prosthesis became progressively immobilized, with total blockage of the leaflets in the open position due to insufficient transvalvular negative pressure gradient. After evaluation of this particular situation in an experimental model, we p

Digital twin and artificial intelligence technologies for predictive planning of endovascular procedures.

Current planning of aortic and peripheral endovascular procedures is based largely on manual measurements performed from the 3-dimensional reconstruction of preoperative computed tomography scans. Assessment of device behavior inside patient anatomy is often difficult, and available tools, such as 3-dimensional-printed models, have several limitations. Digital twin (DT) technology has been used successfully in automotive and aerospace industries and applied recently to endovascular aortic aneurysm repair. Artificial intelligence allows the treatment of large amounts of data, and its use in med…

Endovascular Technique for Ascending Aorta Repair Based on TEVAR and TAVI Procedures.

PURPOSE: Ascending aorta (AAo) acute pathology still has an open-surgery indication with a high mortality rate associated to cardiopulmonary bypass and circulatory arrest. In these cases, the endovascular aortic approach could be an excellent option. The aim of the present study is to detail an optimized technique for the endovascular treatment of AAo diseases, based on thoracic endovascular aortic repair (TEVAR) and transcatheter aortic valve implantation (TAVI) procedures. TECHNIQUE: The procedure implies the usual preparation for TEVAR and TAVI implants. A transient pacemaker lead is neces

The ascending pharyngeal artery and its relevance for neurosurgical and endovascular procedures.

OBJECTIVE: The ascending pharyngeal artery (APA), a branch of the external carotid artery (ECA), supplies the lower cranial nerves, superior cervical ganglion, and nasopharyngeal structures. The APA can also supply blood to various intracranial lesions. We studied the anatomy of the APA in the context of its neurosurgical and endovascular relevance. METHODS: The cervical origin, branching pattern, and course of the APA were studied in 20 human cadaveric craniocervical sides. The diameter of the APA, the extension of its main trunk, and the distance of its origin from the common carotid artery

Preoperative Evaluation and Endovascular Procedure of Intraoperative Aneurysm Rupture During Thoracic Endovascular Aortic Repair.

UNLABELLED: Thoracic aortic aneurysms are now routinely repaired with endovascular repair if anatomically feasible because of advantages in safety and recovery. However, intraoperative aneurysm rupture is a severe complication which may have an adverse effect on the outcome of treatment. Comprehensive preoperative assessment and considerate treatment are keys to success of endovascular aneurysm repair, especially during unexpected circumstances. Few cases have reported on intraoperative aortic rupture, which were successfully managed by endovascular treatment. Here, we present a rare case of a

Anticoagulation for heparin-induced thrombocytopenia before, during, and after peripheral endovascular procedures.

Acute heparin-induced thrombocytopenia (HIT) type II requires the immediate discontinuation of heparin and initiation of a non-heparin anticoagulant. Lifelong avoidance of heparin is generally recommended due to the risk of recurrence. Therefore, anticoagulation management in endovascular procedures remains challenging, as heparin is the preferred anticoagulant and evidence on alternative strategies is limited. This narrative review outlines an institutional approach aligned with current guidelines and examines the available data on non-heparin anticoagulants in this setting. Current evidence …

Graduating vascular surgery trainee proficiency in endovascular and open peripheral revascularization procedures.

BACKGROUND: Endovascular interventions for peripheral artery disease have increased in prevalence over time given the inherent benefits of minimally invasive approaches. Although it is essential that vascular surgery graduates are facile with endovascular techniques, the results of the BEST-CLI (Best Endovascular vs. Best Surgical Therapy in Patients With Critical Limb Ischemia) trial highlight the equivalent importance of ensuring trainee competence in open skills. Recent studies demonstrate increasing case volume of both endovascular and open procedures during vascular surgery training. Case

Safety of adjuvant open axillary access during complex thoracoabdominal aortic endovascular procedures.

BACKGROUND: Endovascular treatment of aortic diseases with complex anatomy may require an additional upper extremity arterial access (like axillary access) for support during aortic navigation and allowing the use of larger sheaths for thoracoabdominal antegrade access. The objective of this study is to evaluate the safety of the open axillary approach as adjuvant access in complex thoracoabdominal aortic endovascular interventions. METHODS: A monocentric retrospective study was performed, including all patients with a complex aortic endovascular procedure (fEVAR, bEVAR, chEVAR or TEVAR), ele

Neuroprotective role of neurophysiological monitoring during endovascular procedures in the spinal cord.

The endovascular treatment of spinal vascular malformations places the spinal cord at risk for ischemia. When these procedures are performed … l artery territory than do somatosensory evoked potentials (SEPs), although their use during spinal endovascular procedures remains anecdotal in the literature. In the study reported here we assessed: (1) the feasibility of intraoperative neurophysiological monitoring, (2) the role of provocative tests with Amytal and Xylocaine, and (3) the specific but complementary rol …

Analysis of procedural performance after a pilot course on endovascular training for resuscitative endovascular balloon occlusion of the aorta.

PURPOSE: As resuscitative endovascular balloon occlusion of the aorta (REBOA) is performed in an extremely emergent situation, achieving competent clinical practice is mandatory. Although there are several educational courses that teach the REBOA procedure, there have been no reports evaluating the impact of training on clinical practice. Therefore, this study is aimed to evaluate the effects of the course on procedural performance during resuscitation and on clinical outcomes. METHODS: Patients who were managed at a regional trauma center in Dankook University Hospital from August 2016 to Fe

Current state and future directions of endovascular ascending and arch repairs: The motion towards an endovascular Bentall procedure.

Endovascular repair of the ascending aorta and aortic arch has evolved at an astonishing pace in the past several decades. Results of endovascular arch repair in experienced centers have been improving and the technology evolving, and it has begun to challenge the current gold standard status of open surgery in some groups of patients. Hybrid strategies with adjunctive cervical debranching for distal arch lesions are being replaced by fenestrated arch repairs. Total endovascular repair for proximal aortic arch pathologies with the use of inner branches has achieved the best results; however, t

Nexfin versus conventional oscillometric cuff for blood pressure monitoring during neuro-endovascular procedures.

BACKGROUND: Nexfin, a noninvasive blood pressure (BP) monitoring system, can provide continuous real-time measurements of BP, which can be useful for patients with acute neurological injury who require urgent neuroendovascular interventions. While Nexfin has shown promising reliability for other clinical scenarios, its performance has not been demonstrated during neuroendovascular procedures for patients with neurological injury. METHODS: This was a single-center retrospective study. Consecutive patients who underwent neuro-endovascular procedures with Nexfin BP monitoring were identified at …
